        다양한 증상을 호소한 대퇴 절단지통

        This is a study of decrease in both stump pain and unidentified complaints after removal of neuroma on an amputated left thigh. The patient was a 44 year old woman who received an operation after a motorcycle accident 20th of March, 1991. She started a rehabilitation program in early June of the same year. How- ever the patient complained of a squeezing pain on the amputated area. This symptom became more severe after the removal of the nails in September. The pain was perceived as a mental problem and the patient was released from the previous hospital. The pain continued and on the 9th of March, 1992, the patient was introduced to our pain clinic. The patient complained about the cold sensation and pressure pain of the amputated area at the beginning. Later she also expressed various unidentified complaints. No improvement resulted after conducting an epidural block and a lumbar sympathetic ganglion block. MMPI test showed psychological in- stability. Local injection showed some positive effects, which led to considerations concerning the possibility of neuroma. After confirming the existence of neuroma through CT and MRI, neuromectomy was performed. After the removal of neuroma, the unidentified complaints as well as the stump pain decreased

        전략적 인적자원관리에 있어서 인사부서의 역할이 조직성과에 미치는 영향

        오늘의 지식기반경제시대에서 인적자원은 물론 인적자원관리 또한 기업의 경쟁 우위와 지속가능 발전을 위한 핵심요인으로 부각되고 있으며 전략적 인적자원관리와 조직성과의 관계에 관하여는 선행연구가 적지 않으나 인적자원관리의 유용성에 대한 인식의 제고와 실무적 발전을 위하여는 여전히 연구의 축적이 필요하다. 연구결과 전략적 인적자원관리의 7가지 구성요소는 모두 조직성과 면에서 유용성이 실증적으로 검증되어 전략적 인적자원관리의 보편적 접근법의 선행연구들과 일치하는 결과를 나타냈으며 이들간 상대적 중요도도 확인되었는데 상대적 중요도가 가장 높은 것은 정보공유였으며 다음으로 지위격차해소, 참여적 직무설계 등의 순이었으며 가장 낮은 것은 고용보장의 정도였다. 또한, 인사부서의 4가지 역할특성 중 최근 경영환경변화에 따라 그 중요성이 점차 부각되고 있는 전략적 파트너특성, 변화 촉진자특성은 전략적 인적자원관리와 조직성과의 관계에서 조절효과가 부분적으로 검증되었다. In the knowledge-based economy era when is marked by rapid globalization, accelerated digital revolution and growing uncertainty of management environments, not only human resources but human resources management is one of key factors to attain the competitive advantage and sustainable growth. The findings of the study were as follows. The seven practices of SHRM, which were selective hiring, extensive training, compensation contingent on organizational performance, participative job design, extensive sharing of financial and performance information, reduction of status differences and employment security suggested by Pfeffer(1997), turned out to be useful for organizational performance involving job satisfaction, organizational commitment and turnover intention. Thus, the importance of SHRM was proven. When the relative importance of the subvariables of SHRM was checked, the relative importance of extensive sharing of financial and performance information was largest, and that of employment security was smallest. The hypothesis that the two subvariables of the HR's department role characteristics, which include the characteristics of strategic partners, those of change agents, might have a significant moderating effect on the relationship between SHRM and organizational performance was partially accepted.

        긍정심리자본과 영감적 동기부여가 종업원 인게이지먼트에 미치는 영향에 대한 연구

        본 연구는 긍정심리자본과 영감적 동기부여 특성이 종업원 인게이지머트에 미치는 영향에 관한 연구이다. 글로벌화의 가속화, 4차 산업혁명시대의 도래, 비대면 활동 증가 등 경영환경의 변화 가운데 그 중요성에 비해 연구가 미흡한 종업원 인게이지먼트와 선행요인으로서 긍정심리자본 및 상사의 영감적 동기부여에 관하여 검증하였다. 본 연구를 통하여 변화된 경영환경에서 종업원의 인게이지먼트를 향상 시킬 수 있는 개인의 심리적 역량인 긍정심리자본과 리더십 역량으로서 영감적 동기부여의 구축방안과 시사점을 제시하고자 한다. 국내 일반기업의 임직원을 조사 대상으로 설문지를 이용한 조사를 실시하여 통계자료를 확보하였고, 상관관계분석 및 위계적 회귀분석 등을 통하여 검증을 실시하였다. 가설검증에서 독립변수인 긍정심리자본의 네 가지 하위요인은 모두 조직구성원의 인게이지먼트와의 관계에서 정(+)의 관계를 나타냈으며 해당가설들이 채택되었고 긍정심리자본이 기업의 지속적 경쟁우위와 성과창출을 위한 종업원 인게이지먼트에 매우 중요한 요인임이 확인되었다. 회복력과 종업원 인게이지먼트 간의 관계에서 리더십역량으로서 영감적 동기부여특성의 조절효과에 관한 가설이 채택되어 상사의 리더십 역량으로서 영감적 동기부여의 조절효과가 부분적으로 규명되어 종업원 인게이지먼트 제고의 중요한 하나의 요소임을 확인할 수 있었다. This examined the relationship among positive psychological capital, inspirational motivation and employee engagement and verified the moderating effects of inspirational motivation. Among the changes in the organization environment, such as the acceleration of globalization, the advent of the 4th industrial revolution, and the increase in non-face-to-face activities, it examined inspirational motivation as prior factors affecting employee engagement. In this organization environment, it proposes the very impotant implications for leadership and individual psychological competency to improve business performance by enhancing employee engagement. The results were as follows. First, the positive psychological capital factors had a positive influence on employee engagement. Second, inspirational motivation had moderating effects on relationship between resilience of the positive psychological capital and employee engagement. Results emphasize the importance of positive psychological capital as personal competency and inspirational motivation as leadership competency with employee engagement.

        하악신경차단으로 도움받은 삼차신경통환자의 영양섭취 및 간기능회복

        It is well known that many patients with trigeminal neuralgia suffer from electric shock-like stabbing pains. The pain can be triggered by nonnoxious stimuli such as touching of the face, chewing, talking or swallowing. This 62 year old woman was urgently admitted to the internal medicine department due to abdominal distention and severe general weakness. She has suffered characteristic violent pain triggered by chewing and swallowing for little over 4 years. This resulted in poor oral feeding for prolonged period which left her severely debilitated. The large amount of ascites that developed 20 days before admission and extreme emaciation forced her to bed rest. She also suffered from Herpes Zoster. After medical treatment to improve liver function and severe pain was persisted, the patient was referred to our department for control of pain. We performed right mandibular block with 1% dibucaine 0.4 ml and the effect was excellent. After the pain had subsided, patient was able to take meals more comfortably and improved liver function returned.

        직무스트레스와 이직의도에 미치는 선행요인에 대한 연구 : 변혁적 리더십 및 자기효능감을 중심으로

        본 연구는 조직성과차원에서 직무스트레스와 이직의도에 미치는 선행요인으로서 변혁적 리더십과 자기효능감이 직무스트레스 및 이직의도에 미치는 영향을 연구하였다. 최근 비대면 활동이 증가되고 있는 경영환경에서 연구가 미흡한 조직성과차원의 직무스트레스 및 이직의도의 선행요인으로서 변혁적 리더십 및 자기효능감에 관하여 검증하여 변화된 경영환경에서 직무스트레스 및 이직의도를 낮추어 조직성과를 향상시키기 위하여 필요한 구축방안과 시사점을 제시하고자 한다. 국내 일반기업의 조직구성원들을 대상으로 설문조사를 통하여 자료를 수집하였고, 검증을 위하여 상관관계분석과 위계적 회귀분석 등을 실시하였다. 가설검증에서 영감적 동기부여 특성은 직무스트레스 및 이직의도와 부(-)의 관계를 나타냈고, 지적 자극 특성은 이직의도와의 관계에서 부(-)의 관계를 나타냈으며 해당 가설이 채택되었다. 구성원의 자기효능감의 조절효과에 관한 시험적 가설은 채택되지 않아 자기효능감이 조절변수로 작용하지 않음을 확인하였다. 가설검증 결과, 변혁적 리더십이 효과적으로 발휘되면 직무스트레스와 이직의도의 수준을 줄이는 영향을 발휘하는 중요한 요인임을 확인할 수 있고 리더십 역량 및 변혁적 리더십의 개발과 구축이 조직의 전반적 성과향상에 중요한 요소임을 확인하였다. This study examined the relationship among of transformation al leadership, job-related stress, turnover intention and verified the moderating effects of self-efficacy. Using survey data from employees of the Korean enterprises in Korea, performed correla tion analysis and hierarchical regression analysis. The results were as follows. First, inspirational motivation had a positive influence on the job related stress and turnover intention. Second, intellectual stimulation had a positive influence on turnover intention, but has no influence on job related stress. Third, self-efficacy had no moderating effects on relationship between the leadership traits and job-related stress, or turnover intention. Results emphasize the importance of inspirational motivation, intellectual stimulation of transformational leadership as leadership competency given its crucial relationship with job-related stress and turnover intention.

        대요근 근구차단술에 대한 임상경험

        N/A Psoas compartment block has been used to provide anesthesia for orthopedic surgical procedures and postoperative pain control. Nowadays this block is advocated for the man- agement of pain in the thighs, legs and lumbar area for various reasons. To relieve pain in the thigh and leg, 2l times of block were performed and observed clinically in the l7 patients at our hospital. The results were excellent for the relief of pain in metastatic cancer patients. And this block is satisfactory in the thigh pain but not in the leg pain. So we recommend psoas compartment block as a useful and simple method for the relief of pain in metastatic can- cer pain in the thigh.

        자기공명분석기에 의한 반사성 교감신경성 위축증의 치험

        Reflex sympathetic dystrophy is a syndrome characterized by persistent, burning pain, hyperpathia, allodynia & hyperaesthesia in an extremity, with concurrent evidence of autonomic nervous system dysfunction. It generally develops after nerve injury, trauma, surgery, et al. The most successful therapies are directed towards blocking the sympathetic intervention to the affected extremity by regional sympathetic ganglion block or Bier block with sympathetic blocker; other traditional treatments include transcutaneous electrical stimulation, immobilization with cast & splint, physical therapy, psychotherapy, administration of sympathetic blocker, calcitonin, corticosteroid and analgesic agents. The purpose of this report is to evaluate and describe the effects of magnetic resonance following unsatisfactory results with traditional treatments of RSD. A 17 year old female patient, 1 year earlier, had received excision and drainage of pus at the right femoral triangle due to an injury caused by a stone. Afterwards, she experienced burning pain, knee joint stiffness, and muscle dystrophy of the right thigh, especially when standing and walking. Despite a year of number of traditional treatments such as: lumbar sympathetic block, continuous epidural analgesia, transcutaneous electrical stimulation, & administration of predisolone, her pain did not improve. Surprisingly, the patients was able to walk free from pain and difficulty after just one application of magnetic resonance. The patient has been successfully treated with further treatment of two to three times a week for approximately ten weeks. More recently, magnetic resonance has been demonstrated to produce effective results for the relief of pain in a variety of diseases. From our experiences we recognize magnetic resonance as a therapeutic modality which can provide excellent results for the treatment of RSD. It has been suggested that polysynaptic reflex which are disturbed in RSD may be modulated normally on the spinal cord level through the application of magnetic resonance.
