      • GO-32 : Therapeutic Targeting of C-MET in Ovarian Clear-Cell Carcinoma

        목적: Recent reports revealed that c-Met activation is specifically associated with epithelial ovarian carcinoma and is association with poor prognosis. In this study, we investigated c-Met expression and the effects of its therapeutic targeting in ovarian clear-cell carcinoma (CCC). 방법: Expression levels of c-Met in the CCCs, serous carcinoma and normal ovarian tissues were evaluated using real-time PCR. To test c-Met inhibitors in cell lines including ES2, RMG1, RMG2, OVISE and OVSAHO, we performed in vitro experiment including MTT and apoptosis assay. We performed Western blots to evaluate the c-Met expression and down-stream pathway. Moreover, we performed the in vivo therapy experiment in patient-derived xenograft models (PDX, Avatar) of ovarian CCC to confirm these effects. 결과: The c-Met expression was significantly increased in CCCs compared with serous carcinoma and normal ovarian tissues (p<0.05). When we treated c-Met inhibitor (SU11274, Crizotinib) to ovarian CCC cells, cell viability was significantly decreased and apoptosis was significantly increased. Western blot assay demonstrated that the protein expressions for c-Met signaling pathway were decreased by c-Met inhibitors. Moreover, we found that c-Met inhibitor (SU11274) had significant decreased tumor weight compared with control in a case of PDX (Avatar) models for ovarian CCC. 결론: These results revealed that c-Met inhibitor have significant anti-tumor effects in ovarian CCC, and suggested that c-Met may have a potentials of therapeutic target for ovarian CCCs.

      • GO-50 : Correlation between location of transposed ovary and function in patients with cervical cancer who underwent radical hysterectomy

        To investigate the association between the location of transposed ovaries and ovarian function in patients with early cervical cancer (IB1-IIA). A retrospective study was performed with cervical cancer patients who underwent radical hysterectomy with ovarian transposition at Samsung Medical Center between July 1995 and July 2012. Ovarian function was estimated using serum FSH levels. We identified 31 patients and 21 patients were enrolled in this study. The median age and body mass index (BMI) were 31 years (range, 24 - 39) and 21.3 kg/m2 (range, 17.7 - 31.2). The median serum FSH level was 7.9 mlU/ml (range, 2.4 - 143.4). The median location of transposed ovary based on iliac crest was 0.5 cm (range, -2.7 - 5.2). As the location of transposed ovary heightened, serum FSH and E2 showed negative (r = -0.151, p =0.514) and positive correlation (r = 0.337, p=0.135). In the multivariate analysis, transposed ovarian function, which was assessed by serum FSH levels, was significantly associated with location of transposed ovary (β = -8.1, p=0.032), concurrent chemoradiotherapy (CCRT) (β = 71.08, p=0.006) and BMI (underweight; β = -59.93, p=0.05, overweight; β = -40.62, p=0.041). There was no significant association between transposed ovarian function and age. Location of transposed ovary, postoperative adjuvant CCRT and BMI are associated with ovarian function. We suggests that ovary should be transposed as highly as possible for preservation of the ovarian function.

      • GO-13 : Prognostic factors and outcomes in endometrial stromal sarcoma with the 2009 FIGO staging system; A multicenter review of 114 cases

        To assess the prognostic factors associated with disease-related survival in endometrial stromal sarcoma (ESS), to which the 2009 FIGO staging system was applied. From January 1990 to January 2012, 114 patients with ESS were identified at the Samsung and Asan Medical Center and data were retrospectively analyzed. Ten (8.7%) patients died of the disease and 33 (28.9%) patients relapsed. The 5- and 10-year overall survival (OS) rates for the entire cohort was 92.6% and 87.1%, and the 5- and 10-year recurrence-free survival (RFS) rates were 71.8% and 52.1%. The estimated median survival after recurrence of 33 patients was 133 months (95% CI, 7.7 - 258.4), and 5-year survival after recurrence was 68.9%, retrospectively. Stage I (p=0.006), positive of estrogen and/or progesterone receptor (ER/PR) (p=0.0027) and no nodal metastasis (p=0.033) were associated with a good prognosis for OS in the univariate analysis. Ovarian preservation was revealed to be an independent predictor for poorer RFS (HR, 6.5; 95% CI, 1.23 - 34.19; p=0.027). Positivity of ER/PR (HR, 0.05; 95% CI, 0.006 - 0.4; p=0.006) and cytoreductive resection of recurred lesions (HR, 0.14; 95% CI, 0.02 - 0.93; p=0.042) were independent predictors for better survival after recurrence. Stage, expression of ER/PR and nodal metastasis are significantly associated with OS in ESS. Bilateral salpingo-oophorectomy (BSO) as the primary treatment and cytoreductive resection of recurred lesions should be considered for improving survival of patients with ESS.

      • Evaluation of New FIGO Staging System for Endometrial Stromal Sarcoma of the Uterus

        The aim of this study is to detect differences in overall survival between the 1988 FIGO staging and revised 2008 FIGO staging of uterine endometrial stromal sarcoma. The aim of this study is to detect differences in overall survival between the 1988 FIGO staging and revised 2008 FIGO staging of uterine endometrial stromal sarcoma. Of 109 patients with LGESS, 86 patients (78.9%) had stage I disease, 5 patients (4.6%) had stage II disease, 9 patients (8.3%) were diagnosed as stage III, and 9 patients (8.3%) had stage IV disease by 1988 FIGO staging system. According to revised 2008 FIGO staging system, 8 patients (7.3%) were downstaged; 3 patients were downstaged from stage II to I and 5 patients were downstaged from stage III to II respectively. The difference in median survival of 4 stages (I-IV) was statistically significant using both staging system (1998 vs. 2008 FIGO staging system, p=0.006 vs. p=0.013). Comparing the median survival of 2 subgroups, both staging system had significant difference but previous staging system had more accurate discriminability (1998 vs. 2008, p=0.006 vs. p=0.019, HR 5.470 vs.4.286). Based upon our analysis of 109 patients with uterine LGESS, the revised FIGO staging system was appropriate to predict survival.

        Background: Macrophage Migration Inhibitory Factor (MIF) is known as pro-in- fi ammatory and pro-oncogenic cytokine. Its role in gastric cancer has been revealed recently. We evaluated the relationship between MIF and H. pylori in the gastric carcinogenesis. Methods: Human gastric tissue samples from 522 patients were subdivided into 6 groups according to H. pylori infection status and pathologic diagnosis (cancer, dysplasia, or control). Tissue MIF mRNA level was measured by real-time PCR, shown as 2^delta-delta CT (2^ddCT). Results: Gastric cancer tissue expressed signifi cantly higher level of MIF mRNA (n=214, mean±SE; 9.16±1.5) than non-cancer counterpart (n=308, 4.22±0.9, p=0.005). There was no signifi cant difference in MIF mRNA level between H. pylori negative and positive tissues. In subgroup analysis, H. pylori-positive cancer tissue showed signifi cantly higher MIF expression (n=137, 10.27±2.1) than that of H. pylori-positive dysplasia (n=55, 1.91±0.5, p<0.05) or H. pylori-positive control (n=113, 3.29±0.6, p<0.05). However, no such fi nding was noted among the H. pylori-negative cancer/dysplasia/control group. (fi gure) Conclusions: Tissue MIF mRNA level showed signifi cant difference between gastric cancer and dysplasia only in H. pylori-positive group. From these results we can conclude that MIF could be an important factor in the H. pylori induced gastric carcinogenesis. Now the experiments regarding underlying mechanism are undergoing.

        Background: To evaluate the effi cacy of Helicobacter pylori ( H. pylori) eradication between a personalized therapy based on the results of antibiotics resistance by using H. pylori culture and minimal inhibitory concentration (MIC) and the traditional 2nd rescue regimens. In addition, we analyzed the prevalence of the antibiotic resistance after 1st eradication of H. pylori in the tailored therapy group. Methods: The patients who had shown the evidence of persistent H. pylori infection after the 1st eradication were prospectively enrolled. If the patients chose the tailored therapy, culture and MIC tests were undergone. Two regimens are as following: 14 day PBMT (Proton pump inhibitor (PPI) regular dose b.i.d., tripotassium dicitrate bismuthate 300 mg q.i.d., metronidazole 500 mg t.i.d., and tetracycline 500 mg q.i.d.) and 14 day MEA (moxifi oxacin 400 mg q.d., esomeprazole 40 mg dose b.i.d., and amoxicillin 1g b.i.d.)]. For analyzing the antibiotic susceptibility of H. pylori, microbiological culture and MIC of amoxicillin, clarithromycin, metronidazole, tetracycline, azithromycin, ciprofi oxacin, levofi oxacin, and moxifi oxacin according to the agar dilution method was used. Results: 108, 47, and 40 consecutive patients were enrolled in the PBMT, MEA and tailored therapy group, respectively. The intention-to-treat and per-protocol eradication rates (ITT/PP) were 75.0%/84.4% in PBMT group, 66.0%/73.8% in the MEA group, and 85.0%/100.0% in the tailored therapy group (PP: p=0.006) (figure). Analyzing the antibiotic resistance in the tailored therapy group, 27 patients (70.0%) showed multidrug resistance of H. pylori isolates (Table). Conclusions: Tailored therapy based on MIC showed a superior eradication rate (100.0% on the PP analysis, p=0.006) compared with traditional 2nd rescue therapy. Even there is a diffi culty of clinical practice tailored therapy might necessary choice to deal with the increase of antimicrobial resistance.

      • Alteration in Lipid and Protein Profiles of Ovarian Cancer: Similarity to Breast Cancer

        <B>Abstract</B><P>This study was undertaken to evaluate protein and lipid profiles of ovarian cancer tissue samples. Twenty-three frozen ovarian cancer samples and 6 adjacent normal samples were analyzed using histology-directed, matrix-assisted laser desorption/ionization mass spectrometry. Sinapinic acid and 2, 5-dihydroxybenzoic acid/α-cyano-4-hydroxycinnamic acid were manually deposited on areas of each tissue section enriched in epithelial cells to identify protein and lipid profiles respectively, and mass spectra were acquired using a matrix-assisted laser desorption/ionization-time of flight instrument. Protein and lipid profiles classify 11 cancer and 3 adjacent normal samples in 100 random test sets with 92.9% median accuracy. Phosphatidylcholines {32:3} [M + Na]<SUP>+</SUP> (<I>m</I>/<I>z</I> = 750.66), {34:1} [M + K]<SUP>+</SUP> (<I>m</I>/<I>z</I> = 798.60), and {36:2} [M + K]<SUP>+</SUP> (<I>m</I>/<I>z</I> = 824.56) were found to be increased in ovarian cancer. Interestingly, breast cancer-associated changes in lipid and protein profiles were also found in ovarian cancer. Thus, protein and lipid profiles accurately distinguish ovarian cancer from adjacent normal tissue samples. Common cancer-associated alterations in lipid and protein profiles were identified between ovarian and breast cancers.</P>

        Supplementation Effect of Onion Peel Extracts on Small Intestine of Obese Mice

        본 연구는 양파껍질을 발효주정 에탄올 70%와 95% 농도로 추출하여 항균활성, 항산화활성 및 비만쥐의 소화기관에 미치는 영향에 대하여 알아보고자 실시하였다. 각 주정 추출물 5%는 E. coli와 Listeria monocytogens에서 유의적인 감소를 나타내었다. 각 주정 추출물 내 총 페놀함량은 각각 166.89±0.03 mg/g과 160.89±0.13 mg/g을 나타내었다. 항산화효과를 살펴본 결과 DPPH 라디칼 소거능과 SOD 유사 활성은 100 ug/ml의 농도에서 높은수준을 나타내었다. 고지방식이를 급여하여 유도한 비만을 유도한 마우스에 70%와 95% 주정으로 추출한 양파껍질 추출물을 1, 3, 5%의 농도로 4주간 급여한 후 체중, 사료섭취량, 사료효율, 소장무게, 길이 장내 융모의 길이, 소장 내 미생물수를 측정하였다. 각 주정으로 추출한 양파 추출물 5%의 농도로 급여하였을 때 대조군에 비해 체중이 유의적으로 낮은 수준을 나타내었으나(p<0.05) 사료섭취량은 증가함을 나타내었다. 실험동물 마우스 소장의 무게와 길이는 양파 껍질추출물 급여에 따른 유의적인 변화는 보이지 않았으나 소장 융모의 길이는 대조군보다 유의적으로 증가함을 나타내었다. 총균수, Cl.perfringenes와 E.coli는 5%의 농도로 양파껍질 주정 추출물을 처리했을 때 유의적으로 감소함을 나타내었으나 젖산균은 증가함을 나타내었다. 이러한 결과는 추후 부가적인 추가 연구가 필요하겠지만 70%와 95%의 주정으로 추출한 양파껍질 추출물을 5% 수준으로 이용하면 체중조절 첨가제로서 그리고 장의 건강을 증진시키기 위한 첨가제로 사용할 수 있는 잠재성이 있는 것으로 기대된다. This study was performed to determine the effect of onion peel extract using 70% ethanol and 95% ethanol on its anti-oxidation activity, small intestine length, and intestinal villi of high-fat fed mice. Five percent of each onion peel extract using 70% and 95% ethanol showed significant decrease of E. coli and Listeria monocytogenes. Total phenolic contents of onion peel extracts using 70% and 95% ethanol were 166.89±0.03 mg/g and 160.89±0.13 mg/g, respectively. In anti-oxidation activity, DPPH radical scavenging activity and SOD-like activity of onion peel extracts were higher at 100 ug/ml concentration. The obese mice were fed high-fat diets supplemented by 1, 3, and 5% of the onion peel extracts using 70% and 95% ethanol for 4 weeks. Body weight, feed intake, feed efficiency, small intestine weight, length, villi’s length, and number of bacteria in intestine were determined. Body weight of mice fed 5% of onion extracts using both 70% and 95% ethanol was significantly lower than that of control (p<0.05). However, feed intake was increased in mice fed 5% of onion extracts at both fermented ethanol levels. Small intestine weight and length of mice showed no significant change with supplementation of the onion peel extracts. However, length of small intestine villi was significantly longer than that of control. Total bacteria counts of Cl. Perfringenes and E. coli in small intestine of the mice were significantly reduced by supplementation of 5% of onion extract using ethanol, while lactic acid bacteria were increased. These results suggest that 5% of onion peel extracts using ethanol at either 70% or 95% concentration have potential to be used as an additive for body weight control and enhanced gut health; however, more research on its effectiveness is needed.
