Monitoring cancer antigen 125 (CA-125) serum levels is very useful for determining how epithelial ovarian cancer is responding to treatment and for predicting a patient`s prognosis after treatment. An increase in CA-125 levels within patients in a remission is a strong predictor of the recurrence of ovarian cancer. However, while CA-125 is best known as a tumor marker for ovarian cancer, it may also be elevated in other various malignancies and in some benign conditions such as peritonitis, pancreatitis, chronic liver diseases, pelvic inflammatory diseases, or endometriosis. We describe herein a case of solitary intra-abdominal tuberculous lymphadenopathy mimicking a local relapse of epithelial ovarian cancer. A 51-year-old woman presented with progressive rise in serum CA-125. She had been diagnosed with advanced epithelial ovarian malignancy and undergone debulking surgery about 4 years ago. She had been given platinum-based adjuvant chemotherapy and achieved complete remission (CR). During regular follow-up in CR, a progressive rise in serum CA-125 was detected. The abdominal computed tomography (CT) showed a cystic mass, 3.6 cm in diameter, located adjacent to the right common iliac artery, suggesting a metastatic lymph node. We had planned second-look operation in settings of suspected focal recurrence of ovarian cancer, supported by evidence of rising CA-125 levels. However, the suspected tuberculous lesion was detected incidentally on preoperative chest CT. The patient had no symptoms and signs associated with tuberculosis (TB) and chest X-ray was also normal. We postponed the planned second-look operation and anti-TB medication was started. Mycobacterium TB was identified on culture of sputum specimens later. During anti-TB medication, the suspected metastatic isolated intra-abdominal lymphadenopathy improved and serum CA-125 level was also normalized. We emphasize here to consider not only the recurrence or progression of ovarian cancer itself but also benign conditions including TB in their differential diagnoses, when serum CA-125 level increases during the follow-up of epithelial ovarian cancer patients.

Objective This study evaluated the clinical effectiveness of Minds.NAVI, a depression screening kit combining psychometric measures and stress hormone biomarkers, in a prospective clinical trial. The objective was to assess its potential as a depression screening tool and investigate the associations between psychological assessments, salivary hormone staging, and depression severity.Methods Thirty-five participants with major depressive disorder and 12 healthy controls (HCs) were included. The Minds.NAVI software, utilizing the PROtective and Vulnerable factors battEry Test (PROVE) and salivary cortisol/dehydroepiandrosterone (DHEA) analysis, was employed. The PROVE test is a comprehensive self-report questionnaire that assesses depressive symptoms, suicide risk, attachment style, adverse childhood experiences, mentalization capacity, and resilience. In addition, salivary cortisol and DHEA levels were measured to evaluate the functional stage of the hypothalamic–pituitary–adrenal (HPA) axis.Results Minds.NAVI exhibited 100% sensitivity, 91.7% specificity, and 97.9% accuracy in distinguishing depression from HCs within an exploratory small group. Salivary stress hormone phases showed changes with depression stage (p=0.030), and the proportion of patients with “adrenal exhaustion stage” was higher in the moderate/severe depression group (p=0.038). Protective/vulnerable factors differed significantly between controls and depressed groups (p<0.001). Cortisol awakening response inversely correlated with depressive symptom severity (r=-0.31, p=0.034).Conclusion This study suggested possible clinical effectiveness of Minds.NAVI, a depression screening tool that integrates psychometric measures and stress hormone biomarkers. The findings support the potential association between depression, chronic stress, and HPA axis hyporesponsiveness.
<i>In Vitro</i> and <i>In Vivo</i> Assessment of FK506 Analogs as Novel Antifungal Drug Candidates

<P>FK506 (tacrolimus) is an FDA-approved immunosuppressant indicated for the prevention of allograft rejections in patients undergoing organ transplants. In mammals, FK506 inhibits the calcineurin-nuclear factor of activated T cells (NFAT) pathway to prevent T-cell proliferation by forming a ternary complex with its binding protein, FKBP12, and calcineurin. FK506 also exerts antifungal activity by inhibiting calcineurin, which is essential for the virulence of human-pathogenic fungi. Nevertheless, FK506 cannot be used directly as an antifungal drug due to its immunosuppressive action. In this study, we analyzed the cytotoxicity, immunosuppressive activity, and antifungal activity of four FK506 analogs, 31-O-demethyl-FK506, 9-deoxo-FK506, 9-deoxo-31-O-demethyl-FK506, and 9-deoxo-prolyl-FK506, in comparison with that of FK506. The four FK506 analogs generally possessed lower cytotoxicity and immunosuppressive activity than FK506. The FK506 analogs, except for 9-deoxo-prolyl-FK506, had strong antifungal activity against Ctyptococcus neoformans and Candida albicans, which are two major invasive pathogenic yeasts, due to the inhibition of the calcineurin pathway. Furthermore, the FK506 analogs, except for 9-deoxo-prolyl-FK506, had strong antifungal activity against the invasive filamentous fungus Aspergillus fumigatus. Notably, 9-deoxo-31-O-demethyl-FK506 and 31-O-demethyl-FK506 exhibited robust synergistic antifungal activity with fluconazole, similar to FK506. Considering the antifungal efficacy, cytotoxicity, immunosuppressive activity, and synergistic effect with commercial antifungal drugs, we selected 9-deoxo-31-O-demethyl-FK506 for further evaluation of its in vivo antifungal efficacy in a murine model of systemic cryptococcosis. Although 9-deoxo-31-O-demethyl-FK506 alone was not sufficient to treat the cryptococcal infection, when it was used in combination with fluconazole, it significantly extended the survival of C. neoformans-infected mice, confirming the synergistic in vivo antifungal efficacy between these two agents.</P>

<P>Harvesting water from humid air via dewing can provide a viable solution to a water shortage problem where liquid-phase water is not available. Here we experimentally quantify the effects of wettability and geometry of the condensation substrate on the water harvest efficiency. Uniformly hydrophilic surfaces are found to exhibit higher rates of water condensation and collection than surfaces with lower wettability. This is in contrast to a fog basking method where the most efficient surface consists of hydrophilic islands surrounded by hydrophobic background. A thin drainage path in the lower portion of the condensation substrate is revealed to greatly enhance the water collection efficiency. The optimal surface conditions found in this work can be used to design a practical device that harvests water as its biological counterpart, a green tree frog, Litoria caerulea, does during the dry season in tropical northern Australia.</P><P><B>Graphic Abstract</B> <IMG SRC='http://pubs.acs.org/appl/literatum/publisher/achs/journals/content/langd5/2012/langd5.2012.28.issue-27/la3013987/production/images/medium/la-2012-013987_0006.gif'></P>
Production of External Quality Assessment Materials for Anti-Müllerian Hormone Assay

Background: Anti-Müllerian hormone (AMH) is regarded as a sensitive and specific biomarker for assessing the ovarian reserve and has gained widespread clinical use. However, discrepancies between methods, different reference values, and inter-laboratory variation complicate the interpretation of AMH values and their clinical implications. To resolve these problems, external quality assessment (EQA) is important for standardizing AMH assays, thereby ensuring accurate and precise results. To conduct an EQA program, a large amount of quality control (QC) material is required. This study aimed to produce AMH QC materials for an EQA program and evaluate their homogeneity and stability. Methods: QC materials for three different concentrations of AMH were produced from the collected remnant sera of 632 patients in whom AMH assays were performed. These materials were evaluated for homogeneity between vials and short-term stability over 7 days at three different storage conditions. Results: The total coefficient of variation of the AMH QC materials at the three different concentrations ranged 2.09%–2.48%. No significant inhomogeneity was noted between vials; therefore, the samples were considered to be homogenous. With respect to short-term stability, three levels of AMH QC materials were found to be stable for at least 7 days when refrigerated or frozen condition. Conclusions: The AMH QC materials produced from remnant sera were found to be homogenous between vials and remained stable in a refrigerated and frozen condition for at least 7 days. The findings of this study may be practically applied for producing AMH EQA materials.
