Purpose: Open fractures of the foot and ankle require prompt repair of the wound due to the complexity of anatomy, insufficiency of soft tissues and inadequate blood supply. Early flaps and skin grafts are used for this purpose yet general condition of the patient as well as local wound environment often precludes such treatment options. Vacuum- Assisted Closure (VAC) is recently being used in such cases. This study was done to validate the use of VAC together with silver antimicrobial dressing materials in contaminated open fracture wounds. Materials and Methods: We have selected 10 patients with Gustillo-Anderson type III open fractures of the foot & ankle treated with VAC and silver antimicrobial dressing materials from March 2007 to January 2008. The relationship between duration of treatment with wound size, contamination, and degree of soft tissue damage was analyzed. Results: The average age of patients was 36.6 years. The average amount of VAC application time was 23.4 days. Silver dressing materials were used for 16.8 days. Average wound healing time was 51.9 days. Statistically significant relationship was found between wound size, VAC application time and silver dressing material application time. No complications such as osteomyelitis were found after treatment. Conclusion: VAC technique is recently being used in open fractures with wide skin and soft tissue defects, producing good results. A wide array of dressing materials such as silver dressing is in development. We have incorporated the VAC technique together with silver dressing materials in the treatment of open fractures and achieved complication free results.
한국인 남자에서 Synapsin III 유전자의 D22S280 표지자와 정신분열병의 연합연구

Objectives : Synapsin III near VCFS region on chromosome 22q affects. It could be an interesting candidate gene for schizophrenia. D22S280 is a highly polymorphic genetic marker residing in synapsin III. We examined association of D22S280 marker on synapsin III with Korean patients with schizophrenia. Methods : The subjects were 46 male Korean patients with schizophrenia and 60 male normal controls. Using polymerase chain reaction, gel electrophoresis, ABI 310 genetic analyzer, and GeneScan Collection 3.1 software, we confirmed genotypes of D22S280 marker. We examined Hardy-Weinberg equilibrium and case-control association using SAS/Genetic 9.1.3. Results : Genotypes of both schizophrenia and control groups were in Hardy-Weinberg equilibrium. We could not find any significant statistical differences in allele-wise(${\chi}^2$=10.4, df=6, p=0.098) and genotype-wise (${\chi}^2$=22.1 df=19, p=0.258) analyses of D22S280 marker between schizophrenia and normal controls. Individual allele analyses with df=1 showed significant differences in A1(p=0.025) and A7(p=0.034) allele, which were not significant following Bonferroni corrections(A1:p=0.177, A7:p=0.235). Conclusion : We couldn't find any association between schizophrenia and the synapsin III gene. Given the small number of subjects studied, further investigations are needed.

층응답스펙트럼(FRS)은 통상 원자력 발전소 내에 설치된 기기를 평가하기 위해 사용된다. 관례적으로 사용되는 집중질량모델에서 층응답스펙트럼을 작성하면 층의 모든 응답이 간략화된다. 하지만 실제로는 서로 다른 위치에서의 개별 응답 간에는 차이가 존재하며, 이는 기기의 취약도에 상당한 영향을 미칠 수 있다. 본 연구에서는 이러한 특성을 고려하여 3-D 유한요소 모델을 통해 다른 위치에서 층응답스펙트럼을 생성하여 비교하였다. 그 결과 동일 층 내에서도 무시할 수 없는 변동성이 존재함을 보였다. 앵커 파괴에 대한 취약도 해석을 수행하여 위치에 영향을 받는 층응답스펙트럼의 변동성에 의해 고신뢰도저파괴확률(HCLPF)에도 편차가 발생함을 확인하였다.

The floor response spectrum (FRS) is used to evaluate the seismic demand of equipment installed in nuclear power plants. In the conventional design practice of NPP structure, the FRS is simplified using the lumped-mass stick model (LMSM), assuming the floor slab as a rigid diaphragm. In the present study, to study the variation of seismic response in a floor, the FRSs at different locations were generated by 3-D finite element model, and the response was compared to that of the rigid diaphragm model. The result showed that the FRS significantly varied due to the large opening in a floor, which was not captured by the rigid diaphragm model. Based on the result, seismic fragility analysis was performed for the anchorage of a heat exchanger, to investigate the effect of location-dependent FRS disparity on the high confidence low probability of failure (HCLPF).
