        Tetracaine 안장마취에서 진통 기간에 대한 Clonidine 과 Epinephrine 의 비교

        Clonidine, α₂ adrenergic agonist, has a vasoconstrictive property like epinephrine when injected into the subarachoid space. Thus duration of anesthesia would be prolonged in regional block. Besides this effects clonidine also produces direct analgesic effect at spinal cord level and potentiates the effect of local anesthetics injected into intrathecal space. In this study saddle blocks were induced in sitting position by the intrathecal injection of tetracaine for the perianal procedures. 45 patients were classified randomly into three groups. In group I (control) 0.5% tetracaine 4 mg (0.8 ml) and normal saline 0.2 ml was injected intrathecally at L_(4-5) interspace. In group II (epinephrine) epinephrine 0.2 mg (0.2 ml) was added to 0.5% tetracaine 4 mg and in group III (clonidine) clonidine 30 ㎍ (0.2 ml) was added to tetracaine 4 mg. After intrathecal injections, sensory block level and motor impairment was checked at 5 min and 50 min, The duration of analgesia was defined by the patients who complain the postoperative pain at the incision site. The results in each group were as follows. Sensory block level was Sl (control), L51 (epinephrine) and L5±1 (clonidine) at 50 min after injections. The duration of analgesia was significantly prolonged in epinephrine group (369.6±64.1 min) and clonidine group (297.9±33.8 min) compared with control group (194.3±18.2 min). The number of patients with motor block was 12 in epinephrine group, 4 in clonidine group and 1 in control group. In conclusion, tetracaine plus clonidine 30 ug to prolong the duration of analgesia is considered as adequate and safe dosage without significant adverse effects in saddle block.

      • 70세이상 노인마취례의 임상적 고찰

        With the advent of medicare and the increase in population over 70 years, anesthesiologist is faced in his daily practice with an ever-larger number of geriatric patients. Because the aged patients have a multiplicity of preoperative pathologic and physiologic states which may affect them during surgery, postoperatively they must be considered in their perioperative proper management. Two hundred twenty three surgical records of patient over 70 years of age were reviewed for the period of 1968 though 1981 from a total 26,722 such operations performed in Chung-Ang University Hospital. The details of these 223 operations were analyzed with respect to the variable incidence, sex, surgical department and disease, surgical site, elective or emergency surgery, concomittant disease, anesthesia technique and mortality. The results were as follows: 1. Total number of geriatric cases over 70 years of age were 223 cases(0.83% in average) in this period and there were 118 male(52.9%)and 105 female(47.1%) cases. 2. There were 97 cases(433.5%) in general surgery, but the most prevalent diagnosis were begign prostatic hypertrophy(BPH) in urosurgery which amounted to 35 cases(15,7%). 3. The most frequent surgical aite in this study was the lower abdomen(53.8%). 4. The rates of emergency surgery were 16% and most of them were diseases of general surgery(72.2%). 5. Preoperative concomittant diseases were 85% of all and were cardiovascular diseases and respiratory diseases in order. 6. General inhalation anesthesia(86.1%) was administered more oftem than regional anesthesia(13.9%) and halothane was used mainly since 1975. 7. The rates of postoperative complication were 10% and the relarionship between complications and duration of anesthesia was noted. 8. There were 8 cases(3.6%) of postoperative death, but nothing since 1976. In summary, the patients over 70 years of age had a poor surgical risk, but it is emphasized that the improvement of medicare and medical equipment will be needed to decrease the morbidity and mortality.

        Lactobacillus curvatus WiKim38 isolated from kimchi induces IL-10 production in dendritic cells and alleviates DSS-induced colitis in mice

        Probiotics such as lactobacilli and bifidobacteria have healthpromoting effects by immune modulation. In the present study, we examined the immunomodulatory properties of Lactobacillus curvatus WiKim38, which was newly isolated from baechu (Chinese cabbage) kimchi. The ability of L. curvatus WiKim38 to induce cytokine production in bone marrow-derived dendritic cells (BMDCs) was determined by enzyme-linked immunosorbent assay. To evaluate the molecular mechanisms underlying L. curvatus Wikim38-mediated IL-10 production, Western blot analyses and inhibitor assays were performed. Moreover, the in vivo anti-inflammatory effects of L. curvatus WiKim38 were examined in a dextran sodium sulfate (DSS)-induced colitis mouse model. L. curvatus WiKim38 induced significantly higher levels of IL- 10 in BMDCs compared with that induced by LPS. NF-κB and ERK were activated by L. curvatus WiKim38, and an inhibitor assay revealed that these pathways were required for L. curvatus WiKim38-induced production of IL-10 in BMDCs. An in vivo experiment showed that oral administration of L. curvatus WiKim38 increased the survival rate of mice with DSS-induced colitis and improved clinical signs and histopathological severity in colon tissues. Taken together, these results indicate that L. curvatus Wikim38 may have health-promoting effects via immune modulation, and may thus be applicable for therapy of various inflammatory diseases.

        Design optimization of tunneling field-effect transistor based on silicon nanowire PNPN structure and its radio frequency characteristics

        Recently, a number of semiconductor devices have been widely researched in order to make breakthroughs from the short-channel effects (SCEs) and high standby power dissipation of the conventional metal-oxide-semiconductor field-effect transistors (MOSFETs). In this paper, a design optimization for the silicon nanowire tunneling field-effect transistor (SNW TFET) based on PNPN multi-junction structure and its radio frequency (RF) performances are presented by using technology computer-aided design (TCAD) simulations. The design optimization was carried out in terms of primary direct-current (DC)parameters such as on-current (Ion), off-current (Ioff), current ratio (Ion/Ioff), and subthreshold swing (SS). Based on the parameters from optimized DC characteristics, basic radio frequency (RF) performances such as cut-off frequency (fT) and maximum oscillation frequency (fmax) were analyzed. The simulated device had a channel length of 60 nm and a SNW radius of 10 nm. The design variable was width of the ndoped layer. For an optimally designed PNPN SNW TFET, SS of 34 mV/dec and Ion of 35 mA/mm were obtained. For this device, fT and fmax were 80 GHz and 800 GHz, respectively.

        실리콘 나노 와이어 기반의 무접합 MOSFET의 최적 설계 및 기본적인 고주파 특성 분석

        기존의 n-type metal-oxide-semiconductor field effect transistor(NMOSFET)은 n?/p<SUP>(+)</SUP>/n? type의 이온 주입을 통하여 소스/채널/드레인 영역을 형성하게 된다. 30 ㎚ 이하의 채널 길이를 갖는 초미세 소자를 제작함에 있어서 설계한 유효 채널 길이를 정확하게 얻기 위해서는 주입된 이온들을 완전히 activation하여 전류 수준을 향상시키면서도 diffusion을 최소화하기 위해 낮은 thermal budget을 갖도록 공정을 설계해야 한다. 실제 공정에서의 process margin을 완화할 수 있도록 오히려 p-type 채널을 형성하지 않으면서도 기존의 NMOSFET의 동작을 온전히 구현할 수 있는 junctionless(JL) MOSFET이 연구중이다. 본 논문에서는 3차원 소자 시뮬레이션을 통하여 silicon nanowire(SNW) 구조에 접목시킨 JL MOSFET을 최적 설계하고 그러한 조건의 소자에 대하여 conductance, maximum oscillation frequency(fmax), current gain cut-off frequency(f<SUB>T</SUB>) 등의 기본적인 고주파 특성을 분석한다. 채널 길이는 30 ㎚이며 설계 변수는 채널 도핑 농도와 채널 SNW의 반지름이다. 최적 설계된 JL SNW NMOSFET에 대하여 동작 조건(V<SUB>GS</SUB> = V<SUB>DS</SUB> = 1.0 V)에서 각각 367.5 ㎓, 602.5 ㎓의 f<SUB>T</SUB>, f<SUB>max</SUB>를 얻을 수 있었다. The source/channel/drain regions are formed by ion implantation with different dopant types of n?/p<SUP>(+)</SUP>/n? in the fabrication of the conventional n-type metal-oxide-semiconductor field effect transistor(NMOSFET). In implementing the ultra-small devices with channel length of sub-30 ㎚, in order to achieve the designed effective channel length accurately, low thermal budget should be considered in the fabrication processes for minimizing the lateral diffusion of dopants although the implanted ions should be activated as completely as possible for higher on-current level. Junciontless (JL) MOSFETs fully capable of the the conventional NMOSFET operations without p-type channel for enlarging the process margin are under researches. In this paper, the optimum design of the JL MOSFET based on silicon nanowire (SNW) structure is carried out by 3-D device simulation and the basic radio frequency (RF) characteristics such as conductance, maximum oscillation frequency(f<SUB>max</SUB>), current gain cut-off frequency(f<SUB>T</SUB>) for the optimized device. The channel length was 30 ㎚ and the design variables were the channel doping concentration and SNW radius. For the optimally designed JL SNW NMOSFET, fT and fmax high as 367.5 ㎓ and 602.5 ㎓ could be obtained, respectively, at the operating bias condition (V<SUB>GS</SUB> = V<SUB>DS</SUB> = 1.0 V).
