Purpose : To evaluate the relative value of arterial, portal venous and delayed phase images of spiral CT in he detection of metastatic tumor of the liver. Materials and Methods: Forty-three metastatic tumors in twelve patients were underwent tri-phasic spiral CT examination with injection of 120 ml ionic contrast material (36 g of iodine) at the rate of 3 ml/sec. Arterial, portal venous and delayed phase CT images were obtained 35, 65, and 360seconds after the start of contrast injection, respectively. Results : Arterial phase images detected 35(81%),portal venous phase images 43(100%), and delayed phase images 34(79%) lesions, respectively(p<0.05). All masses larger than 2cm(n=23) were detected in all three phases, whereas 60%, 100%, 55% of the masses smaller than2cm(n=20) were detected in arterial, portal venous and delayed phase CT, respectively. The best contrast between masses and the hepatic parenchyma was in portal venous phase followed by arterial and delayed phase(p<0.01). In two hypervascular masses, the contrast was better in arterial phase. Conclusion : Portal venous phase of spiral CT is optimal in the detection of metastatic tumor of the liver. Arterial phase may be helpful for the detection of hypervascular metastasis. Delayed phase should be used restrictively.
초음파 유도하 미세침 비흡인 세포검사: 갑상선 결절 검사의 유용성

목적 : 초음파 유도하 미세침 비흡인 세포검사가 시술자의 집중도를 높이는 편안한 검사이며 일반적으로 사용되어 온 세침 흡인 세포검사만큼 유용한 술기임을 알아보고자 한다. 대상 및 방법 : 2005년 8월부터 12월까지 초음파상 단독 혹은 다발성의 결절을 가진 262명의 환자, 415예의 갑상선 결절을 대상으로 하였다. 초음파 유도하에서 25-gauge 세침으로 모세 삼투압을 이용해 세포검사를 시행하였고 결절당 1~2회 천자를 실시하였다. 결과 : 세포병리결과 검체 적합율은 89.4%(371/415), 부적합 검체율은 10.6%(44/415)였다. 적합한 검체 371예중 양성이 282예(68%), 악성이 55예(13.3%), 여포성 종양이나 악성의증이 34예(8.2%)였고, 부적합 검체 44예 중 단단한 석회화 병변이 9예, 고형성분이 거의 없는 낭종이 2예, 33예가 고형 혹은 고형 및 낭종 혼합형이었다. 결론 : 초음파 유도하 미세침 비흡인 세포검사는 갑상선 결절의 진단에 필요한 검체의 적절한 채취를 위한 유용한 술기이고 흡인방법보다 검사자로 하여금 편안한 시술을 할 수 있도록 도움을 주는 방법으로 생각된다. PURPOSE : The purpose of this study is to evaluate whether ultrasound (US)-guided fine-needle non-suction technique is more comfortable and useful than the conventional suction technique for the cytopathologic evaluation of thyroid nodules. MATERIALS and METHODS : Cytopathologic findings at consecutive fine-needle non-suction biopsies of 415 thyroid nodules in 262 patients were retrospectively reviewed. The procedure was performed by non-suction technique with 25-guage needle under US guidance. For each nodule, the biopsy was repeated up to two passes. RESULTS : Cytopathologic diagnostic rate was 89.4% (371/415), and inadequate specimen rate was 10.6% (44/415). Among 371 adequate specimens, the nodules were benign in 282 (68%), malignant in 55 (13.3%), and suspicious for malignant or follicular neoplasm in 34 (8.2%). Among 44 inadequate specimens, nodules were calcified in 9, cystic in 2, and solid or mixed echoic in 33. CONCLUSION : US-guided fine-needle non-suction technique is more comfortable than and as useful as the conventional suction technique for the cytopathological evaluation of thyroid nodules.
바륨현탁액 투여후 메틸셀룰로스를 사용한 개조형 소장 바륨추적검사 : 전통적 바륨추적검사와의 비교

Purpose : To compare modified small bowel follow-through (SBFT) using methylcellulose after the administration of barium suspension with a conventional series. Materials and Methods : In order to evaluate small bowel pathology, modified SBFT was performed in 155 patients during a 15 month period. All patients received 600mL of methylcellulose ; 98 had taken 250mL of 40% wt/vol barium suspension and 57 had taken 150mL of 70% barium. For the group of 98, the barium suspension was prepared by mixing barium powder with water (n=46) or with methylcellulose in(n=52). For comparison with a modified series, 49 patients who underwent conventional SBFT using 500mL of 40% wt/vol barium were also included. Image quality was rated by three radiologists as "poor", "fair", "good", or"excellent". We analyzed the relationship between image quality, transit time and small bowel pathology; the sensitivity and specificity of each technique was also determined. Results : Among the four techniques, modified SBFT with 250mL of 40% wt/vol barium suspension, prepared by mixing barium powder with methylcellulose, showed the best image quality ["excellent" result in 33 of the 52 patients (63%)] and shortest transit time to the cecum. The high image quality of this technique was not affected by the presence of small bowel pathology; its use resulted in the lowest incidence and slowest development of flocculation. The sensitivity (91-95%) of the three modified SBFT procedures was superior to that of a conventional series(76%), but there was no difference in specificity. Conclusion : Modified SBFT using methylcellulose after administering barium suspension with barium powder as a mixing agent is a simple technique. Its use easily improves the image quality and diagnostic accuracy of peroral SBFT.e quality and diagnostic accuracy of peroral SBFT.
