        장기간 Bisphosphonate계 약물 복용 후 발생한 비정형 대퇴 전자 하부 골절 및 골 생검

        Although alendronate was considered generally safe and effective to osteoporosis in short-long term studies, it remains the potential risk of severely suppressed bone turnover that causes insufficient fracture of femur. In this report, we describe a patient of atypical subtrochanteric fracture who treated with prolonged alendronate. A 68-years old woman who had a history of medication of alendronate for 12 years was presented to the outpatient clinic with right thigh pain. The radiographs revealed thickening and fracture line of the lateral side of subtrochanteric area of the right femur. She discontinued taking the medicine and follow up closely. After 1 month later, she sustained atypical subtrochanteric femoral fracture after simple fall, and underwent an internal fixation with compression hip screw. After operation,bone biopsy was done with tetracycline staining for valuation of bone metabolism and severe osteoporosis was observed. Although long-term use of bisphosphonate could increase bone mineral density, this case showed that it could lead insufficient fracture by decreasing bone strength through severely suppressed bone turnover, inhibition of normal bone formation and decreased bone quality. [Korean Journal of Bone Metabolism, 18(2): 131-135, 2011] 비스포스포네이트계 약물은 장단기간의 안정성과 효능은 입증되었으나, 장기간 사용시 지속적인 골 교체 과정의 억제로인한 부전 골절을 일으킨다는 보고들이 있다. 알렌드로네이트를 12년간 꾸준히 복용하고 있는 68세 여자 환자가 수개월전부터 발생한 우측 대퇴부 통증으로 내원하여 전자 하부 부전 골절로 진단 후, 알렌드로네이트의 투여를 중지하고 외래추시 1개월 만에 대퇴골 전자 하부의 전이성 비정형 골절로 수술적 치료를 시행받았다. Tetracyclin을 사용한 조골세포 표지 골 생검을 시행받았으며, 검사 소견상 심각한 골다공증이 의심되는 소견이 관찰되었다. 본 증례는 골다공증 치료를 위하여 장기간 사용한 비스포스포네이트가 골밀도는 증가시킬 수 있지만, 과도한 골 교체의 저하를 유발하며, 정상적인 골재형성을 억제하여 골질을 저하시킴으로 오히려 골 강도를 떨어뜨리고, 비정형 골절을 조장할 수 있다는 소견을 골 생검을 통하여 추정할 수 있었다.

        주위의 지인들로부터 골다공증에 대하여 여러 가지 질문을 받곤 한다. 골다공증이란 무엇이고, 그 원인은 무엇이며, 검사방법과 증상 등은 어떻게 되는지 자세히 묻는다. 하지만 정작 기대하는 것은 어떻게 하면 골다공증을 막을 수가 있는가에 대한 적절한 대답이다. 이에 대한 정답은 너무나 쉽고 명확하다.

        Cross-Calibration of Bone Mineral Densities and Body Composition between GE Lunar Prodigy and Osteosys Primus

        Background: The aim of this study was to investigate the correlation between bone mineral density (BMD) and body composition measured by the Osteosys Primus® and the GE Lunar Prodigy® and to calculate the conversion rate between the 2 devices. Methods: The 40 subjects were men and women in aged 20 to 29 years old. All participants were scanned twice on both the Osteosys Primus (OsteoSys) and the GE Lunar Prodigy (GE Healthcare) DXA systems using the manufacturers’ standard scanning and positioning protocols. Results: Compared to the GE Lunar device, the mean Osteosys fat mass was overestimated to be 12.1% (1,776.9 g) in the whole body, 5.1% (163.9 g) in gynoid, and 6.7% (87.2 g) in android. Compared with the GE Lunar device, the mean BMDs of the Osteosys Primus were underestimated to be 2.3% (0.023 g/cm2) in the whole body and 3.1% (0.035 g/cm2) in L1-4. Compared with the GE Lunar device, the mean lean mass derived by the Osteosys Primus were underestimated to 2.3% (1,045.3 g) in the total body, 3.8% (179.4 g) in arms, and 7.7% (1,104.8 g) in legs, respectively. There were a strong correlation of BMD and body composition between both groups. Conclusions: Linear correction equations were developed to ensure comparability of BMD and muscle mass between the Osteosys Primus and the GE Lunar Prodigy. Importantly, use of equations from previous studies would have increased the discrepancy between the Osteosys Primus and the GE Lunar Prodigy.

        Synovial Chondromatosis in the Temporomandibular Joint: Report of Two Cases

        Background Synocial chondromatosis(SC), a proliferative disorder of the synovial membrane. The etiology or cause of SC remains unclear. SC usually occurs in large articular joints such as knee, hip, elbow, and ankle. SC of the TMJ is very rare. It is a benign disease that mainly affects unilateral side. It can form cartilagenous and calcified loose bodies of various sizes and cause abnormal function of TMJ. Case Report In this paper, we report two cases of SC in the upper joint space of the left TMJ. One complained that "Sometimes the left jaw joint feels disoriented" and the other had no symptoms. CT scan and MRI showed left TMJ space widening, multiple tiny calcified mass. After clinical and radiographic analysis, we performed surgical removal of the lesion under genereal anesthesia. In the histologic examination, synovial chondromatosis was diagnosed in both patients. Conclusions We report two cases of synovial chondromatosis in the upper joint space of the left TMJ. We performed surgical removal of the lesion. The two patients showed good prognosis without recurrence or pain up to date.

        Measurement of Uncertainty Using Standardized Protocol of Hand Grip Strength Measurement in Patients with Sarcopenia

        Background: The aim of this study was to determine the accuracy and error range of hand grip strength measurement using various methods. Methods: Methods used for measurement of hand grip strength in 34 epidemiologic studies on sarcopenia were analyzed. Maximum grip strength was measured in a sitting position with the elbow flexed at 90 degrees, the shoulder in 0 degrees flexion, and the wrist in neutral position (0 degrees). Maximum grip strength in standing position was measured with the shoulder in 180 degrees flexion, the elbow fully extended, and the wrist in neutral position (0 degrees). Three measurements were taken on each side at 30 sec intervals. The uncertainty of measurement was calculated. Results: The combined uncertainty in sitting position on the right and left sides was 1.14% and 0.38%, respectively, and the combined uncertainty in standing position on the right and left sides was 0.35 and 1.20, respectively. The expanded uncertainty in sitting position on the right and left sides was 2.28 and 0.79, respectively, and the expanded uncertainty in standing position on the right and left sides was 0.71 and 2.41, respectively (k=2). Conclusions: Uncertainty of hand grip strength measurement was identified in this study, and a significant difference was observed between measurement. For more precise diagnosis of sarcopenia, dynamometers need to be corrected to overcome uncertainty.

        한국 골다공증의 역학

        The aging of the Korean population is expected to increase the prevalence of osteoporosis and related fractures. This study aimed to perform an intensive review of osteoporosis and related fractures in Korea during the last decade. Although direct comparison of the prevalence of osteoporosis reported in different studies is difficult, the prevalence of osteoporosis reported in the last decade has ranged from 6.1% in men and 24.3% in women to 13.1% in men and 35.5% in women in studies of Korean populations. According to reimbursement records from the National Health Insurance Servicesdatabase in 2012, the incidence and mortality of osteoporotic fracture including in the spine, hip, distal radius, and proximal humerus are 655.4/100,000 and 7.0%, 171.3/100,000 and 16%, 474.1/100,000 and 1.7%, and 90.9/100,000 and 7.0%, respectively. The numbers and incidence of hip and spine fracture were trending upward during the study period. However, mortality from osteoporotic fractures was steady or trended downward. Gender-specific assessment showed that although the incidence of osteoporotic fracture in men is less than in women, the mortality of men after osteoporotic fracture ranged from 1.4- to 2.2- fold that of women. Our review of epidemiologic studies regarding osteoporosis and related fractures demonstrated that Korea is a country with increasing trends of osteoporosis and related fractures. Therefore, a public health strategy for treating osteoporosis and preventing osteoporotic fracture is mandatory.
