Follicular thyroid carcinoma is the second most common form of thyroid cancer. It accounts for 10 percent of all thyroid cancer, and is found three times more frequently in men than in woman. It is well known that follicular thyroid carcinoma usually occurs in regions where it is hard for people to intake iodine. Most follicular thyroid carcinomas occur asymptomatically but occasionally reveal as masses in the thyroid nodules. The occurence of metastasis of follicular thyroid cancer in the lymph node is less than 10%, which is relatively low compared to that of papillary cancer. Follicular carcinomas are metastasized hematogenously and are common in the bone, liver, lung and brain. It is especially rare for metastatic follicular thyroid cancer to be diagnosed as renal tumor. We recently experienced and further investigated a case of follicular thyroid carcinoma that metastasised to the kidney in a 57-year-old woman, who had had abdominal distension. Endoscopic medial maxillectomy (EMM) and its modifications are surgical techniques areused to treat recalcitrant maxillary sinusitis as well as maxillary sinus tumors. In this report,we propose a simple and efficient modification of EMM, called endoscopic trans-turbinal medialmaxillectomy (ETTMM), by which the inferior turbinate (IT), nasolacrimal duct, and anatomicalintegrity of the nasal valve area are preserved. A total of 10 patients (five tumorousand five nontumorous maxillary diseases) underwent ETTMM. Briefly, a turbinate mucosalflap on the superior aspect of the IT was elevated after middle meatal antrostomy. Then atrans-turbinal window was developed to expose the inferior meatus, after which an extendedmaxillary antrostomy was generated. Finally, the turbinate mucosal flap was repositioned aftercomplete removal of the antral lesions. All lesions were successfully treated using ETTMM. Our modification was easy to perform, and we achieved good endoscopic visualizationand accessibility throughout the whole antrum by creating a trans-turbinal window and extendedmaxillary antrostomy. We could perform postoperative surveillance easily through thewide antrostomy using rigid endoscopes of various angles. ETTMM is a simple and usefulmodification of EMM that provides clear visualization and great accessibility to most aspectsof the maxillary antrum while preserving the nasal functional units, including the IT and nasalvalve area.

Inverted papilloma is a rare sinonasal tumor that mainly occurs in adults during the 5th decade. Three characteristics make this tumor very different from other sinonasal tumors: a relativelystrong potential for local destruction, high rate of recurrence, and a risk of carcinomatous evolution. Etiology remains little understood, but an association with human papilloma virus hasbeen reported in up to 40% of cases, raising the suspicions of implication in the pathogenesisof inverted papilloma. Treatment of choice is surgery, by endonasal endoscopic or external approach,depending on extension and tumoral characteristics. We experienced a case of 42-yearoldmale with inverted papilloma which originated from the ethmoid roof and spread into thefrontal sinus. He was treated successfully by unilateral osteoplastic flap surgery without obliteration,and we report on the treatment of this case, along with a review of literature.

Mucosa-associated lymphoid tissue (MALT) lymphomas are low-grade extranodal B-cell lymphomas that may involve various sites in the head and neck including the thyroid, salivary, and lacrimal glands. In particular, primary salivary gland MALT lymphoma is rare, with sublingual gland tumors especially rarer and frequently malignant. In Korea, there has been no reported case of MALT lymphoma in sublingual glands. Here we report a case of a MALT lymphoma in the sublingual gland. Previous pathophysiologic studies have revealed that the upper airway dimension reaches aminimal value at the end of expiration, and pharyngeal negative pressure is not an essentialprerequisite for upper airway collapse. Furthermore, pharyngeal collapse occurs in spite of increasedactivity of upper airway dilator muscles in patients with obstructive sleep apnea (OSA)compared with normal subjects. Pharyngeal sensory threshold is elevated in patients withOSA, which contributes to inadequate activity of upper airway dilator muscles. Pharyngeal narrowingresults in several types of airflow limitation, of whom the mechanisms are mainly explainedusing Starling resistor model and negative effort dependence. Several studies have beenconducted over the past years to validate polysomnographic surrogate markers which indicatespecific pathophysiologic phenotypes, and such attempts may be able to lead us to personalizedtreatment for OSA.

Primary pneumatization of the frontal bone occurs in the first year of life, but further pneu-matization continues until 18 years of age. Elongation of the ethmoidal infundibulum andfrontal recess, or upwards migration of the anterior ethmoidal cells has been proposed as amechanism of frontal sinus development. The frontal sinus drainage pathway is displaced inthe anterior or posterior direction depending on the pneumatization pattern of the frontoeth-moidal cells. Also, frontal sinuses can be connected to the infundibulum or directly to themiddle meatus depending on the superior attachment pattern of the uncinate process. Bilateralfrontal sinuses are separated by their bony septum, and thus have their own drainage path-ways. Contrary to this general rule, we experienced two cases of frontal sinuses that wereconnected to each other, showing a single drainage pathway from both frontal sinuses. We re-port these two cases along with a literature review.
