        속목정맥 도관 삽입에서 새로운 해부학적 표지자로 쇄골의 유용성

        Objective: The ipsilateral nipple has been used as a directional guide for the insertion of a conventional central venous catheter via the internal jugular vein (IJV). However, the nipple is not an ideal marker since the position of the nipple varies among the characteristics of patients. The utility of the clavicle was investigated for IJV catheterization instead of the ipsilateral nipple through cadaver dissection. Methods: A total of 14 cadavers were dissected (six fresh cadavers, eight fixed cadavers). Each cadaver was kept in its supine position, and its head was rotated 30。to the left. Length A was the distance between the jugular notch and the point that the line from the nipple to sternocleidomastoid muscle apex meets the clavicle. Length B was the distance between the jugular notch and the clavicle point that passed the IJV. IJV was cut and its circumference measured at regular intervals. Results: Lengths A and B were 3.79±0.35 cm (mean±standard deviation) and 3.71±0.32 cm, respectively, and were significantly correlated (r=0.865, P<0.001). The mean circumference of the vessels were measured to be 3.56±1.29 cm in the right IJV and 3.00±1.32 cm in the left IJV with a significant difference (P=0.013). Conclusion: IJV catheterization via the right one, larger than the left one, is appropriate for the first attempt, and we recommend that the needle should be pointed at the clavicle located at a width of two fingers (2nd, 3rd finger average width 1.9 cm) away from the jugular notch.

      • 초임계 유체를 이용한 두 종류의 항암제가 함유된 고분자 미립자 제조

        Doxorubicin (DOX) is an anthracycline drug commonly used in the treatment of a large spectrum of tumors. It works by interfering with the growth of cancer cells. It binds and intercalates into the DNA strand, thus inhibiting further DNA and RNA biosynthesis, eventually causing cell death. Gemcitabine (GEM) is a synthetic pyrimidine nucleoside analogue with a high antitumor efficacy for ovarian, breast, bladder, and pancreatic cancer. It replaces one of the building blocks of nucleic acids, in this case cytidine, during DNA replication. The process arrests tumor growth, as new nucleosides cannot be attached to the faulty nucleoside, resulting in apoptosis. The purpose of this study is to prepare biodegradable poly(L-lactic acid) microparticles containing two different drugs by a supercritical aerosol solvent extraction system (ASES) process. The effects of operating parameters such as temperature, pressure, and drug composition on the morphology and characteristics of microparticles were studied in detail. The entrapment efficiency and in vitro release profiles of two anticancer drugs from the microparticles were also determined using HPLC. The entrapment efficiency increased with decreasing temperature. The entrapment efficiency of DOX was found to be higher than that of GEM, mainly due to larger molecular weight of DOX. On the other hand, GEM showed higher solubility in aqueous solution, thereby resulting in a faster release compared with DOX.

      • 계층적 분석과정을 이용한 고무장갑 품질요인 평가에 관한 연구

        본 연구는 고무장갑 품질요인 평가에 대한 연구로 먼저, 불량요인을 내구성불량과 외관불량으로 구분하고, 각 요인에 관계된 불량종류를 분류하였다. 이를 바탕으로 불량종류 별로 세부 불량요인 항목을 고무장갑 생산 공정을 고려해 도출하였다. 제시된 품질(불량)요인이 불량종류에 미치는 영향을 평가하기 위해 국내 중견 고무장갑기업을 대상으로 수집된 실증자료를 바탕으로 AHP분석을 수행하였다. 고무장갑에 대한 국내외 연구가 매우 미진한 상태이기 때문에 본 연구의 결과는 국내 고무장갑 생산업체의 효율적인 품질관리 체계 및 프로세스 정립에 기여할 것으로 판단된다.

      • Gemcitabine-PLLA 컨쥬게이트의 합성과 초임계 유체를 이용한 Gem-PLLA 미립자 제조

        Nano- and microparticulate drug delivery systems have widely been studied to achieve selective targeting of tumor cells through the enhanced permeability and retention effect. Conventional methods used to prepare drug-loaded polymeric nano- and microparticles include dialysis, emulsion/solvent evaporation and micelle technique. However, these methods generally have some limitations such as low drug loading efficiency, low drug distribution, and broad particle size distribution. In this study, to increase the loading efficiency of gemcitabine, an anti-tumor agent, gemcitabine-poly(L-lactic acid) (PLLA) conjugates were synthesized through an amide linkage reaction of N-hydroxysuccinimide with triethylamine. Then, gemcitabine-PLLA conjugate particles were prepared using a supercritical fluid process, called aerosol solvent extraction system (ASES) and their mean particle size and distribution as well as drug release profiles were investigated in detail. The FT-IR and 1H-NMR spectra confirmed that gemcitabine-PLLA conjugates were prepared successfully. The conjugation yield of Gem-PLLA varied significantly with the PLLA molecular weight. The Gem-PLLA particles obtained from ASES process showed a non-spherical shape and their mean size was varied from nano- to micro-scale depending on the PLLA molecular weight. No significant amount of gemcitabine was released in a phosphoric acid buffer solution of pH 7.4 up to 120 h at 37oC.

        Doxylamine 중독 후 발생한 횡문근융해증 4예

        Doxylamine is common over-the-counter sleep preparations & frequently involved in overdoses. The clinical course is dominated by the anticholinergic effects, including central nervous system & autonomic effects. We report 4 cases of suicide attempts in adults where ingestion of the doxylamines were complicated by rhabdomyolysis. They ingested doxylamines variable amount & were carried to emergency department. They complained gastrointestinal or central nervous system symptoms. Gastric lavages & administrations of activated charcoal were done. Creatine phosphok inase levels were normal or markedly elevated on arrival, but peaked several days later. Serum creatinine levels were normal. 99mTc-MDP bone scans were showed increased muscle labelling at the regions of muscle injury. They were treated with hydration, urine alkalinization, & supportive measures in hospital. On considering cause of rhabdomyolysis, our patients did not show any evidence of viral illness or coingestion of other potential myopathic toxins to support a secondary cause of rhabdomyolysis. The mechanism of rhabdomyolysis in cases of doxylamine overdose seems to be a direct toxic effect of the drug on striated muscle, but the exact mechanism is not clear. In all cases where such overdoses are suspected, consideration should be given to obtaining a urinalysis & a creatine phosphokinase level on arrival & creatine phosphokinase levels are carefully followed. Primary detoxication included gastric lavage & administration of activated charcoal. The patient's urine output & renal function should be closely monitored.

        원발성 위 림프종에서 전환된 Burkitt 림프종 세포 백혈병 1 예

        Burkitt's lymphoma is a distinct clinical entity classified as an undifferentiated lymphoma of B-lymphocytic origin. Burkitt's lymphoma cell leukemia occurs when Burkitt's lymphoma cells represent more than 20% of marrow cells and exceed about 5000/ul in blood. We report a case of Burkitt's lymphoma cell leukemia in primary gastric lymphoma in 55-year-old man with 5kg of weight loss for 2 months and epigastric pain for 3 days. Primary gastric lymphoma was diagnosed by biopsies with gastrofiberoscopy and upper abdominal CT scan in this patient. During admission, peripheral blood smear revealed sudden increase in abnormal blasts with multiple vacuoles. Burkitt's lymphoma cell leukemia was diagnosed by peripheral blood, bone marrow aspiration and biopsy, immunophenotyping and cytogenetic analysis in this patient. For its great rarity, we report this case with review of literature.

        위와 회장말단부에 발생한 Hodgkin 씨병 1예

        Cases with extranodal Hodgkin's disease located in sites outside those defined as lymphatic structure are quite rare. The exact incidence of this localized form of Hodgkin's disease is unknown, but it is estimated to be only 0.25% for overall incidence of Hodgkin's disease. Localized primary extranodal Hodgkin's disease seems to have an excellent prognosis in some cases including skin, tongue, lung, stomach, small bowel, and thyroid. We report a case of primary gastrointestinal Hodgkin's disease involved in the stomach and terminal ileum in 48-year-old man with 7 kg of weight loss for 3 months, fever with chills for 1 week, and intermittent epigastric pain for 1 year. The histologic type of this case was lymphocyte predominance, which was diagnosed by biopsy of the stomach and terminal ileum with immnophenotyping study. And the stage of this case was IIE. We tried combination chemotherapy with adriamycin, bleomycin, vinblastin, and dacabazine with excellent response. For its rarity, we report this case with review of literatures.

      • 초임계 유체를 이용한 미세조류 바이오디젤의 직접 생산

        In this study, we attempted to evaluate the feasibility of a supercritical methanol process for direct production of biodiesel from microalgae. The effects of various factors such as temperature, reaction time, water content and pretreatment on the efficiency of biodiesel conversion were investigated. The conversion efficiency increased with both temperature and extraction/conversion time. The supercritical methanol process produced a fatty acid methyl ester (FAME) yield of around 88% within 20 minutes. The optimal conditions for this process were found to be a microalgae/methanol ratio of around 1:9, a reaction temperature of about 270℃, and a reaction time of approximately 20 minutes. The effect of water content in microalgae samples on the FAME conversion was also evaluated. The supercritical methanol process was not affected by the water content up to 250 wt%. Our experiments demonstrated that the supercritical methanol process is more effective compared with conventional in situ transesterification processes.
