Objective: High blood neutrophil to lymphocyte ratio (NLR) has been reported to be associated with worse outcome in treatment of nonsmall cell lung cancer (NSCLC). However, NLR in pleural effusion of NSCLC has not been evaluated for its predictive role. Present study evaluated whether pleural neutrophil to lymphocyte ratio has clinical impact on overall survival, progression free survival (PFS) and time to malignant pleural effusion (MPE) reexpansion in NSCLC with MPE. Methods: The medical records of 108 NSCLC patients with pathologically and clinically proven MPE were reviewed. Demographic factors, tumor characteristics and laboratory parameters in both pleural effusion and blood were evaluated for their influences on overall survival, PFS and MPE reexpansion. Significance of each factors was evaluated by Cox proportional hazard regression. Results: Number of male patients was 63 (58.4%) and 91 patients (84.3%) were diagnosed with adenocarcinoma. Median survival of the patients was 10.7 months. In univariate analysis, higher pleural NLR was not a risk factor for shorter survival and MPE reexpansion. However, higher pleural NLR was significant risk factor for shorter PFS in univariate analysis, along with higher neutrophil and lower lymphocyte percentages. Nevertheless, pleural NLR was not significant in multivariate analysis. Conclusion: Higher pleural NLR was significant risk factor for shorter PFS in univariate analysis. Study with more population is necessary to elucidate the predictive role of pleural NLR.

In this article, we prove the Cohen type theorem for S-∗w-principal ideal domains, which states that D is an S-∗w-principal ideal domain if and only if every nonzero prime ideal of D (disjoint from S) is S-∗w-principal.

Objective: Patients with interstitial lung disease (ILD) who show evidence of autoimmunity without meeting criteria for a defined connective tissue disease are categorized as interstitial pneumonia with autoimmune features (IPAF). The present study compared clinical characteristics and outcomes of patients with IPAF to patients with connective tissue disease related-interstitial lung disease (CTD-ILD) and patients with idiopathic pulmonary fibrosis (IPF). Methods: ILD patients who were consecutively enrolled in a single institution ILD cohort between 2008 and 2015 were evaluated. Results: Out of 306 patients with ILD, 54 patients met the classification of IPAF, 175 patients had IPF, and 77 patients were diagnosed of CTD-ILD. The proportion of males (35.2%) in the IPAF group was low when compared to the IPF group. Mean age and the proportion of ever smokers were significantly different between the three groups (p<0.001 and p<0.001, respectively). Among 54 IPAF patients, 34 (63.0%) patients showed nonspecific interstitial pneumonia (NSIP) pattern on HRCT. Among 34 IPAF patients who underwent lung biopsy, 12 patients showed histopathologic pattern which were coherent with morphologic domain of IPAF criteria. Compared to IPF, IPAF patients showed longer time to first acute exacerbation (P=0.122). From multivariate analysis for mortality, IPAF was independently associated with better overall when compared to IPF (P=0.039). Conclusions: The IPAF group showed distinct clinical characteristics and better survival when compared to IPF group.
UPFC를 포함한 송전성에서의 전류차동 보호 방식의 개선

본 논문에서는 대표적인 유연송전기기인 UPFC(Unified Power flow Controller)를 대상으로 UPFC 운전이 송전계통에 미치는 영향을 분석하여 송전선 보호를 위한 전류차동방식을 개선하였다. UPFC가 포함된 송전계통을 대상으로 UPFC의 여러 운전 모드에 대한 동적 해석을 통하여 UPFC 동적 특성이 송전선 보호를 위한 전류차동 계전방식에 미치는 영향을 분석하였다. 이를 바탕으로 UPFC가 운전하고 있을 때의 고장을 효과적으로 판단할 수 있도록 개선된 전류차동 보호방법을 제안하였고, 제안된 방법의 타당성을 시뮬레이션으로 입증하였다. The objective of this paper is to analyze power system dynamics influenced by UPFC(Unified Power flow Controller) operation and to develop a refined DCRM(Differential Current Relaying Method) to protect the transmission line with UPFC effectively. The implementation of control strategies for UPFC introduces new power system dynamic problems that must be considered while applying the conventional DCRM. In this paper, impact of UPFC operation on the DCRM has been reviewed and a refined DCRM has been proposed to detect faults properly in spite of UPFC operation. The porposed method is verified by simulation on the line-faulted system with UPFC.

Objective: Air trapping, expressed as increased residual volume (RV)/total lung capacity (TLC) is a risk factor associated with mortality and frequent exacerbations of COPD. Present study evaluated longitudinal change in clinical parameters of the COPD patients who were stratified by pattern of changes in RV/TLC over 3 years. Methods: Patients enrolled in the Korean Obstructive Lung Disease (KOLD) study cohort from June 2005 to October 2015 were evaluated. The study patients were spirometrically diagnosed with COPD. RV/TLC, an air trapping index was considered abnormal when it was≥40. Results: Total of 279 patients were evaluated. According to pattern of change in RV/TLC, patients were categorized into 4 groups: 76 in “normal to normal” (N→N), 34 in “abnormal to normal” (A→N), 33 in “normal to abnormal” (N→A) and 136 in “abnormal to abnormal” (A→A). After 3 years, the N→A group showed significant increase in expiratory/inspiratory ratio of mean lung density (MLD), and showed the most increase in emphysema index (EI), while the A→N group showed the decrease in EI. From Kaplan meier curve analysis, A→A showed the shortest overall survival (P=0.009).In 3rd year, the N→A group showed the highest proportion of frequent exacerbators, which was relatively higher than A→A group (27.3% vs 25.7%). The COPD phenotype according to RV/TLC change was independently associated with exacerbation during 1st year. Conclusion: Patients with COPD whose RV/TLC changed from normal to abnormal showed increase in expiratory/inspiratory MLD, EI and frequency of exacerbations.
