Eisenmenger씨 증후군 환자의 고관절 전치환술을 위한 마취 관리

Eisenmenger’s syndrome describes the elevation of pulmonary arterial pressure to the systemic level caused by an increased pulmonary vascular resistance with reversed or bi-directional shunt through an intracardiac or aortopulmonary communication. We report the case of 48-year-old woman with an elective total hip replacement arthroplasty for right femur neck fracture with Eisenmenger’s syndrome secondary to large atrial septal defect. Anesthesia was induced with etomidate and rocuronium, maintained with desflurane 5 vol% and O2 3 L/min. Mirinone and norepinephrine were infused continuously to decrease right to left shunt. The patient was extubated after spontaneous breathing recovery and transferred to the intensive care unit. She was treated with mirinone, norepinephrine, dopamine, and dobutamine for hypoxemia and hypotension. After 3 hours of admission to intensive care unit, the patient had a cardiac arrest and died 30 minutes later.

1980년 5월 당시 서독 거주 한인들은 베를린을 비롯한 주요 도시에서 광주민주화운동의 희 생자를 기리고 고국의 민주주의와 인권을 위한 거리 시위를 진행하였다. 그 이후로 한 해도 거르지 않고 사십 년 이상 매년 재유럽오월민중제를 개최하며 5·18정신의 확대와 재생산을 위 해 노력하고 있다. 본 연구는 현재까지 재유럽오월민중제를 주최 주관해온 주요 활동가들속에 서 진행한 인류학적 참여관찰과 심층 인터뷰를 바탕으로 독일에서 발현되는 5·18 민주화운동 의 의미를 고찰한다. 유럽 냉전체제의 최전선이자 분단국가였던 서독에 국가 재건을 위한 이 주노동자로 파견되었던 전 광부와 간호사 출신의 한인들은 어떻게 광주민주화운동을 간접 체 험했으며, 그로 인한 정서적 반응과 가치관 및 정체성 변화를 경험했는지, 그리고 새롭게 형 성된 신념을 통해 실천해온 독일과 한국을 잇는 초국가적 활동들은 어떤 함의를 지니는지 서 사적으로 분석한다. 이를 통해 궁극적으로는 5·18민주화운동은 기념되는 것으로 머무는 과거 의 역사가 아니라, 그 속에 내재해있는 “주먹밥”정신이 글로벌 코리안 공동체 형성을 위한 정 서적 연대, 즉 미래지향적 사회통합의 열망으로 끊임없이 재생산되고 있음을 강조한다. In May 1980, Koreans living in West Germany held street protests in Berlin and other major cities for democracy and human rights in their home country in honor of the victims of the Gwangju Democratization Movement. Since then, they have organized the May People's Festival in Europe every year for more than 40 years, striving to expand and reproduce the May 18 spirit. This study examines the meaning of the May 18 Democratic Movement articulated in Germany based on anthropological participation observation and in-depth interviews among major activists who have hosted the May People's Festival in Europe so far. It explores the ways in which the former Korean miners and nurses who were dispatched as migrant workers to West Germany, the front line of the European Cold War, indirectly experienced the Gwangju Democratization Movement, what changes they experienced in emotions, values, and identity, and further what kinds of transnational activities they have practiced between Germany and Korea through newly formed beliefs. Ultimately, the May 18 Democratic Movement is not celebrated as an event of the past, but it emphasizes that the rice ball spirit inherent in it is constantly reproduced as an emotional solidarity for forming a global Korean community, that is, a desire for future-oriented social integration.

The Religious-Political Aspirations of North Korean Migrants and Protestant Churches in Seoul

This article highlights an aspiration specific to Seoul that is projected onto, experienced, and contested by North Korean refugee-migrants who have recently arrived by way of China in this capitalist city of a divided Korea. I pay particular attention to the role of the evangelical Protestant Church in the process of subjectification of these migrant individuals and the performative rituals by which they negotiate religious-political aspirations toward the future. The bodily-spiritual transformation of individual North Korean migrants into Christians is not strictly teleological and is more complicated, ambivalent, and diversified. By comparing two distinctive North Korean migrant activities—the balloon leaflet campaigns and the With-U music concerts and activities—this article discusses the efficacies of the performative rituals of violence and peace that contest and constitute the particular religious-political aspirations in the context of late-Cold War Seoul.

Natural Light에 근거한 Digital Light 연출에 관한 연구

It is getting more and more to research and develop about digital moving images these days. The articles which had been published only explained new techniques about digital light, so it couldn't be standardized until now even though they should study light and shadow on real world first than new techniques. It is changed from analog to digital to process moving images these days and necessary to study natural light. This article focuses on the most important factors on the real world. Especially, this paper contains the result of the simulation for the properties of the skylight and the sunlight which is one of the most important ones. Also it is written about digital light based on real light to try to remove artificial feeling that had been handled troubles on computer graphics.On this article which tries to help and give directions to light the digital scenes for TV commercial, digital film and digital animations as it simulates properties of real light on digital one. 아날로그 영상에서 디지털 영상으로 변해가는 지금 Digital Lighting(디지털 조명연출)에 관한 연구는 절실히 요구되고 있다. 디지털 라이팅은 3D 가상공간상에서 물체를 단순히 보여주기 위함이 아니라 컴퓨터그래픽(Computer Graphic)의 극적인 연출이다. 인공조명으로 연출할 경우 빛의 위치, 방향, 색등을 조절할 수 있지만 반사, 굴절 등의 속성은 조절할 수가 없다. 반면에 디지털 조명은 인공조명에서 조절할 수 있는 것 뿐 아니라 빛의 속성까지도 조절할 수 있다. 하지만 자연광이나 인공조명처럼 빛의 그림자, 빛의 굴절, 빛의 반사 등을 자동적으로 생성하진 않는다. Digital Light를 연출함에 있어서 자연광의 원리는 매우 중요하며 그것이 바탕이 되어야 자연스럽고 드라마틱한 빛을 연출 할 수 있다. 그러므로 자연광에 대한 연구가 선행되어야한다. Computer Graphic Lighting 기술력은 자연광을 표현하기 위해 사용하며, Digital Lighting은 극적인 연출의 방법으로 활용할 수 있다. 본 논문은 우리들 눈에 익숙해져있는 자연광의 특징들을 디지털로 구현하여 디지털 애니메이션, 영상광고 및 영화 등 컴퓨터그래픽 장면에서 디지털 조명연출에 도움을 주고자 했다.

의례로서의 냉전문화와 북한이탈주민의 열망

본 연구는 국가권력에 의한 냉전의례와 평화지향적 의례의 불균형을 성찰하면서 통일담론의 한계를 지적하고, 그러한 의례로서의 냉전문화가 북한이탈주민들의 한국사회 적응 과정에서 어떠한 유형들로재생산되거나 협상되는지 민족지적으로 중층기술한다. 냉전을 의례로서 고찰함으로써, 한국 근대사의 조작된 위험을 상징적으로 보여주는냉전문화가 국가 차원에서는 어떻게 재생산되는지, “평화적 통일”이라는 헌법 정신은 왜 점차 외면되고 있는지에 대한 분석을 현실적으로하고자 평화적 의례와의 불균형성에 대해 심층적으로 논의한다. 나아가 북한이탈주민들이 기독교와 만나고 종교적 회심을 경험하는 과정에서 어떻게 냉전의례가 재생산되고 변형되는지에 대해서 민족지 기술을 시도한다. 궁극적으로 본 논고는, 미래지향적 열망인 통일감수성, 관용과 환대의 가치를 로컬공동체 차원에서 실현하는 과정과 소통 중심의 플랫폼 구상을 제안한다.

Efficacy of noninvasive pulse co-oximetry as compared to invasive laboratory-based hemoglobin measurement during spinal anesthesia

Background: The Masimo Radical 7 (Masimo Corp., Irvine, CA,USA) pulse co-oximeterⓇ noninvasively determines the hemoglobinconcentration using the principle of transcutaneous spectrophotometry. We compared hemoglobin levels determined using thisdevice (SpHb) with those determined using an invasive laboratory-based technique (tHb) during spinal anesthesia. Methods: Thirty patients received spinal anesthesia with 0.5%hyperbaric bupivacaine. The pulse co-oximeter probe was mountedon the second toe, and arterial blood samples were obtained froma radial artery catheter. SpHb, tHb, and perfusion index (PI) valueswere recorded before and 20 and 40 min after intrathecal injectionof bupivacaine. Results: Before spinal anesthesia, the SpHb and tHb showed asignificant difference of −2.86 ± 1.56 g/dl (P < 0.005), but nosignificant differences were found between tHb and SpHb at 20 and40 min after spinal anesthesia (−0.16 ± 2.45 g/dl and 0.29 ± 2.68g/dl). Additionally, PI was significantly increased at 20 and 40 minafter spinal anesthesia compared to the pre-anesthetic value (P <0.001). Conclusions: The toe is not the monitoring site for pulseco-oximetry in adult patients, but the pulse co-oximetry on the toeappears to be appropriate as a noninvasive hemoglobin monitoringdevice after spinal anesthesia.
