        거짓침 피부접촉부 형태에 따른 침감 차이 연구

        Objectives: Proper acupuncture stimulation is associated with a characteristic set of sensation usually referred to as "De-Qi". In order to develop the appropriate sham acupuncture, various sensations to each stimulation should be considered through analysis of the profiles of acupuncture sensation. It was therefore investigated to compare the acupuncture sensation scale (ASS) of two types of sham acupuncture to that of the real acupuncture. Methods: Ninety-four participants (mean age 26.4, range 22-49) were asked to complete five point-Likert scale ASS developed by Vincent et al. after real or two-kinds of sham acupuncture stimulation: blunted tip sham acupuncture (BT) and round tip sham acupuncture (RT). Needling was done at LI4 acupoint on non-dominant hand and stimulated for 30 seconds with real or two-kinds of sham needle. Finger withdrawal Latency (FWL) of each group was also measured to evaluate the pain sensitivity to noxious heat stimuli. Results: BT acupuncture significantly Less produced penetrating, numb, intense, hurting, pulling, shock, tingling, throbbing sensation than real acupuncture stimulation. RT acupuncture significantly Less produced penetrating, burning, electric, numb, intense, hurting, pulling, aching, sharp, shock, stinging, tingling, throbbing sensation than real acupuncture. Each group did not demonstrate the differences of pain sensitivity to noxious stimuli. Conclusions: These results indicated that types of tip of acupuncture produced different kinds of acupuncture sensation. Our finding provides a general information of sensations to two kinds of sham acupuncture for development of ideal placebo sham needle.

        금단기간 중 흡연관련 시각자극에 의한 교감신경 반응에 대한 신문혈 침자극의 억제 효과

        Objectives: Cue reactivity is a key factor that modulates motivational goal-directed behavior associated with compulsive drug-taking and relapse. We investigated whether acupuncture attenuated the skin conductance response and the heart rate response to smoking-related visual cues in smokers. Methods: Twenty-nine smokers were treated with real acupuncture (RA) or sham acupuncture (SA). The stress response inventory was measured on the 5th day after quitting smoking. The skin conductance response and the heart rate were measured to evaluate the autonomic changes to the smoking related visual cues on the same day. Results: The sympathetic alterations in skin conductance and the heart rate induced by the smoking-related visual cues were significantly lower in the RA group, as compared to the SA group. The stress response inventory, such as somatization and frustration, in the RA group was significantly lower than that of the SA group. Conclusions: Acupuncture ameliorated the stress symptoms as well as the sympathetic activation to smoking cues during withdrawal. These findings indicate that acupuncture might be a useful tool in smoking cessation by inhibiting smoking cues-induced autonomic responses.

        최소 침(Minimal Acupuncture)이 침 임상연구에서 적절한 대조군인가?

        Objective: Acupuncture, one of the Oriental medical treatment techniques in East Asia, is growing in popularity all over the world. Based on bibliographical and clinical data, the depth of needling, i.e. superficial or deep needling, has been considered to be a variable as potential modifiers of needling effects, such as Localization, manipulation of the needle, or elicitation of DeQi . The purpose of this review is to summarize clinical trials using minimal acupuncture and to evaluate its appropriateness as a control. Methods: A survey of computerized Literature searches for randomized controlled trials using minimal acupuncture revealed that a total of 10 studies were published until April 2008. Results: Results obtained from clinical trials showed that acupuncture were more effective than minimal acupuncture in 3 out of 10 trials, while others were no more effective than control. However, minimal acupuncture might neither be a inert placebo nor be indiscriminable. Conclusion: Minimal acupuncture as `placebo` controls seems misleading and scientifically unacceptable.

        침과 자율신경계 고찰: 인체의 심박변이도 중심으로

        Objectives: Acupuncture restores the flow of vital energy in the body by stimulating the organ-specific meridians and enhances cardiac vagal activity and suppresses sympathetic activity. The purpose of this review is to summarize and evaluate the acupuncture studies on autonomic nervous system (ANS). Methods: We conducted a PubMed search to obtain a fair sample of acupuncture studies related ANS published in English in 2007. Each article was reviewed for study objectives and outcomes, as well as acupuncture points, experimental and control interventions. Results: We reviewed a total of 15 studies published from 1997 to 2007. In normal states, acupuncture enhanced either vagal or sympathetic tone depending on the stimulated acupuncture point sites. On the other hand, most of studies demonstrated that acupuncture restored the autonomic dysfunctions in various kinds of stressful states. Conclusions: We reviewed studies that contributed to an understanding of the autonomic mechanisms of acupuncture with the hope that this review will be of benefit to the future studies. Although the relationship between acupuncture and ANS response is still uncertain, acupuncture would be a noteworthy modality for modulating autonomic dysfunction.

        경혈정위 보조도구의 정확성 및 편의성 평가 연구

        Objectives: Accurate and reproducible location of an acupuncture point (AP) have been considered an essential component of meaningful acupuncture research as well as clinical practice. Several kinds of devices have been developed and widely used for the convenience of locating APs. However, the accuracy and convenience of these devices have not been systematically evaluated. The present study was performed to find out the most suitable tools for the location accuracy and location easiness of APs among the devices respectively. Methods: Twenty subjects attempted to locate APs, including PC5 and SP6 in the arm and leg, using four different AP positioning methods: one Conventional Ruler method and three alternative methods including Cunometer, Transparent AP meter, and Elastic Ruler method. The position marked on each AP was plotted onto a thin, flexible, and transparent plastic film, and the dispersion rate of the positions was determined and recorded. The elapsed time for locating each AP was measured. After that each subject was answered to a short questionnaire regarding the degree of convenience and confidence of use of each method for AP location. Results: All of three alternative methods took less time than Conventional Ruler method did. Among these alternative methods, the accuracy of Elastic Ruler method was markedly higher than others. The degree of convenience of the Cunometer and the confidence of Elastic Ruler method were the highest among these alternative methods. Conclusions: The present study indicates that the Elastic Ruler method was the most compatible for the conventional Ruler methods. However, there are many factors need to be reconsidered. Improved devices for locating AP are imperatively needed for clinical practice.

        침감과 침 진통 효과의 상관성 연구

        Objectives: The elicit of DeQi, a composite of sensation including numbness and fullness that develops at the site of acupuncture stimulation, is considered to be clinically essential to establish treatment efficacy. However, there was little studies investigating the relationship between the subjective feeling of acupuncture sensation and acupuncture analgesia. The current study was therefore performed to determine the correlation between the acupuncture sensation scale (ASS) and the degree of acupuncture analgesia. Methods: Ninety-two female subjects were stimulated at LI4 acupoint on non-dominant hand with real (RA) or sham acupuncture (SA). Finger withdrawal latency (FWL) of each group was also measured to evaluate analgesic effect of acupuncture on the noxious heat stimuli on the dominant hand. All subjects were asked to complete seven point-Likert scale ASS developed by Vincent et al. Results: The increase rate of FWL of the RA group was significantly higher than that of the SA group(36.03±4.45% vs 24.50±3.73%). RA stimulation produced significantly higher rating of all the ASS except for dull sensation, compared to SA stimulation. Significant correlations between analgesic effect of RA, but not of SA, and the degree of the ASS, including burning sensation(r=0.349), intense sensation(r=0.299), pulsating sensation(r=0.335), and stinging sensation(r=0.306) were found. Conclusions: The results of our study indicate that the DeQi sensation are associated with acupuncture analgesia. Our findings suggest that the evocation of DeQi might be useful clinical indicator of acupuncture treatment.

        투로(套路)(기무료법(氣舞療法): TuRo)가 여중생들의 정신건강에 미치는 효과

        Objectives: TuRo, defined as a dance therapy applying the meridian Qi system into the dancing movements, may have effects on ameliorating the psychological symptoms that often appear in adolescent students in a stressful situation. The Objective of the present study was to examine whether TuRo training relieves the psychological symptoms in adolescent female students. Methods: Symptom Check List-90-Revision (SCL-90-R), a 5 point Likert scale, consisted of 90 items that represent psychological symptoms, were used to measure the psychological health status in healthy adolescent female. Twenty two adolescent female students were trained TuRo dancing for two months while 29 in control group were trained mimicked movements without the concept of Qi for the same period. Changes in Somatization, Obsessive-Compulsive behaviors, Interpersonal Sensitivity, Depression, Anxiety, Hostility, Phobic Anxiety, Parinoid Ideation and Psychotic symptoms by the two months of training were compared between TuRo and control groups using SCL-90-R. Results: Among the nine-categorized psychological symptoms, the somatization and hostility were significantly reduced in TuRo group (p<0.01, p<0.05, respectively). Obsessive-Compulsive symptoms, Interpersonal Sensitivity, Phobic Anxiety, Hostility, Psychoticism were significantly reduced by two months of training (p<0.05, p<0.01, p<0.05, p<0.01, p<0.01), however, group differences were not detected. Conclusions: Symptoms of somatization and hostility were significantly reduced by TuRo training. Our finding indicates that TuRo training may have therapeutic effects on the psychological symptoms in teen age girls who may suffer stress-related symptoms.

        국내 학술지를 통해 살펴 본 辨證의 의미 고찰

        Objectives : The term ByunJeung is usually used to represent the characteristic of Korean Medicine. However the meaning of ByunJeung has not been consistent. After the 1980s when Traditional Chinese Medicine was introduced into Korea, the meaning of ByunJeung in Korea became diverse. In this paper, we aimed to research how the meaning of ByunJeung has been varied. Methods : We used four major journal database in Korea to search articles related to ByunJeung. Using the terms such as "변증" and "辨證", which mean ByungJeung in Korean and Chinese, we collected 500 articles in total. Among them, we chose only 61 articles selectively which mainly focused on the meaning of ByunJeung. We categorized those according to the specific meaning of ByunJeung which each article used, and qualitatively analyzed those articles. Results : Articles related to ByunJeung were categorized into three different groups. The first group was for the articles which dealt with changing meanings of ByunJeung. The second group was for the articles which focused on the modernized meaning of ByunJeung. The third group was for the articles which was on the traditional meaning of ByunJeung. Conclusions : The term ByunJeung holds a key concept in the current field of Korean medicine. Nevertheless, since the meaning of ByunJeung has been diverse, it often caused confusion among the people who used the term ByunJeung. Therefore we need to clarify the exact meaning when we intend to use the term. Also there should be a further discussion on the meaning of ByunJeung in the field of Korean medicine to facilitate better understanding on the characteristic of Korean medicine.
