Can the Acoustic Rhinometry Reflect the Real Volume Change of the Nasal Cavity?

Background and Objectives:Acoustic rhinometry (AR) measures nasal cavity geometry by analyzing reflected acousticimpulses. The authors aimed to find out whether AR could reflect the volume change developed from conchotomy. Materialsand Methods:To establish the test-retest reliability of the AR, 20 normal nasal cavities were tested with AR beforeconducting main study. The volumes of the 31 conchotomy specimens were measured with water displacement method(WDM). The nasal volume changes in accordance with conchotomy operations were measured with AR, and the paired valueswere compared. Results:AR revealed highly consistent results as there was statistically significant correlation between testand retest values (r=0.98, p<0.0001). The volume of the conchotomy specimens measured with WDM was 1.40±0.63 cm3(mean±SD) and the volume change measured with AR was 1.49±1.48 cm3 (mean±SD). There was statistically significantcorrelation between the two values (r=0.55, p<0.01), though they were not so consistent with each other. Conclusion:Thenasal volume change after conchotomy measured with AR correlates with the conchotomy specimen volume with statisticalsignificance, though the correlation between them does not always show consistency.
문재인 정부의 공공부문 일자리 정책 담론에 대한 분석- 공무원 증원을 둘러싼 국가개입 논쟁을 중심으로

In the government of Moon Jae-In, the government became a direct employer and created 810,000 public jobs including the recruitment of 174,000 new public officials through government financial input, thereby causing social benefits such as solving youth unemployment problems and raising economic growth rate. However, in this process, the government-led public sector job creation policy has been controversial about the opposition to the 'big government' and the appropriateness of national intervention in job creation. Therefore, this study aims to understand the overall landscape of job creation in the public sector more deeply by paying attention to the controversy surrounding the contents and characteristics of national intervention in the policy of 'expansion of the amount of jobs' promoted by the Moon Jae-In government. As a result of analyzing the adequacy of the expansion of public sector jobs through national intervention, it can be seen that the perception of national intervention is formed in a relative and complex manner in the creation of public sector jobs in Korean society, and that national intervention is symbolized by various values and cultural codes in the creation of public sector jobs. In addition, it was found that there is a need to maintain a balanced view on job creation in the public sector and to secure objective evidence for positive or negative results. 문재인 정부는 2022년까지 17.4만 명의 신규공무원 채용을 포함한 공공부문 일자리 81만 개를 창출함으로써 청년실업문제의 완화와 경제성장률 제고 등과 같은 사회적 편익을 유발하고자 하였다. 하지만 공무원 증원을 둘러싸고 ‘큰 정부’에 대한 우려 및 국가개입의 적절성 문제가 제기되었다. 이에 본 연구에서는 문재인 정부에서 추진하고 있는 ‘일자리의 양 확대’ 정책에 있어서 공무원 증원을 둘러싼 논란에 주목함으로써 정부 주도의 공공부문 일자리 창출에 대한 전체적인 지형을 보다 심층적으로 이해하고자 하였다. 국가개입을 통한 공공부문 일자리 확충의 적절성 논쟁에 대한 분석결과, 우리 사회에서 공무원 증원을 통한 공공부문 일자리 창출에 대한 인식이 상대적이고 복합적으로 형성되어 있으며, 공공부문 일자리 창출에 있어서 국가개입과 관련된 논란은 다양한 가치와 상징들을 반영하고 있음을 알 수 있었다. 또한 공공분야 고용창출에 대한 균형된 시각과 긍정적 또는 부정적 결과에 대한 객관적 근거 확보 노력이 지속적으로 이루어져야 할 필요성이 있음을 발견하였다.
시험관내 항원 투여가 알레르기성 비염 환자의 비점막내 호산구 침윤과 접착분자의 발현에 미치는 영향

We aimed to investigate the effect of in vitro allergen challenge on activation of eosinophils and expression of intercellular adhesion molecule-1(ICAM-1) in the epithelial and endothelial cells of the nasal mucosa of allergic rhinitics. Materials and methods . Inferior turbinate mucosal specimens which were obtained from 16 perennial allergic rhinitics and sensitized to D. farinae during conchotomy operations were used. Each mucosal specimen was divided into six pieces. The first two pieces were used as basic control group without culture, the second two pieces as culture-control group which was cultured with diluent only, and the last two pieces as antigen-challenge group cultured with D. farinae solution for 24 hours. Specimens were immunohistochemically stained using monoclonal anti- body against eosinophil cationic protein(EG1 for all eosinophils and EG2 for activated forms) and ICAM-1. Results .' Average counts of EG1 cells and EG2+ cells within 1 mm of lamina propria of nasal mucosa were 14 and 11 respectively in the basic control group, 17 and 19 in culture control group, and 16 and 16 in the antigen-challenge group. ICAM-1 expression on vessels was observed more than half of t,he vessels in the nasal mucosa and there was no significant difference between groups, however, its expression on the epithelial cells was most prominent in the antigen-challenge group (2.1 in basic control group, 3.3 in culture control group, and 6.2 in antigen-challenge group). Conclusion: Most of the eosinophils infiltrat,ing into the nasal mucosae out of vessels in allergic rhinitics are activated forms. Significant up-regulation of epithelial ICAM-1 expression by antigen-challenge suggests that ICAM-1 contributes to the transmigration of eosinophils from the subepithelial tissue into the epithelial cell layer and then finally into the nasal secretion.
