Incidence and Types of Sphenoethmoid Cell in Patients with Chronic Paranasal Sinusitis

The sphenoethmoid cell is the most posterior ethmoid cell, pneumatizing far laterally and to some degree superiorly to thesphenoid sinus. A accurate understanding of the sphenoethmoid cell is important for avoiding fatal injury to the orbit and safelyapproaching the sphenoid sinus. To investigate the incidence of sphenoethmoid cells, the authors analyzed the coronal CT scansof 50 patients who underwent endoscopic sinus surgery for sinusitis. Sphenoethmoid cells were identified in 39 out of 100 nasalcavities and divided into two types according to their relationship with the optic canal and the sphenoid sinus:type 1 cells, wherethe optic nerve bordered the sphenoethmoid cell and the sphenoid sinus, were observed in 14 cavities, and type 2 cells, where theoptic nerve bordered only the sphenoethmoid cell, were observed in 25 cavities. We anticipate the existence of an optic canal inthe posterior ethmoid sinus in one third of the cases, and the direction of the approach to the anterior wall of the sphenoid sinusshould be modified according to the type of sphenoethmoid cell if one is present.

외국인근로자에 대한 강제퇴거처분과 절차적 구제수단의 모색

With the increase of alien labors and their activities within Korea, the matters of alien protection and expursion has become a social problem. But the contents of protecting rights and interests of alien labors were not included in removal procedures, as other parts of the Korean Immigration Laws. This paper has been studying various elements of executing removal procedures with more regard to a lot of difficulties of alien labors being removed. The Non-staying principle of Execution was adapted by the Administrative Litigation Act and the Administrative Appeals Act, bet it is necessary to restrict that principle. In main issue of this paper, I emphasized that noncitizen suspect has the right to appeal by Administrative Appeals Act, apart from the Korean Immigration Laws, and the decision whether the suspect is removed or not is made by judicial procedures apart from the execution body. It means 'division of examination body and execution body' and 'accomplishment of removal hearing' in reference to the Immigration and National Act of US. And in this paper I proposed 'Remedial Measures' which is ruled by judicial part. To put it concretely, suspending of execution or effect of removal measure is indispensable and efficacious to the noncitizen labors who were arrested by the immigration officers.

An Extremely Rare Variety of Anomalous Coronary Artery: Right Coronary Artery Originating From the Distal Left Circumflex Artery

A single coronary artery (SCA) is a rare congenital anomaly of the coronary circulation, which is often associated with myocardial ischemia and other congenital cardiac anomalies. A 77-year-old woman visited our hospital complaining of typical chest pain. Coronary angiography revealed an isolated SCA. The right coronary artery did not originate from the aorta, but instead emerged from the distal left circumflex artery, with significant stenosis at the proximal portion of the left anterior descending artery. A stent was successfully implanted at the culprit lesion. There was no perfusion defect detected by a cardiac SPECT study.

The Shape of Expiratory Nasal Flow

The characteristics of the expiratory nasal flow consist of three parts - amount, content and shape. By far, most reports areconfined only to the nasal resistance, nasal pressure, temperature, humidity and gas concentration of the expiratory nasal flow. There are few reports on the shape of the expiratory nasal flow. We aimed to visualize the expiratory nasal flow and to see whetherthere is any correlation between the shape of the expiratory nasal flow and the shape of the external nose. For each sevennormal adult males and females, the shape of the expiratory flow was checked with infrared imaging thermometer placed onthe frontal and sagittal planes. We also measured the external nasal indices and compared them with the shape of the nasal flow. We could see the shape of the expiratory nasal flow through the infrared imaging thermometer in real time. The mean of the anglesformed by each side of the expiratory nasal flows on the frontal plane was 39 degrees, and the frontal angle showed goodcorrelation with the shape of the external nose. The real-time visualization of the expiratory nasal flow, conducted with theinfrared imaging thermometer, was an absolutely non-invasive technique and may be used to investigate nasal functions.

A Case of Delayed Fatal Hemobilia after Radiofrequency Ablation of Hepatocellular Carcinoma

Radiofrequency ablation (RFA) has been accepted as a safe and effective treatment for unresectable hepatic tumors because of the advantages of a minimally invasive procedure and the local treatment effect. Although RFA can be performed safely in most patients,early and late complications related to mechanical or thermal damage may be observed at follow-up examination. Hemobilia is a very rare complication of RFA and most hemobilia is an immediate complication. However, delayed hemobilia is an even more rare complication after RFA and is known to be a fatal complication. We present a case of delayed fatal hemobilia after RFA for hepatocellular carcinoma.

A Comparison between Coronal and Axial Scans in PNS CT

Background and Objectives:A coronal section computerized tomography (CT) is essential for the diagnosis of paranasalsinusitis, especially in the evaluation of the ostiomeatal unit. Although the coronal scan is preferred to the axial scan, the role ofthe axial scan during endoscopic sinus surgery has not been thoroughly examined. The aim of this study is to compare the advantagesand disadvantages of coronal and axial scans in the paranasal sinuses. Materials and Methods:Selected for study werethe coronal and axial scans of 30 preoperative sinus CTs showing minimal sinus pathology. The rates of detecting 15 anatomicstructures in the coronal and axial sections were analyzed across 60 sides. Results:The coronal scan was superior to the axialscan in detecting eight normal structures, including the anterior ethmoid artery, the opening of the maxillary sinus and the superiormeatus (p<0.05). The axial scan was useful in showing the vertical portion of the 3rd basal lamella and the opening and theanterior wall of the sphenoid sinus (p<0.05). Conclusion:The coronal scan is more informative than the axial scan but haslimitations in showing the structures lying on the coronal plane. The axial scan plays a complementary role in covering thislimitation. Together, the coronal and axial sections of a sinus CT can help the surgeon to understand the three dimensional structureof the paranasal sinus before endoscopic sinus surgery.

술자 입장에서 바라본 비강 및 부비동 내시경 해부

Endoscopic sinus surgery is a treatment method for chronic rhinosinusitis not controlled withmedical treatment. It had started with functional endoscopic sinus surgery and the concept ofreboot approach was introduced recently. For safe surgical treatment, understanding the anatomyespecially personal variations between well-known structures is important. The practicalpoints of surgical concept during endoscopic sinus surgery was suggested.

내시경적 제거가 가능한 직장 점막하 병변으로 오인한 자궁내막증 1예

Endometriosis occurs most frequently in the intestine. In the pelvic organs intestinal endometriosis presents with various symptoms and endoscopic findings. If an asymptomatic submucosal lesion is found in the sigmoid colon or rectum of reproductive women, a differential diagnosis should be done. Owing to advancements in endoscopic therapy, endoscopic excision has been attempted for various subepithelial lesions. To successfully do an endoscopic excision, accurate diagnosis should be obtained through diagnostic tests such as endoscopic ultrasonography prior to excision. Here the authors report a case of rectal endometriosis in an asymptomatic woman of reproductive age. They attempted endoscopic resection based on the endoscopic finding that the subepithelial lesion was limited to the submucosal layer in endoscopic ultrasonography. This conclusion turned out to be a mistaken one. Because of tumor adhesion to the proper muscular layer, we failed to successfully conduct an endoscopic excision. Ultimately, we did surgery and diagnosed rectal endometriosis. 장관에 발생하는 자궁내막증은 골반내 장기를 제외하면 가장 호발하는 부위이며 다양한 양상의 내시경 소견 및 증상을 보인다. 특히 가임기의 여성에서 구불결장 혹은 직장에서 무증상의 점막하 병변이 발견되었을 때는 감별진단에 포함시켜야 한다. 내시경적 치료술의 발전으로 다양한 점막하 병변에 대한 내시경적 절제가 시도되고 있다. 하지만 실패없는 내시경적 절제를 위해서는 시술전 내시경 초음파 등의 진단적 검사를 통하여 정확한 정보를 얻어야 한다. 저자들은 가임기 여성에서 별다른 증상 없이 내시경으로 발견한 점막하 병변을 내시경 초음파에서 점막하층에 국한된 것으로 오인하여 내시경적 점막하 절제술을 시도하였으나 종괴의 고유근육층 유착으로 실패 후 수술적 절제로 자궁내막증으로 최종 진단된 1예를 문헌 고찰과 함께 보고한다.
