ASC-H 환자의 p16INK4a 면역세포화학염색 검사의 유용성

Background: The grey zone of cervical cytology, and in particular atypical squamous cells, cannot exclude HSIL (ASC-H) causes diagnostic difficulties and increases medical expenses. We analyzed p16INK4a expression in ASC-H liquid-based cytology specimens (LBCS) to develop more effective methods for the management of ASC-H patients. Methods: We carried out p16INK4a immunostaining with 57 LBCS of ASC-H diagnostic categories, all of which were histologically cofirmed and 43 cases of which were compared with the results of a human papillomavirus (HPV) chip test. Results: p16INK4a immunostaining with ASC-H LBCS was positive in 20% (3/15) of cervicitis, 25.0% (3/12) of tissue-low-grade squamous intraepithelial lesion, 75.0% (18/24) of tissue-high grade squamous intraepithelial lesion (HSIL), and 100% (6/6) of invasive cancer cases. The positivity of p16INK4a in LBCS was correlated with higher grade of histologic diagnosis (r=0.578, p=0.000). The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of p16INK4a immunostaining for the prediction of tissue-HSIL+ were 80.0%, 77.8%, 80.0%, and 77.8%, respectively. The sensitivity, specificity, PPV, and NPV of p16INK4a immunostaining plus HPV chip test for predicting tissue-HSIL+ were 71.2%, 86.4%, 84.2%, and 79.2%. Conclusions: p16INK4a immunostaining as well as HPV chip testing with remaining LBCS with ASC-H are useful objective markers for the prediction of tissue-HSIL+.

The research for speech quality improvement of the linear predictive vocoding is under way in two points of view: an accurate extraction of vocal tract parameter and a modeling of excitation signal in a speech production model. This paper proposes the new hybrid excitation model for linear predictive vocoding which gives an improved speech quality over that of a conventional excitation model, and an evident data compression effect over that of a typical multi-pulse excitation model. To verify the validity of the proposed model, a computer simulation is carried out. It is observed that the proposed excitation model in vocoding performs an improved speech quality and a higher data compression effect.
Design and Implementation of Digital Signal Processor and Development System

A real-time microprogrammable digital signal processor is designed and implemented using the bit-slice logic, a parallel multiplier, 74 series TTLs and MOS memories. A microinstruction set for the processor is defined and an application program development system is constructed. For its performance evalution, a digital filter and FFT are implemented with this digital signal processor. It is proved that this processor is faster than commrcially available single chip digital signal processors such as \ulcornerD 7720, AMI 2811, enabling very high speed digital signal processing.
Image Analysis of HER2 Immunohistochemical Staining of Surgical Breast Cancer Specimens

Purpose: Trastuzumab is an effective treatment for human epidermal growth factor receptor 2 (HER2)-amplified breast cancers. We sought to develop a simple protocol for HER2 image analysis of breast cancer specimens. Materials and Methods: In a preliminary test, we found that at least 1000 tumor cells need to be examined in the most stronglystained areas. Next, we evaluated the clinical usefulness of this established protocol of image analysis in 555 breast cancer patients. Results of the HER2 immunohistochemical (IHC) staining were compared between manual scoring and image analysis. Results: The HER2 IHC results obtained by the image analysis method correlated well with those obtained by the manual scoringmethod (Cohen’s kappa=0.830). Using the HER2 silver in situ hybridization (SISH) results as a gold standard, sensitivity valueswere 72.1% for manual scoring and 74.0% for image analysis; specificity values were 96.2% for manual scoring and 94.7% for imageanalysis; and accuracy values were 91.7% for manual scoring and 90.8% for image analysis. McNemar’s test was applied to theresults, and there were no statistically significant differences in sensitivity and specificity between the positive (p=0.688) and negative(p=0.118) SISH groups. Conclusion: HER2 image analysis results were similar to those obtained via the manual scoring method, indicating that the useof image analysis can reduce assessment time and effort. We suggest that image analysis-based evaluation of 1000 tumor cells inthe most strongly IHC-stained area, regardless of stroma content, is sufficient for determining HER2 expression levels in breastcancer specimens.
폐신문지(廢新聞紙)(ONP)로부터 부유선별(浮遊選別)을 이용한 잉크분리(分離)시 초음파처리(超音波處理) 효과(效果)

재생률이 높은 폐신문지(old newspaper, ONP)를 부유선별법을 이용하여 탈묵을 진행하였을 시, 해리시간, 초음파처리에 따른 백색도, 잔류잉크농도(effective residual ink concentration, ERIC), 수율의 효과를 규명하였다. 또한 부유선별시 pH, 포수제에 따른 백색도, ERIC과 수율의 효과도 규명하였다. 해리시간이 증가할수록 백색도와 ERIC이 증가함을 보였으나 수율의 차이는 없었다. 해리시간이 30분일 때 백색도 58% ERIC 182.9 ppm이였고, 해리를 더 진행하더라도 백색도의 증가를 볼 수 없었다. 단 해리 된 ONP를 초음파발생장치를 이용하여 초음파처리를 했을 경우 단 2분의 해리시간으로도 58%이상의 백색도를 얻을 수 있었다. 초음파처리를 20분간하고, pH 11에서 포수제로 등유을 사용했을 때 백색도가 59.32%로 가장 높았다. In this study, the sonication method was applied to remove ink from old newspaper(ONP) using flotation. Results indicated increased whiteness and effective residual ink concentration (ERIC) with longer disintegration. Therefore 58% of whiteness and 182.9 ppm ERIC were obtained for 30 minutes of disintegration time without the change in yield. Longer disintegration than 30 minutes had no effects on whiteness without the application of ultra-sonication. On the other hand, higher than 58% of whiteness was achieved in 2 minutes of disintegration when the ultra-sonication was applied. The highest whiteness (59.32 %) was obtained by applying sonication for 20 minutes and froth flotation with kerosene as a collector under the alkaline condition (pH 11).
