지리정보시스템(GIS)으로 분석한 대전광역시 응급의료서비스 취약지 특성

본 연구는 대전광역시의 응급의료서비스 취약지를 도출하고 취약지역의 보건학적 특성 및 응급의료서비스 취약여부와의 관련성을 분석하여 응급의료 관련 정책의 의사결정에 유용한 기초자료로 제공하기 위한 연구이다. 응급의료서비스 취약지 도출은 Arc GIS의 공간분석 방법 중 가중분석(Cost Weighted distance) 방법으로 응급의료센터로부터의 접근성 분석을 하였으며, 응급의료서비스 취약지의 보건학적 특성 및 응급의료서비스 취약여부와의 관련성은 SPSS 17.0을 이용하여 비모수 t-검정 및 다중회귀분석을 시행하였다. 본 연구의 주요 결과는 다음과 같다. 연구지역의 응급의료기관 분포는 동구와 유성구, 대덕구는 지정된 응급의료센터가 없으나 서구와 중구는 응급의료센터가 2개소 이상 위치하고 있어 응급의료기관 분포가 편중되어 있으며, GIS를 활용하여 응급의료센터와의 접근성 분석을 수행한 결과, 대전광역시 자치구별 전체 면적 대비 응급의료서비스 취약지의 비율이 높은 자치구는 동구가 41.2%로 가장 높았다. GIS를 활용하여 행정동별 응급의료서비스 취약지를 분석한 결과, 대덕구 신탄진동, 동구 대청동과 산내동, 유성구 구즉동과 노은2동, 서구 기성동, 중구 산성동으로 나타났으며, 응급의료서비스 취약지 중 기성동, 대청동이 노인 인구밀도가 높게 나타났다. 응급의료서비스 취약여부에 따른 보건학적 특성별 차이를 분석한 결과, 국민기초생활수급권자, 장애인등록자, 농업인구 비율의 평균은 취약지가 비취약지에 비해 높았으며 통계적으로 유의한 차이를 보였다(p<0.01). 응급의료서비스 취약여부를 종속변수로 하고 지역별 보건학적 특성을 독립변수로 하여 로지스틱 회귀분석을 시행한 결과, 농업인구 비율과 국민기초생활수급권자 비율이 높았으며 이는 통계적으로 유의하여 응급의료서비스 취약여부를 설명할 수 있는 변수인 것으로 나타났다(p<0.01, p<0.05). 이상의 결과를 종합하면 대전광역시 5개 자치구의 행정동 중 응급의료서비스 접근 불평등지역이 도출되었고 이러한 지역은 보건학적 특성 중 농업인구 비율과 국민기초생활수급권자의 비율이 높았으며 이는 통계적으로 유의하여 응급의료서비스 취약여부와 관련성이 있는 것으로 나타났다. 향후 효율적인 응급의료 자원 분배를 위해서는 GIS를 활용한 의사결정이 필요하며, 응급의료서비스 이용의 형평성을 증진시키기 위해서 응급의료서비스의 사각지대에 놓여있는 지역의 보건학적 특성을 고려한 정책이 시행되어야 할 것으로 사료된다.

일제강점기 신경쇠약 담론의 형성과 신경쇠약 매약의 확산

일제강점기의 상황에서 식민지 조선의 정신적 피로와 스트레스는 ‘신경쇠약’이라는 새로운 담론을 만들어냈다. 기존 조선 사회에서는 존재하지 않았던 질병명인 ‘신경쇠약’은 미국에서 시작되어 일본을 거쳐 조선에 들어오게 되었다. 본고에서는 기존의 사회에서는 존재하지 않던 새로운 질병이 어떠한 방식으로 외부에서 들어오고, 확산해 가는지에 대한 과정을 그려보고자 하였다. 일제강점기 조선 사회는 식민지 치하라는 시대적인 특수한 상황에서 ‘매약’의 열풍과 함께 매약 광고 시장이 매우 커지고 있는 상태였으며, ‘신경쇠약’과 ‘매약’ 역시 상호영향력을 주고받으면서 서로 거대한 담론을 형성해 나갔다. 신경쇠약은 매약과 매약 광고를 통해 사람들에게 널리 알려지게 된 부분이 있으며, 한편으로는 신경쇠약의 유행이 신경쇠약 매약의 공급과 광고 시장을 크게 만들었다고 할 수 있다. 한약으로 만든 신경쇠약 매약 광고를 통해서 한의학에서는 신경쇠약을 어떠한 방식으로 이해하고 받아들였는지, 어떻게 치료하고자 하였는지 알아보고자 한다. The psychological and emotional stress and fatigue that Joseon experienced as a colony during the Period of Japanese Occupation gave rise to the new discourse of Neurasthenia. A disease name that had never existed in Joseon, Neurasthenia originated in America and arrived through Japan to Joseon. This paper aims to describe how a disease that did not exist in Korean society previously entered the country and spread there. In Joseon during the Period of Japanese Occupation, the advertisement market for medicines for sale was getting big amidst the frenzy of medicines for sale under the extraordinary circumstances of the colonial rule. Neurasthenia and medicines for sale in turn built grand narratives through the exchange of their influences. Neurasthenia came to be known to the general public partly through the Medicines for sale and the advertisement of the Medicines for sale. On the other hand, the prevalence of Neurasthenia increased the supply of Medicines for sale and the advertisement market for them. This study will examine the advertisements for Neurasthenia medicines for sale that were made of Korean medicines to see how the Korean medicine understood and received Neurasthenia and how it tried to treat it.

우주환경 지상관측기 자료통합시스템 개발

We have developed a data integration system for ground-based space weather facilities in Korea Astronomy and Space Science Institute (KASI). The data integration system is necessary to analyze and use ground-based space weather data efficiently, and consists of a server system and data monitoring systems. The server system consists of servers such as data acquisition server or web server, and storage. The data monitoring systems include data collecting and processing applications and data display monitors. With the data integration system we operate the Space Weather Monitoring Lab (SWML) where real-time space weather data are displayed and our ground-based observing facilities are monitored. We expect that this data integration system will be used for the highly efficient processing and analysis of the current and future space weather data at KASI.

보장책임에 근거한 국가 관여의 정당성과 스마트 규제 : 민간투자를 중심으로

보장책임은 국가가 임무를 스스로 실현하지는 않지만 그 임무가 실현되도록 보장하는 것을 내용으로 한다. 보장국가 시대의 공행정은 과거 명령이나 금지 등을 통한 고권적이고 강제적인 형태와 달리, 부드러운 행정을 지향하고 규제 방식 또한 연성적이다. 이와 같이 연성적인 규제 방식을 스마트 규제(Smart Regulation)라고 한다. 그러나 민관협력에서의 국가 규제는 사기업에 대하여 행하는 것으로, 기업의 자율성을 침해할 수 있다는 문제가 제기될 수 있다. 우리 헌법은 사회적 시장경제질서를 채택하고 있고, 헌법 제119조 제2항에서 경제력의 남용을 방지하기 위한 국가의 경제에 대한 규제와 조정 권한을 규정하고 있다. 또한 역사적 접근과 비교법적 고찰의 측면에서 2008년 세계금융위기의 예에서도 국가의 개입은 정당성을 가진다. 즉, 세계 대공황 이후 금융 시스템의 안정을 위하여 제정된 글라스 스티걸 법을 폐지하고 탈규제를 한 결과, 금융 시스템의 붕괴를 초래하여 2008년 세계금융위기의 발단이 된 예에서 국가가 적절하게 규제를 할 필요성이 있다는 것을 보여준다. 민간투자제도에 있어서 사회의 자율적 임무수행이 잘 이루어지도록 관리 및 감독하는 방법으로는 다음의 두 가지를 고려할 수 있다. 첫째, 행정청이 민간투자사업시설에 대하여 관리자를 지정하는 방법을 고려해 볼 수 있다. 지정관리자 제도는 당해 공공시설의 관리책임을 행정청(일본의 경우 지방자치단체)에 유보하면서 관리 및 운영에 있어서의 민간사업자의 자율성을 최대한 확보하기 위한 제도이다. 둘째, 위법한 행위에 의하지 아니하더라도 공공의 이익을 위하여 행정청이 민간투자사업 계약을 해제할 수 있는 공익을 위한 처분을 들 수 있다. 이러한 공익을 위한 처분은 원칙적으로 대등한 입장에서 협약을 체결한 당사자들 간의 사업 계약에서 행정청이 고권적인 행정처분을 하는 것으로 민간투자사업의 취지와 거리가 있는 것으로 보인다. 그러나 국가는 민간투자사업이 목적에 맞게 잘 이루어지기 위하여 관리·감독할 책임을 지며 동시에 공익을 보호할 의무도 있기 때문이다. New Public Management means that administrative agency performs public service along with private sector and the State has responsibility that it should guarantee good quality of public service. This is so called Gewährleistungsstaat. Although the State doesn't control directly, the State leads and manages in order that Private Finance Initiative would perform well through self-regulation. In spite of the State's delegation of infrastructure management to the private actor, the fundamental legal base is not civil law principle, rather the State should act as the continuous supervisor. This sort of regulation is called as "Smart Regulation". Someone argues that Smart Regulation might violates the private enterprise's autonomy. But according to articles of Constitution of the Republic of Korea, the State's intervention is evaluated as legitimate. Because Constitution of the Republic of Korea is considered to have adopted Social Market Economy. According to article 119 (2), the State may regulate and coordinate economic affairs in order to maintain the balanced growth and stability of the national economy, to ensure proper distribution of income, to prevent the domination of the market and the abuse of economic power. In historical and comparative law view, especially after Global Financial Crisis in 2008, the State's intervention is considered legitimate. The Global Finance Crisis in 2008 is caused by abolition of Glass-steagall Act 1933 that had been made to stabilize financial system. This historical example shows that the State's intervention and regulation are necessary. The State can manage and supervise the private actor to perform self-regulating well. The following two ways can be considered. First, the State can consider whether to adopt Designated Management System. Designated Management System guarantees the private actor's autonomy in the management of infrastructure. But still public actor(in Japan, local government) has the responsibility of management. Second, The administrative disposition for public interest can terminate Private FInance Initiative contract, even though the private actor doesn't violate the law. In different perspective, this administrative disposition has contradiction, because Private Finance Initiative is contract between the public actor and the private actor as mutual contract parties. But the State has responsibility that it should manage and supervise Private Finance Initiative and should protect public interest.

사회과학분야 : 무용수업에 참여한 초등학생의 신체자아효능감, 무용활동만족, 학교생활만족의 관계

본 연구에서는 무용수업에 참여한 초등학생들의 신체자아효능감, 무용활동만족, 학교생활만족의 관계를 규명하기 위하여 J시와 D시에 위치하고 있는 초등학교에서 방과 후 무용수업에 참여하는 초등학생들을 대상으로 2013년 3월부터 5월까지 약 2개월에 걸쳐 총 300부를 배포하였으며, 그 중 신뢰성이 없거나 불성실한 자료 109부를 제외한 191부가 실제 분석에 사용되었다. 최종수집된 자료의 분석 방법은 탐색적 요인분석, 상관분석 및 다중회귀분석을 실시하여 나타난 결과를 기초로 본 연구에서는 다음과 같은 결론을 도출하였다 첫째, 무용수업에 참여한 초등학생들의 신체자아효능감의 하위변인인 인지된 신체 자신감은 무용활동만족의 하위변인인 외재적 만족도에 유의한 영향을 미치는 것으로 나타났다. 둘째, 무용활동만족도의 하위변인인 내재적 만족도는 학교생활만족의 하위변인인 학교행사에 유의한 영향을 미치고 무용활동만족의 외재적 만족도는 학교생활만족의 교우관계, 규칙준수, 학교행사에 영향을 미치는 것으로 나타났다. 셋째, 신체자아효능감의 인지된 신체 자신감은 학교생활만족의 하위변인인 교우관계, 규칙준수, 학교행사에 유의한 것으로 나타났으나 신체자아효능감의 하위변인인 외모타인인식은 학교생활만족의 교우관계에만 유의한 영향을 미치는 것으로 나타났다. 넷째, 무용활동만족은 신체자아효능감과 학교생활만족에서 매개효과를 가지는 것으로 나타났다. The present study aims to explore the interactions of and relationships among physical self-efficacy and dance activity satisfaction and school life satisfaction of elementary school students. For this aim, subjects were sampled out from those who attended dance class after school at elementary schools in J and D cities and 300 copies of questionnaires were distributed to them for 2 months from March to May in 2013. From among the returned replies, 191 ones were put to analysis after putting aside 109 which were deemed less reliable and not or double replied. SPSS 18.0 program was employed to examine the final data by means of frequency, exploratory factor, correlation and multiple regression analyses. From the findings of the analyses, following conclusions could be drawn: First, physical self-efficacy had positive effects on dance activity satisfaction. Second, dance activity satisfaction had positive effects on school life satisfaction. Third, physical self-efficacy had positive effects on school life satisfaction. Forth, dance activity satisfaction mediated between physical self-efficacy and school life satisfaction.

자하차약침(紫河車藥鍼)의 MIF 활성 억제를 통한 LPS 유발 류마티스성 관절염의 치료 효과

Objectives : This study is to evaluate Effect of Inhibition Macrophage Migration Inhibitory Factor(MIF) activation by Hominis Placenta Herbal Acupuncture(HPA) on Rheumatic Arthritis(RA). Hominis Placenta is the placenta of healthy human, which is vital-strengthening medical stuff. In recent years, Hominis Placenta applied to chronic diseases because it makes us more resistance to diseases. Therefore it is supposed that HPA is effective on RA, a kind of autoimmune disease. When RA is induced, MIF is activated, too. MIF affects the process of inflammatory disease including RA. Methods : In order to investigate the effect of Hominis Placenta extraction on MIF(early RA inducing cytokine) and MMP(Matrix Metallo Proteinase)-9 mRNA expression by means of Reverse Transcriptase- Polymerase Chain Reaction(RT-PCR). In this study, we investigated the effect of Hominis Placenta extraction on MIF(early RA inducing cytokine) and MMP-9 mRNA expression by means of RT-PCR. Besides we investigated changing of MIF in synovial membrane and, Interleukin-6 receptor(IL-6R)-$\alpha$(pro-inflammatory cytokine), Signal transducers and activators of transcription(STAT)-3, MMP-9 after treating mouse, which is artificially attacked with RA, with HPA on its $ST_{35}$, LE201 in vivo. Results : 1. As a result of treating Lipopolysaccharide(LPS)-stimulated Raw246.7cell with HPA, MIF(RA related cytokine) and MMP-9 mRNA expression is reduced in vitro. And this reaction is concentration-dependatant. 2. In synovial membrane of the mice treated with HPA, inhibition of MIF, IL-6R-$\alpha$, STAT3 & MMP-9 activation is observed in vivo. Conclusions : From the above results, it might be suggested that HPA mitigate tissue damage originated from RA, because it intercepts the early process of by inhibition MIF activity.

임상간호사의 월경태도, 월경전증후군, 스트레스가 소진에 미치는 영향

Purpose: The purpose of this study was to investigate factors which effect clinical nurses` exhaustion. Methods: This research was conducted targeting 140 clinical nurses. Data were collected from 18 September to 30 September 2015. Data were analyzed using the program SPSS/WIN 18.0. Descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and Stepwise multiple regression analysis were used. Results: Burnout was significantly different by age (F=7.99, p=.001), marital status (t=8.69, p=.004), department area (F=7.65, p<.001), frequency of night work in a month (F=7.65, p=<.001), and clinical career (F=3.68, p=.028). There was positive correlations between menstrual attitudes of participants and premenstrual syndrome (r=34, p<.001), menstrual attitudes and stress (r=.40, p<.001), and menstrual attitudes and burnout (r=.16, p=.031). There were positive correlations between premenstrual syndrome and stress (r=.46, p<.001), between premenstrual syndrome and burnout (r=.35, p<.001), and between stress and burnout (r=.36, p<.001). Factors influencing burnout were premenstrual syndrome (β=.335) and age (β=.216), with an explanatory power of 18.0%. Conclusion: There is a need to develop and apply program for reducing clinical nurses` pre-menstrual syndrome. In addition, policies are needed to allow for menstruation leave, thereby making it legal.
