        Purpose: Chronic urticaria is a common disease, but clinically, it is not easy to treat and predict the prognosis since the causes and pathophysiology of chronic urticaria remain unknown. Many studies have been done that defined the association between chronic urticaria and thyroid autoimmunity. However, the clinical role of antithyroid antibodies (ATAs) has not been fully evaluated. Methods: We retrospectively reviewed the medical records of patients with chronic urticaria and compared the duration of treatment, the frequency of steroid use, and the level of medications used in the treatment of urticaria according to the presence or absence of ATAs including the antithyroglobulin (anti-TG) antibody, antithyroid peroxidase antibody, and antithyrotropin-stimulating hormone receptor antibody. Results: A total of 194 patients with chronic urticaria was enrolled; of which, 108 patients were free of ATAs and 86 patients had at least one ATA. The treatment duration was significantly longer in the anti-TG antibody-positive patients compared to the patients without ATAs (39.6±10.2 months/18.0±3.4 months, P=0.02). The patients with the anti-TG antibody also showed more frequent steroid use than that of the patients without the anti-TG antibody (2.1±0.4/1.1±0.3, P=0.05). The level of medications used to achieve control of urticaria tended to be higher in patients with the anti-TG antibody than in the ATA-negative patients group. Conclusion: From this study, we suggest that the evaluation of thyroid autoantibodies, especially the anti-TG antibody, can be a useful tool in predicting the prognosis and making decisions for the treatment strategy of patients with chronic urticaria.         핵융합 공정주기에서의 생산 계획 최적화

        본 연구에서는 화학공정 최적화에 사용되는 생산계획최적기법을 도입하여 핵융합 공정에서의 삼중수소 재고량을 최소로 유지하는 수학적 모델을 구축하여 최적 운전 시나리오를 도출하였다. 핵융합 발전을 위한 공정 중 연료주기 공정(fuel cycle)은 반응연료인 중수소와 삼중수소를 저장 하고 공급하는 시스템과 핵융합 반응 배가스로부터 이를 회수 및 분리하는 세부 공정들로 구성되어 있다. 이들 공정들은 삼중수소가 방사성 물질이라는 것을 제외하면 대부분 촉매반응과 분리공정으로 이루어져 화공플랜트에 적용된 기술과 유사한 특성이 많아 화학공정에 사용되는 스케줄링 기법을 통해 최적 운전 시나리오를 도출 가능하다. 본 연구에서는 핵융합로의 다양한 장치의 특성을 반영해서, 펌프내부의 삼중수소량을 최소로 하는 최적 재생주기를 구하고, 구해진 최적 재생주기 결과를 반영하여 후단의 트리튬 플랜트에서의 최적 운전 시나리오를 확인해 보았다. 구축된 모델은 실제 토카막 시나리오에 적용되어 ITER 연료주기 내 공정의 연료흐름 및 밸런스 분석에 활용되었다. In this study, a mathematical model for optimal operation of the fusion fuel cycle is developed based on scheduling approach. The fusion fuel cycle consists of a system for storing and supplying deuterium and tritium, and receiving and separating process after the fusion reaction. Except that tritium is a radioactive material, most of these processes consist of catalytic reactions and separation process. For these reasons, it is possible to apply scheduling approach which is also widely utilized to chemical plants to derive the optimal operating scenarios. The developed model determined the optimal regeneration cycle to minimize the amount of tritium inside the vacuum pumps. Based on the characteristics of various device in the fusion reactor, the optimal tritium plant operation scenario is evaluated. The formulated model was applied to the actual tokamak scenario and utilized to analyze the fuel flow and balance of ITER fuel cycle.

        설문조사를 통한 국내 유전성 혈관부종 진료 현황 및 국내 사례 문헌고찰

        Purpose: Hereditary angioedema is a familial disease which is caused by a genetic deficiency or functional defect of the C1 inhibitor, and it features episodic swelling that can affect any part of the body. A great number of patients are estimated not to have an accurate diagnosis after the onset of symptoms, and close attention is required because sudden hereditary angioedema attacks can result in even death. Methods: We sent an e-mail questionnaire to 975 members of the Korean Academy of Asthma, Allergy and Clinical Immunology. A total of 82 members replied. The questionnaire, including 15 questions about the diagnosis and management of hereditary angioedema, was developed by the anaphylaxis/urticaria, angioedema workgroup of the Korean Academy of Asthma, Allergy and Clinical Immunology. Results: Forty-two percent of the respondents had experience with treatment of a suspected case of hereditary angioedema, and 15.9% made a confirmed diagnosis of hereditary angioedema. When the respondents suspected of cases, 91.4% of them performed tests for C3 and C4 concentrations and C1 inhibitor level. For maintenance treatment, most of the respondents used androgen, and only 22% found that C1 inhibitor concentrates can be prescribed through the Korea Orphan Drug Center in Korea. Conclusion: Allergy physicians in Korea substantially recognized the correct diagnosis and treatment of hereditary angioedema. However, there was a lack of awareness for the latest treatments, such as C1 inhibitor concentrates. Education of doctors and the public is needed.

        As the use of chemotherapeutic agents increased rapidly in recent years, more patients are under the potential risk of chemotherapy related adverse reactions. Multiple regular exposures to the same drug by chemotherapy protocol may increase the risk of sensitization to a chemotherapeutic agent, which can result in hypersensitivity reactions. Once severe hypersensitivity reactions occur, causative drugs should be avoided. However, a substitute with equal efficacy is not always available. When there is no effective alternative, desensitization is a safe tool for maintenance of chemotherapeutic agents causing hypersensitivity reaction. In this review, we introduce the latest knowledge about desensitization protocol for chemotherapeutic agents which are frequently used recently.

        Background: Fluorescein angiography is the standard tool for the diagnosis of vitreoretinal diseases. Fluorescein sodium has been widely used for decades. Although its adverse reactions are mostly non-immunologic and mild, fatal reactions, such as anaphylaxis, occur rarely. Case History: A 64-year-old woman experienced erythema on her face and neck, dyspnea, hypotension and loss of consciousness immediately after injection of fluorescein for angiography. Her symptoms recovered after intravenous infusion of fluid along with injection of epinephrine, steroid and antihistamine. Results: The skin prick test showed a positive reaction to fluorescein sodium. Conclusion: Based on her symptoms and the skin prick test result, the patient was diagnosed with fluorescein-induced anaphylaxis. (Korean J Asthma Allergy Clin Immunol 2011;31:223-225)

        전기자동차 충전스테이션 서비스디자인에 관한 연구

        환경부는 2020년까지 수송부문에서 이산화탄소 배출량을 전망치 대비 34.3% 감축을 계획했고, 전기차 100만대 보급 및 공공?민간 충전인프라 확충을 목표로 하며 전기차 대중화에 나섰다. 그러나 일반차량 유지비용의 1/20이라는 비용절감 효과에도 불구하고 제대로 갖춰지지 않은 충전인프라 때문에 대다수의 이용자들이 전기차 구매를 쉽게 결정하지 못하고 있다. 이처럼 전기차는 충전인프라 의존도가 매우 높으며, 전기차 보급을 위해 필수적으로 충족되어야 하는 사항이다. 현재 정부의 주도로 민간주도의 전기차 보급에 노력하고 있다. 앞으로 완성차업계 등의 민간 기업의 참여가 기대되면서 이용고객 중심의 전기차 충전인프라 구축이 이루어질 것이라고 예상한다. 이에 본 연구에서는 향후 구축될 전기차 충전인프라 의 만족스러운 서비스디자인을 제공 할 수 있도록 개선방향을 모색하는 데 목적이 있다. 이러한 목적에 대한 결과를 얻기 위해 서울시내 7개소의 전기차 충전스테이션 이용과정에서 일어나는 이용자의 행동을 관찰하였다. 관찰에서 얻어진 이용자의 잠재적 요구와 터치포인트 요소를 Kano모델에 적용하여 전기차 충전스테이션에 대한 고객여정지도를 작성하고, 서비스블루프린트로 구체화하여 30항목을 도출하였다. Kano 설문법을 통해 30항목을 6가지 속성에 따라 분류 후 고객만족계수를 산출하여 고객들의 서비스품질에 대한 만족 정도를 알아보았다. 분류된 매력적 요구사항(A) 7항목, 일차원적 요구사항(O) 7항목의 개선방향에 대한 서비스디자인을 제안하였다. 본 연구는 현재 부족한 충전서비스에 구체적인 서비스 디자인을 고민하고 발전시켜 나아가야 한다는 관심을 촉구했다는 점에서 나름 의미가 있다고 생각되며 향 후 구축될 전기차 충전스테이션 서비스디자인 개선에 도움이 되기를 기대한다. The Ministry of Environment has estimated 34.4% reduction in carbon dioxide emissions by 2020. To prepare for this situation, they have prepared one million electric cars and targeted public dissemination to promote electric vehicles. Even though electric vehicles will reduce 1/20 maintenance costs compared to general vehicles, majority of the consumers are still struggling to make the decision to purchase due to lack of electric charging stations. With that being said, charging stations are extremely important for electric cars and it is necessary to build more for the availability to the public. Currently, the government is leading to supply more of the private sector’s electric vehicles. We’re expecting to see higher development of charging stations as major automotive companies participate to serve the consumers. From this study, we’re hoping to see more charging stations and hope to provide satisfactory design and user experience to the public. To better the user experience and gather more information, we observed user behavior in 7 charging stations throughout Seoul. From the observations, we’ve obtained user’s needs on potential touch point and elements and applied in Kano model to create charging station map and derived 30 points for the service blueprint. Kano method survey of properties over 30 items classified according to six factors. Then calculated the customer satisfaction, service quality, customer satisfaction degrees were evaluated. The classification included attractive 7 requirements(A), immediate 7 requirements(O), for the improvement of service and designs. The purpose of this research is to show the lack of charging services and contemplate many issues to better the customer need and hope to improve design and services.
