연명치료 중단에 대한 태도 연구 암환자 가족을 대상으로

본 연구의 목적은 사회적으로 이슈화되고 있는 연명치료 중단에 대한 태도를 알아보고 그 태도와 관련 있는 요인들을 밝히는 것이다. 외국에서는 사전 의료지시서의 작성을 의무화하고 있어 연명치료 중단에 대한 환자의 자기결정권을 존중하고 있는 반면, 국내에서는 연명치료 중단 결정과정에서 환자는 배제되고 환자의 가족이 결정의 주체이다. 따라서 결정의 주체인 환자 가족들을 대상으로 연구를 수행하였다. 경기도 소재 ○○대학병원 종양내과 환자 가족 168명이 본 연구에 참여하였다. 본 연구에 참여한 환자 가족들의 약 70%는 환자에게 더 이상의 적극적인 치료가 어려울 경우 연명치료 중단에 동의하겠다는 태도를 보였다. 이러한 비율은 국내외에서 보고된 비율과 큰 차이를 보이지 않는 수준이다. 환자의 의료적 상태, 가족이 느끼는 경제적 부담감, 그리고 주보호자의 학력이 연명치료 중단에 대한 환자 가족의 태도와 관련 있는 요인들로 밝혀졌다. There has been much debate on the issue of withholding life-sustaining medical cares for the terminal cancer patients. This study investigates what affects the DNR (Do-Not-Resuscitate) decision in terms of family traits which might be a decisive factor for the decision in Korea. Because traditionally family members usually make the DNR decision which is considered a part of the decision on withholding life-sustaining medical cares. Thus, the goal of this study is to find out which family traits are linked to their attitude toward the DNR by seeing traits of cancer patients` family members and their attitude toward withholding life-sustaining medical cares. Furthermore, this study also show how their traits affect their attitude toward the DNR decision. To establish and verify the research which is based on these topics, this study collected data by survey research through interviewing the total number of 168 main caregivers of cancer patients who are being treated in Oncology of St. Vincent`s Hospital in Suwon. Considering family traits, the result shows that their economic burden as well as their high education can make them decide to accept DNR. This result is consistent with previous works, verifying that the feeling of economic burden is an important factor for the caregivers to make the DNR decision. This can bring up an ethical issue in the point that there is possibility for caregivers just to make the DNR decision under the pressure of the economic burden without thinking of their patients` medical condition. Therefore, evaluation and intervention of a medical social worker for their socioeconomic status should be necessary in the process of the DNR decision. And virtually, education programs and team approach should be offered to the patients and their family members for their right decision on withholding life-sustaining medical cares.
