Comparison of Both Legs EMG Symmetry during Over-Ground Walking and Stair Walking in Stroke Patients
정무근,김중휘 대한물리치료학회 2015 대한물리치료학회지 Vol.27 No.4
Purpose: Gait is the most basic element when evaluating the quality of life with activities of daily living under ordinary life circumstances. Symmetrical use of the lower extremities requires complicated coordination of all limbs. Thus, this study examined asymmetry of muscle activity quadriceps femoris and tibialis anterior as a baseline for training during over-ground walking and stair walking of stroke patients. Methods: Subjects were 14 stroke patients included as one experimental group. Gait speed used in this study was determined by the subject. Low extremity paretic and non-paretic EMG was compared using the surface EMG system. Results: The low extremity EMG difference was statistically significant during over-ground walking and stair walking (p<0.05). The result of low extremity EMG substituted symmetry ratio formula was compared to EMG symmetry ratio in both legs during over-ground walking and stair walking. The average symmetry ratio of quadriceps femoris during over-ground walking was 0.65, and average symmetry ratio of quadriceps femoris during stair walking was 0.47, with significant difference (p<0.05). Conclusion: EMG data was higher in stair walking than over-ground walking. However, in the comparison of symmetry ratio, asymmetric EMG of quadriceps femoris was significantly increased during stair walking. These findings suggested that application of stair walking for strengthening of both legs can be positive, but the key factor is maintaining asymmetrical posture of both legs. Therefore, physical therapists should make an effort to reduce asymmetry of quadriceps femoris power during stair walking by stroke patients.

Purpose: The purpose of this study was to examine the effects of spinal decompression therapy on pain and disability in patients with chronic low back pain. Methods: Twenty patients with chronic low back pain were divided into an experimental group (spinal decompression therapy, n=10) and a control group (conservative physical therapy, n=10). Both groups were treated three times a week over a four-week period. Results: The comparison of between-group changes post-treatment revealed statistically significant lower levels of pain and disability in the experimental group than the control group. The comparison of within each group changes before and after the treatment showed statistically significant declines in pain and disability indexes of both groups. Conclusion: Spinal decompression therapy may be an effective intervention for improving pain and disability in patients with chronic low back pain.

Purpose The aim of this study was to find effect of task_oriented training including aftereffect after applying low frequency (1Hz) or higt frequency (10Hz) repetitive transcranial magnetic stimulation (rTMS) on hand spasticity in stroke patients. Methods Forty patients with history of stroke and upper limb hemiparesis (age at intervention: 51.3 years, time after stroke onset: 11.3 months) were studied. Forty patients were divided into four groups; task_oriented training including aftereffect after applying high frequency (10Hz) rTMS (TIAH, n=10), task_oriented training excluding aftereffect after applying high frequency (10Hz) rTMS (TEAH, n=10), task_oriented training including aftereffect after applying low frequency (1Hz) rTMS (TIAL, n=10), task_oriented training excluding aftereffect after applying low frequency (1Hz) rTMS (TEAL, n=10). The intervention was performed for four times a week, for four weeks. Results As shown by the result of the Modified Tardieu Scale (MTS), there were significant differences on follow up test in the hand spasticity of the TIAH group, wheras there was a significant difference on during intervention (between baseline and 4 weeks) in those of the TEAL group. Furthermore, results of motor evoked potentials (MEPs) showed significant differences in all four groups. Conclusion The 4 weeks protocol of TIAH seems effective in improving hand spasticity in upper limb hemiparesis after stroke.

Advanced Phase Isolation Ditch (APID) process was studied to develop economic retrofitting technology, for the plants where retrofitting of common activated sludge process is required. In this study, to evaluate and monitor the effluent water quality (BOD5, SS, T-N, and T-P) and operating conditions (Influent, SVI, SRT, and HRT) as process capable and stable parameters for treating municipal wastewater, a demonstration plant was installed and operated in the existing sewage treatment plant of P city. During this study, the average effluent BOD5, SS, T-N, and T-P concentrations were 7.7, 5.6, 10.8, and 1.6 mg/L. Trend analysis of influent BOD5, SS, T-N, and T-P in APID process were illustrated that APID process need for more strong APID process management on the winter session, such as developing new intermediated aeration mode, operating methods, and managements strategy. At the application of control chart, the signal of uncommon effects at APID process was determined much higher existing control chart tntr conventional control chart in this study. These results indicate that conventional control chart has been collected and determined cleary signal at only stable situation. Therefore, newly developed APID process of dynamic control chart can be one of the useful tool for monitoring and management process.
