        관상동맥 우회술을 병행한 대동맥판막 치환술 치험 1례

        Patient with aortic valvular disease have increased left ventricular work and greater myocardial oxygen demand, which may aggravate the effect of concomitant coronary artery disease. Thus in patient who repair aortic valve replacement, concomitant aortocoronary bypass surgery is often performed when angiographically significant coronary artery disease is present. This approach is supported by reports that revascularization does not increase operative risk when associated coronary artery disease is present and significantly reduce the occurrence of late sudden death. Recently we have experienced one case of aortic valve replacement concomitant with aorta-coronary bypass surgery. The patient was 56 year-old male and admitted with complaint of anterior chest pain especially during his exercise. He was diagnosed as aortic valve stenosis and regurgitation [GIII] with proximal right main coronary artery occlusion We performed aortic valve replacement with aorta coronary bypass surgery by use of saphenous vein. Post operative course was uneventful and chest pain was relieved. Post operative coronary angiogram disclosed good patency of grafted vessel.

        우폐문부에 발생한 Angiofollicular Lymph Node Hyperplasia -1례 보고-

        Angiofollicular lymph node hyperplasia is a relatively rare benign lesion. It develops most often in the mediastinum, but also it found in other area of the body, usually where lymph nodes are normally found. There are histologically divided hyaline vascular type, plasma cell type and transitional type. The lesions were discovered most often on routine roentgenograms, and complete surgical excision is the treatment of choice. Recently, the authors experienced a case of angiofollicular lymph node hyperplasia, transitional type, located in the right hilar area. The lesion was removed and the patient is in good condition postoperatively. Now we report this case with review of previously reported literatures.

        성인의 선천성 식도기관지루 - 1례 보고 -

        Congenital esophagobronchial fistula without esophageal atresia is rare, usually has an insidious clinical course, so it usually diagnosed in adulthood. A esophagogram showed a esophagobronchial fistula between diverticulum at the middle third of the esophagus and just proximal site of apical segmental bronchus of left lower lobar bronchus. It belonged to type I of Braimbridges classification for congenital esophago-bronchial fistula. Esophageal diverticulectomy, fistulectomy and left lower lobectomy were done and postoperative course was uneventful.

        Langendorff 분리쥐 심모형에서 L-Carnitine이 허혈성 심근에 미치는 효과

        Beta hydroxytrimethylammonium butyrate[L-carnitine] is highly concentrated in myocardium and it is essential substance for transfer of fatty acids into the mitochondria. We respect that L-carnitine has protective action to myocardium during ischemia. I studied coronary flow and CK - MB isoenzyme of coronary effluent of Langendorff`s isolated rat heart model. As a control group 5 Sprague-Dowley species rat hearts were connected to Langendorff`s isolated rat heart model and perfused for 30 minutes with Kreb-Henseleit buffer solution. After cessation of perfusion for 30 minutes they were reperfused for 30 minutes. In experimental group 10 Sprague-Dowley species rat hearts were perfused with 10mmole /L of L-carnitine contained in Kleb-Henseleit buffer solution. In equilibrium state, coronary flow was 1.7 times greater in experimental group. During reperfusion, both group showed equally decreased flow amount of about 60% of that of equilibrium state. CK-MB isoenzyme level of perfused coronary fluid showed no significant difference in equilibrium state. In reperfusion. CK-MB isoenzyme levels of control group were 17.61$\pm$8. 68U/L at 25 minutes, 23.32$\pm$4.15U /L at 30 minutes; and in experimental group, 13.63$\pm$6. 08U/L at 15 minutes and 13.6$\pm$8.41U /L at 30 minutes respectively. Those values in both states showed significantly lower CK-MB level in experimental group. In conclusion, L-carnitine prevent ischemic myocardial damage during ischemic and reperfusion state of Langendorff`s isolated rat hearts and also I suggest the L-carnitine act potent coronary vasodilator during preischemic and postischemic states of rat hearts.

      • 폐암환자의 객담에서 Melanoma Antigen Gene(MAGE)과 Synovial Sarcoma on X chromosome (SSX)의 발현

        Background : MAGE and SSX genes have been recently discovered as number of cancer/testis antigen, which are showed useful potential for anti-tumor immunotherapy and tumor mark. Expression rates of MAGE genes are disclosed in many cancer cases. Study of expression rate of MAGE and SSX genes in same time in lung cancer cases are rare. We studied about expression of both genes in sputum of lung cancer patients simultaneously. Material and method :Total 75 cases of patients studied, control group was 32 cases of benign lung disease, 43 cases of lung cancer patients were divided into two groups of non small cell lung cancer and small cell lung cancer groups. Patients sputum collected before treatment and extracted mRNA and identify MAGE genes and SSX genes by nested PCR method. Expression rates of both genes werestudied according to cell types, gender, LN metastasis and cancer stage. Results : In benign lung disease cases, expression rates of MAGE and SSX genes were 40.6% and 37.5% respectively. and was 12.5% in both genes. The expression rate of MAGE genes is higher inmale patients and expression rate of SSX genes is higher in female patients ( p < 0.01 ). Expression rate of MAGE genes in NSCLC group was 61.8%, and this is higher than that of control group. In squamous carcinoma, expression rate of MAGE genes is 76.9% and this is higher than that of adenocarcinoma. Expression rate in adenocarcinomaand small cell cancer are lower than control and squamous cell cancer.Expression rate of SSX genes in NSCLC group is 17.6% and is 11.1% in SCLC group. It is lower than that of control group. Expression rate of SSX genes is lower than MAGE genes in lung cancer cases and not found any relationship of expression of between both genes. No significant gene expressions between gender was observed. The present study suspect lymphnode involvement and cancer stage influence on expression of both genes in this study, but not significant statistically. Co-expression rate of both genes are 14.7% in NSCLC group, 11.1% in SCLC group and 12.5% in control group. Conclusion : MAGE genes are more applicable to diagnosing squamous cell cancer than SSX genes, but it's role seems to be still lim-ted to additive as a mark, because of expression in benign lung disease cases and lower expression rate in small cell carcinoma and adenocarcinoma. Not found any relationship on both gene expression in this study.
