Although Crohn's disease is usually found in the ileum and colon, it can be located in the whole gastrointestinal tract from the oral cavity to the anus. The frequency of gastroduodenal Crohn's disease is rare and is reported to range between 0.5% and 4.0% in Crohn's disease. And when Crohn's disease does involve the upper gastrointestinal tract, there is nearly always concomitant disease in the small bowel and colon. Very rarely, isolated Crohn's disease of the stomach and duodenum may occur. Definite diagnosis requires histologic confirmation, however, endoscopic biopsies often fail to reveal granuloma. Thus, if absence of definite histologic findings, combining clinical, radiologic and endoscopic findings suggest this condition. We experienced a first case of Crohn's disease confined to stomach and reported with review of the literature. (Korean J Gastrointest Endosc 2003;26:146-149) 크론병은 구강에서 항문에 이르기까지 전 위장관에 발생할 수 있으며 드물지만 전체 환자의 0.5~4%에서는 위 또는 십이지장을 침범하는 것으로 보고된다. 위 또는 십이지장의 크론병은 다른 위장관에 크론병이 있는 환자에서 함께 관찰되는 것이 보통이며 단독으로 발생하는 경우는 매우 드물다. 진단은 적합한 조직으로 얻은 병리학적 소견으로 이루어지나 내시경적 생검의 어려움으로 임상양상, 내시경, 조직학적 소견 및 방사선학적 소견 등을 토대로 유사한 다른 질환들을 배제한 후 진단을 내릴 수 있다. 저자들은 특징적인 육아종을 관찰할 수 없었으나 임상소견, 내시경 및 방사선 소견에 근거하여 진단된 크론병 1예를 경험하여 문헌고찰과 함께 보고한다.
