        소규모 채혈실적 혈액원 혈액관리 업무 심사평가

        배경: 수혈은 치료방법의 하나로서 환자의 생명과 건강유지를 목적으로 실시되어야 한다. 안전한 혈액을 공급하기 위하여 주기적인 심사 평가를 통한 의료기관 혈액원의 질 향상을 도모하여야 한다. 이를 위하여 혈액관리업무의 체계적인 감시체계를 확립할 수 있는 방안을 제시하고자 한다. 방법: 연간 100단위 이하의 현혈실적이 신고된 의료기관 혈액원 29개 기관대상으로 현장심사를 실시하였다. 심사위원은 자문과 심사가 동시에 가능한 회원을 대상으로 심사위원 워크숍 과정을 이수한 인원으로 제한하였다. 결과: 혈액원 폐쇄 등의 이유로 혈액원 개설이 취소된 4개 기관을 제외한 25개 기관에 대한 심사를 시행하였다. 이중 2개 기관이 부적합으로 판정을 받았다. “해당없음”으로 평가된 항목은 심사기관별로 다양한 분포를 보였으며 주로 혈액제제 관리실 및 제조관리자 등과 같은 시설이나 인력에 대한 사항이었다. 고찰: 혈액관리업무 심사평가에 사용되는 심사평가표는 지속적으로 검토되어야 하며 정확한 심사와 심사위원간의 심사기준을 통일화 시키는 일이 우선적으로 해결되어야 할 것이다. 또한 각 문항별 심사지침 및 가이드라인을 상세히 정립하여야 한다. Background: Transfusion is a choice of treatment and should be used in order to maintain patients’ lives and health. In order to supply safe blood, the quality improvements of hospital blood collection centers should be periodically monitored. To establish systematic surveillance of blood management system, we would like to suggest proper program. Methods: Twenty-nine hospital small scale blood centers, collects less than 100 units of blood collection per year, were evaluated. Qualified were selected among blood bank specialists who were eligible to simultaneously perform inspections and consultations, and who had attended inspectors’ workshop. Results: Among twenty-five blood collection centers, four blood collection centers were closed prior to inspections. Among them, two blood collection centers were evaluated as inadequate. Among adequate centers, some questionnaires were evaluated as “not applicable” mainly for reasons pertaining to personnel and facilities, such as blood component preparation rooms and/or center administrators. Conclusion: A checklist for an inspection program should be reviewed continuously. Additionally, detailed guidelines for inspection should be standardized prior to commencing of subsequent year’s inspection program. Finally, guidelines for inspection should be established for every questionnaire.

        장거리 검체 운송이 검사결과에 미치는 영향

        Background: Accuracy of laboratory test results is an important issue. New guidelines for specimen delivery systems are needed for appropriate pretreatment of specimens and accuracy of results. Methods: We evaluated various laboratory profiles, comparing the effects of specimen rack holders and coolants within transport containers. The hematology profiles (complete blood cell count [CBC], erythrocyte sedimentation rate [ ESR]), chemistry profiles (aspartate aminotransaminase [AST], alanine aminotransaminase [ALT], gamma-glutamyl transferase [γ-GT], electrolytes [Na, K, Cl], glucose, lactate dehydrogenase [LD], creatinine kinase [CK]), and coagulation profiles (prothrombin time [PT], activated partial thromboplastin time [aPTT], fibrinogen level). We also investigated the effects of transportation time including the presence or absence of hemolyzation. We received from 9 different university hospital laboratories using conventional transporation methods. Results: Hemolytic features were seen in short drawn specimens. Fewer result variations were observed in specimens transported with coolants. Average specimen transportation time was 11.3 hours, and average temperatures of container was 10.9℃ with coolant and 25.0℃ without coolants. Non-centrifuged specimens transported with coolants showed increased serum K levels than centrifuged specimens. Coagulation tests showed less than a 10% differences. Centrifuged specimen prior to transportation showed no hemolyzation and no differences in results. Conclusions: Appropriate temperatures for each analyte should be defined to ensure the accuracy of results. To reduce hemolyzation, appropriate temperature and rack holder should be used. Temperature of the transport container should be monitored in objectively. Coagulation tests should be added as referral tests, if appropriate specimen transport monitoring system for time and temperature could be adopted.

        Prevalence of Inducible Clindamycin Resistance in Staphylococcal Isolates at a Korean Tertiary Care Hospital

        Clindamycin resistance in Staphylococcus species can be either constitutive or inducible. Inducible resistance cannot be detected by the conventional antimicrobial susceptibility test. In this study, we determined the prevalence of inducible clindamycin resistance in staphylococcal isolates at a Korean tertiary care hospital. Between February and September 2004, 1,519 isolates of Staphylococcus aureus and 1,043 isolates of coagulase-negative staphylococci (CNS) were tested for inducible resistance by the D-zone test. Overall, 17% of MRSA, 84% of MSSA, 37% of MRCNS, and 70% of MSCNS were susceptible to clindamycin. Of the erythromycin non-susceptible, clindamycin-susceptible isolates, 32% of MRSA, 35% of MSSA, 90% of MRCNS, and 94% of MSCNS had inducible clindamycin resistance. Inducible clindamycin resistance in staphylococci was highly prevalent in Korea. This study indicates importance of the D-zone test in detecting inducible clindamycin resistance in staphylococci to aid in the optimal treatment of patients.

        국내 혈청 알레르기 항원 검사법의 현황

        Background : Due to the westernization of living environments in Korea, a number of allergy patients are greatly increasing. External quality control assessments are difficult for allergy tests due to the lack of reference methods. We surveyed the current status of allergy tests performed in medical laboratories in Korea to prepare for an external quality control assessment in the near future. Methods : We conducted a survey on internal and external quality control trials, calibrations and its intervals, medical staffs, analyzers in use, an average number of tests per months, and report formats. Results : Among the 85 laboratories surveyed, 61 were doing allergy tests including 6 reference laboratories. There were two different types of analyzers (1) qualitative or semi-quantitative and (2) quantitative. These analyzers use either chemiluminescent or immunoblot method. Fifty-five laboratories were using ‘qualitative or semi-quantitative’ analyzers as a screening test and 31 laboratories were using quantitative analyzers. Most of the laboratories were only doing an internal quality control assessment included in the test kits. Excepting a few laboratories, calibrations were not done. About 34 laboratories reported numerical values with interpretative reports prepared by laboratory medical doctors. Conclusions : The necessity of external quality control assessment has become an issue for improving the quality of allergy tests. But due to the lack of standardization, it is difficult to carry out external quality control assessments. By grouping the laboratories in terms of the type of analyzers, we could overcome the problem of analyzer variations and launch an external quality control assessment program in the near future.       • HTML을 적용한 멀티미디어 정보 변환 시스템에 관한 연구

        본 논문에서는 멀티미디어 데이터 정보를 표준화된 형태의 논리적 구조에 기반을 둔 HTML 문서 형태로 전환할 수 있는 시스템을 제안한다. 하이퍼미디어 형태의 다양한 정보 데이터의 활용을 위하여 Web browser를 통한 변환 정보의 표현과 검색이 가능하도록 하며, 미디어 데이터를 표준문서 형태로 전환시킬 수 있는 전환 생성 시스템의 구성은 기존의 일반적인 데이터들과 미디어 형태로 이루어져 있는 데이터들을 바탕으로 한 상호 연동이 가능한 표현 형식을 적용하여 분산된 시스템 환경에서도 정보의 검색과 정보의 응용이 이루어질 수 있도록 하는데 연구의 목적이 있으며, 확장되어지는 HTML문서의 구문을 보다 적극적으로 처리할 수 있도록 한다. 본 논문에서 적용한 인터페이스는 멀티미디어 시스템에서 통신망을 통한 정보의 검색 및 상호 연동 환경에서 적용되어 질 수 있다.
