70세 이상의 대퇴 경부 골절에서 골시멘트를 이용한 양극성 고관절 성형술의 임상적 결과

There have been much controversy about prosthetic replacement or internal fixation for patients over 70 years old. Authors reviewed and analyzed 45 cases of cemented bipolar hemiarthroplasty in patient over 70 years treated at the Department of Orthopedic Surgery, Sung-Ae General hospital from January l988 to July 1995. The purpose of this study was to analyse the clinical and radiological results and to detect the motion study of bipolar cup. Follow up period was average 26.1 months, ranged from 24 months to 37 months. The following results were obtained. l. In clinical evaluation, excellent & goad result were in 31 cases(68.87o). 2. In radiologic evaluation, acetabular erosion were in 2 cases and loosening of the femoral component was in 1 case. 3. With time elapsed, the amount of the inner bearing motion was decreasing with preservation of the total joint motion. 4. Complications were idiopathic pain 7 cases, nerve paresis 2 cases, superficial infection 2 cases, intraoperative fracture I case, dislocation I case, loosening 1 case.

만성 족근 관절 외측 불안정성 환자에서 $Brostr{\ddot{o}}m$ 변형 술식의 치료 결과

Purpose: This study was performed to evaluate clinical and radiographical results of treatment of chronic lateral ankle instability with modified Brostrom procedure. Materials and Methods: This is a retrospective study of 22 cases in 22 patients treated with modified $Brostr{\ddot{o}}m$ procedure under the diagnosis of chronic lateral ankle instability from May 2000 to August 2004. Average age was 32.3 years. Average follow-up period was 23 months. Preoperative and postoperative radiographs of ankle anteroposterior view, lateral view and varus stress view were analyzed. The clinical evaluation was performed according to the Americal Orthopaedic Foot and Ankle Society scales. Results: After the modified $Brostr{\ddot{o}}m$ procedure, Americal Orthopaedic Foot and Ankle Society scales score was improved by average 28 points. Among the 22 cases, 13 cases were excellent, 3 cases good, 4 cases fair, and 2 cases poor with satisfaction of 73%. Five cases had intermittent ankle pain of whom 3 cases had difficulty during competitive sports activity. Lateral tilting of talus improved by average 5.4 degrees on varus stress view. Conclusions: The modified $Brostr{\ddot{o}}m$ procedure is one of the most effective methods for treating of chronic lateral ankle instability.

내고정한 슬개골 골절에서 슬와공을 통한 조기 파절된 강선 조각의 슬와부로의 이동 - 증례보고 -

슬개골 골절의 고정 방법에 있어 강선 고정술은 흔히 사용되는 수술 방법중 하나이다. 슬관절의 조기 재활을 위해서는 견고한 고정술이 필요하다. 일반적으로 슬개골 골절의 수술적 내고정술 후 장기간 제거하지 않을 경우, 강선의 파절이 흔히 발생하며, 파절된 강선 조각은 관절막 외부의 연부조직 내에서 이동한다. 그러나, 저자들은 슬개골 골절의 수술 후 조기 파절된 강선 조각이 관절강내의 슬와공을 통하여 슬와부로 이동한 64세 남자를 관절경적 및 관혈적 방법을 이용하여 성공적으로 치료하였으며, 이에 문헌고찰과 함께 보고하고자 한다. Wiring techniques in the fixation of patellar fracture are commonly used. Rigid fixation is essential for early rehabilitation. Breakages of wires after the fixation of patellar fracture are common with extracapsular migration if not removed for long term. But, we experienced a case of patellar fracture with early breakage of wiring and migration of the broken wire fragment into the popliteal fossa through the popliteal hiatus in 64- year-old man and we successfully treated this case, using both arthroscopic and open procedures. So we report this unusual case with a review of current literatures.

전방 십자 인대 재건술 후 발생한 전경골 결절종 - 1례 보고 -

전방 십자 인대 재건술 후에 발생한 전경골 결절종은 매우 드문 합병증으로 국내외에 그 보고가 매우 적으며, 몇몇 저자들이 그 발생 시기 및 원인들이 다양하다고 보고하고 있다. 저자들은 34세 남자에서 아킬레스 동종건과 생체 흡수성 간섭나사를 이용한 관절경적 전방 십자 인대 재건술 후 발생한 전경골 결절종 1예를 치험하였기에, 그 발생 원인 및 치료를 포함하여 문헌 고찰과 함께 보고하고자 한다. Pretibial ganglion cyst after arthroscopic anterior cruciate ligament reconstruction is a very rare complication, and a few are reported in and outside the country. Some authors report that it may break out with a variety of causes at any time. We experienced a case of pretibial ganglion cyst after arthroscopic anterior cruciate ligament reconstruction using Achilles tendon allograft and bioabsorbable interference screw in 34-year-old male. So we report this case including the cause and the treatment with a review of the literature.

연부조직 육종으로 오인된 대퇴부의 결절성 근막염 - 2예 보고 -

결절성 근막염은 섬유모세포와 근모세포의 증식에 의하여 발생하는 양성 질환으로 그 크기도 보통 3 cm 이내로 작다. 이 질환은 빠른 성장, 풍부한 세포충실성 및 유사 분열과 같은 특징들을 보일 수 있어 연부조직 육종으로 오인될 수 있다. 저자들은 기존의 보고와는 달리 그 크기가 5 cm 이상으로 큰 연부조직 육종으로 오인한 2예의 대퇴부에 발생한 결절성 근막염을 치험하였다. 그 중 1예는 임상적으로 뿐 아니라 조직학적으로까지 연부조직 육종으로 오인하였으나 외부의 숙련된 병리학자들의 도움으로 결절성 근막염으로 최종 진단되었으며, 2예 모두 2년 추시관찰에서 재발의 증거가 없었다. 저자들의 예들처럼 그 크기가 일반적인 것과는 달리 5 cm 이상으로 큰 종괴성 병변인 경우에는 연부조직 육종의 감별 진단으로서 매우 중요하기에, 대퇴부에 발생한 2예의 결절성 근막염을 문헌고찰과 함께 보고하는 바이다. Nodular faciitis is generally considered to be benign proliferation of fibroblasts and myoblasts, and it measures dimension up to 3cm. The characteristics such as rapid growth, abundant cellularity, and mitotic activity occasionally cause these lesions to mimic sarcoma. The authors experienced two cases of nodular fasciitis of the thigh, which were unusually large with dimension of more than 5cm. All of these two cases mimicked sarcoma and one of two cases, which initially mimicked sarcoma clinically and histologically in our hospital, was finally diagnosed as nodular fasciitis after requesting external consultation to several experienced pathologists. All of two cases had no evidence of recurrence at 2 years postoperatively. The lesions of unusually large dimension such as in our cases must be included in the differential diagnosis of soft tissue sarcoma. So we report two cases of nodular fasciitis of the thigh with a review of the current literature.

비복근 파열 후 혈종에 병발한 광범위 심부 농양의 내시경적 치료 - 1예 보고 -

광범위 심부 농양의 치료는 일반적으로 절개 및 배농이 원칙이다. 그러나 철저한 절개배농을 위해서는 피부 절개의 범위가 커지게 된다. 또한 큰 절개에도 불구하고 좋지 않은 수술 영상 및 시야 확보가 힘든 경우가 있을 수 있어, 수술 과정에서 정상 조직에 대한 추가적인 손상과 큰 절개 반흔이 문제가 될 수 있다. 이를 해결하고자 저자들은 비복근 파열로 인한 혈종에 병발한 광범위 심부 농양에 대하여 내시경적 치료를 시행하여 만족스러운 결과를 얻었으며, 기존에 보고된 바 없기에 문헌고찰과 함께 이를 보고하고자 한다. For the treatment of deep extensive abscess, the incision and drainage is necessary. But in order to make a thorough incisional drainage we can't avoid the skin incision enlarged. Even if the incision is enlarged, it is often impossible to obtain a satisfactory view of operative field. Also, the additional damage to surrounding normal tissue during operation and the scars made by large incision may be problematic. To solve these problems, we performed the endoscopic treatment for extensive deep abscesses occurred in hematoma after rupture of gastrocnemius muscle and obtained the satisfactory results. It has not been previously described, so we report it with a review of the literature.

Effects of Annealing on the Electrical Properties of the BLT/STA/Si Structure for Ferroelectric-Gate Field-Effect Transistors

We investigated the eects of annealing on the ferroelectric characteristics of (Bi,La)4Ti3O12 (BLT) .lms deposited on Si(100) substrates with a 25-nm-thick SrTa2O6 (STA) buer layer. The STA thin .lm annealed at 900 ±C for 3 minutes showed a 5.7-nm equivalent oxide thickness (EOT). The XRD analysis indicated that BLT .lms deposited on STA/Si structures had a polycrystalline phase with a preferred (117) orientation. The crystallinity was improved at higher temperatures. The capacitance-voltage (C-V) measurements revealed that the Au/BLT/STA/Si structures showed good ferroelectric properties and that the memory window width became larger as the annealing temperature was increased. For the BLT/STA/Si structures annealed at 750 ±C, the memory window width was about 1.5 V for a bias voltage sweep of §5 V. The leakage current density was lower than 1 ￡ 10¡7 A/cm2 at 5 V. The data retention time of this structure was found to be longer than 15 hours.

Sol-Gel Deposited LaZrOx Buffer Layer for Ferroelectric Thin Films

To investigate the possibility of LaZrO$_{x}$ thin film as a buffer insulator for ferroelectric thin films, we fabricated (Bi,La)$_{4}$Ti$_{3}$O$_{12}$ (BLT) ferroelectric films on a silicon substrate by using the LaZrO$_{x}$ buffer insulator in forming the metal-ferroelectric-insulator-semiconductor (MFIS) structure. We prepared the LaZrO$_{x}$ thin film and BLT films by using a sol-gel method. The films were characterized by X-ray diffraction (XRD) analysis, atomic force microscopy (AFM), capacitance-voltage (C-V) characteristics, and leakage current density-voltage (J-V) characteristics for both LaZrO$_{x}$/Si and BLT/LaZrO$_{x}$/Si structures. From C-V measurement results, equivalent oxide thickness (EOT) values of LaZrO$_{x}$ thin films annealed at 700 $^{\circ}\mathrm{C}$ and 800 $^{\circ}\mathrm{C}$ were about 9 nm and 10 nm, respectively. Negligibly small hysteresis loops were observed for both LaZrO$_{x}$/Si structures. The leakage current density at an applied voltage of 5 V was lower than 1.0 $\times$ 10$^{-7}$ A/cm$^{2}$ for all samples. For the case of the BLT/LaZrO$_{x}$/Si structure, we obtained randomly oriented polycrystalline BLT films. From the C-V measurements, clockwise ferroelectric hysteresis loops were observed and the memory window width became larger as the bias voltage sweep increased from ${\pm}$3 V to ${\pm}$9 V. The memory window width was about 1.0 V for bias voltage sweep of ${\pm}$7 V. The leakage current density was lower than 1.0 $\times$ 10$^{-6}$ A/cm$^{2}$ at 10 V.

무지를 제외한 일측 수부에 발생한 수근중수골 관절의 골절 및 탈구 - 증례 보고 -

Fracture-Dislocations of the ulnar carpometacarpal joints is an uncommon injury. The priorities of management of other more extensive injuries often delay definitive treatment. If such a dislocation is diagnosed early, it can be reduced easily by closed means. We reviewed one case of fracture-dislocation of carpometacarpal joints excluding thumb with stable and pain free carpometacarpal joints in one year after closed reduction and internal fixation.
