전기 비트겐슈타인의 윤리적 가치와 행복에 대한 상대주의적 해석

본 논문에서는 윤리학에 관한 전기 비트겐슈타인의 입장을 윤리적 상대주의로 해석하고자 한다. 윤리적 가치에 관한 전기 비트겐슈타인의 입장을 윤리적 절대주의로 이해할 경우 윤리적 문장과 윤리학의 무의미성에 대한 전기 비트겐슈타인의 주장을 액면 그대로 받아들여야 한다. 코라 다이아몬드는 바로 이런 관점에서 무의미에 관한 엄격한 이론을 주장한다. 그러나 필자가 보기에 이는 질병의 원인은 그대로 놔둔 채 질병의 증상만 치료하려는 방법으로 보인다. 비트겐슈타인이 윤리학을 무의미하다고 본 이유는 윤리적 가치의 절대성에 있기 때문이다. 따라서 필자는 전기 비트겐슈타인의 윤리적 가치에 대한 입장을 상대주의적으로 해석할 경우 윤리학의 유의미성을 살리면서 동시에 전기 비트겐슈타인의 입장도 좀 더 일관되게 해석할 수 있다고 주장할 것이다. 특히 전기 비트겐슈타인 철학에 나타난 언어의 절대주의적 사용에 대한 비판, 유아론, 그리고 행복론을 근거로 전기 비트겐슈타인의 윤리관을 상대주의적으로 해석할 수 있다고 주장할 것이다. In this paper, going against the main trend, I offer a relativist interpretation of the early Wittgenstein s view of ethical values. If we give an absolutist interpretation, we will have to take Wittgenstein’s claims on ethics at face-value, i. e., the claims that ethical sentences are nonsensical, and there is no subject called ‘ethics’. Cora Diamond, best known for her new, resolute reading of Wittgenstein’s Tractatus, adopts this line of approach. According to Diamond, the early Wittgenstein literally wants readers to take ethical sentences as a collection of nonsense. In my view, however, her approach is wrongheaded, because she fails to give due attention to the real cause of the nonsensicality of ethical sentences. According to Wittgenstein s diagnosis, it is the absolutist view of ethical values which is responsible for the nonsensicality of ethical sentences. This, I argue, leaves us room for interpreting the early Wittgenstein’s view of ethics from a relativist perspective. I also try to defend a relativist interpretation drawing on Wittgenstein’s views of solipsism and happiness.

Cis-A2B3 표현형을 보이는 환자의 수혈경험

We report the case of a 64-year-old man presenting to the hospital for treatment of his anemia. Exact ABO blood typing is an essential step to prevent transfusion reactions. The selection of the wrong blood component for transfusion can be a clinical problem and in this case the patient had a cis-AB blood type that could have caused an ABO discrepancy. In this case neither autologous or directed blood transfusion was possible and O+ red blood cell was transfused without a transfusion reaction. (Korean J Blood Transfus 2010;21:154-157) Cis-AB 혈액형은 일반적 ABO 혈액형 검사에서 결과의 불일치로 인하여 임상적인 오진을 야기하는 원인이 되며, 적합한 수혈용 혈액제제 선택에 문제를 야기할 수 있다. 그러므로 정확한ABO 혈액형 진단이 수혈 부작용을 예방하는 가장 근본적인 방법이 된다. 저자들은 빈혈 치료를위하여 농축적혈구 수혈이 의뢰된 64세 남자환자의 혈액형 검사 결과 cis-AB형이었다. 이 환자에대한 자가수혈과 지정수혈이 불가능하여 O형 농축적혈구 수혈을 수혈부작용 없이 경험하였기에문헌고찰과 함께 보고하는 바이다.

Objectives : The purpose of this study was to examine the regional cerebral blood-flow differences between children with pure ADHD and normal controls, and associations between blood-flow abnormalities and cognitive characteristics in children with ADHD Method : The cerebral blood-flow of fourteen children with pure ADHD and ten normal controls were examined by Tc-99m ECD SPECT and cognitive charateristics in children with pure ADHD were measured by ADS. First, regional blood-flow differences between two groups were analyzed by using SPM. Second, associations between types of cognitive characteristics measured by ADS and cerebral regions showing significant difference were examined, and quantitative associations between severity of cognitive impairments and degree of blood-flow decrease were analyzed by using Pearson's coefficient correlation Results : 1) The cerebral blood-flows of children with ADHD were significantly decreased in left orbitofrontal area and left superior temporal gyrus compared to normal controls. 2) There were tendency of positive correlation between the degree of commission errer(impulsivity, disinhibition) among the ADS variables and the degree of blood-flow decrease above two areas Conclusion : These results suggest that the pathophysiology of impulsivity of disinhibition in the characteristic symptoms of ADHD is associated with hypoactivities of left orbitofrontal area and left superior temporal gyrus.

간호사 경력단계에 따라 간호역량이 직무만족에 미치는 영향

Purpose: This study examined the variations in nursing competence and job satisfaction across different career stages. Additionally, it explored the effect of nursing competence on job satisfaction within each career stages. Methods: Participants were 468 nurses working at a university hospital in B city. Data collection was conducted using self-report questionnaires from April 1 to 15, 2020. The data were analyzed descriptive statistics, one-way ANOVA, Scheffé test, and multiple regression analysis employing SPSS 23.0. Results: General nursing competence improved significantly after the proficiency stage. Care execution and critical thinking were improved as nurses progressed from the novice to the advanced beginner stage. Additionally, communication showed progress in the competent stage. Furthermore, responsibility and self-development were improved after the proficiency stage. Among the sub-competencies examined, self-development, care execution, and communication significantly influenced the job satisfaction of a nurse’s career. Moreover, self-development was influential at all stages. Conclusion: This study shows that there was no substantial improvement in nursing competencies as they progressed through various career stages. Self-development was the most crucial sub-competence impacting job satisfaction, with the effects of the sub-competencies varying across career stages. These results will aid in planning clinical continuing education and formulating policies to enhance nurse competency. 목적: 본 연구는 간호사의 간호역량과 직무만족이 경력단계에 따른 차이와 간호역량이 직무만족에 미치는 영향을 확인하기 위해 시도되었다. 방법: 본 연구의 대상은 B 지역 대학병원에 근무하는 468명의 간호사이다. 자료는 자가보고식 설문을 이용하여 2020년 4월1일부터 15일동안 시행되었다. 수집된 자료는 SPSS 23.0를 이용하여 기술통계, 일원분산분석, 사후검정, 그리고 다중회귀분석을 실시하였다. 결과: 전반적 간호역량은 숙련단계 이후에 유의하게 향상되었다. 간호수행, 그리고 임상적 비판적 사고는 상급초보단계에서 초보단계보다 향상되었으나, 의사소통/협력은 유능단계에 이르러 유의한 진전을 보였고, 책임감/윤리, 자기계발은 숙련단계에 이르러 다른 단계보다 높게 나타났다. 하위역량 중 자기계발은 모든 단계에서 직무만족에 영향을 미쳤으며 간호수행은 상급초보단계를 제외한 단계들에서 직무만족에 영향을 미쳤다. 그러나 상급초보단계에서는 자기계발, 의사소통/협력이 직무만족에 영향을 미치는 것으로 나타났다. 결론: 본 연구의 결과는 간호역량이 경력단계별로 유의미한 진전을 보이지 않았고, 하위역량 별로 진전의 속도가 다른 것으로 나타났다. 경력단계에 따라 직무만족에 영향을 미치는 하위역량이 달랐는데 모든 단계에서 가장 큰 영향을 미치는 것은 자기계발이었다. 본 연구결과는 에서 간호교육과 임상계속교육 계획과 간호사 역량증진을 위한 정책 수립에 도움이 될 것으로 생각된다.

By the treatment of herb-med, acupunture and Korean(韓方)- medicine, symptoms of nasal polyp has taken a turn for the better, but nasal polyp itself was removed in completely was imppssible. Althogh a surgical operation of western -medicine, the nasal polyp tend to recur. In China, the new treatment that Chinese(漢方)-medicine and western-medicine combined was tried. The trial was effective in decreasing recurrance rate and treatment period. But it's a case of completely removed Nasal polyp through the more vanous Korean(韓方)-medicine, The patient answered in the affinnative of electric acupunture, nasal bloodletting, vaporization, external application against the existing treatment(herb-med, acupunture, moxa). If electric acupunture, nasal bloodletting, vaporization, external application are receiving careful study, they are a help to nasal polyp and opthalmologic, otolaryngologic disease.

리스(Jean Rhys)의 후기단편소설 「선구자, 오 선구자여와 「수상한 상황」 : '위치지울 수 없는’ 이방인의 존재를 통해 드러나는 제국 시민의 내면

Jean Rhys's many short stories, published in the 1960s and 1970s, are set in the turn of the century and the early 20th century when the British empire had spurted to educate the British people to embody its imperial ideology. Among them, “Pioneers, Oh Pioneers” and “Fishy Waters,” included in Sleep If Off, Lady (1976), capture the moment that British colonists living in Roseau of Dominica get isolated and deteriorated inside their soul. The moment is revealed through colonists's encounter with strangers such as Mr Ramage in “Pioneers, Oh Pioneers” and Jimmy Longa in “Fishy Waters.” Zygmunt Bauman explains in Modernity and Ambivalence that strangers are, in principle, unplaceable in between the subject and the other. Thus strangers are the ones that can question the very principle of the opposition of the subject and the other which should be clearly placed under the social norms of the European colonialists. This paper examines how the British colonists presented in these two stories respond to these strangers. Rhys portrays the white colonial society through Rosalie's point of view, a little girl aged 9, to reveal its blindness while applying the detective narrative in “Fishy Waters” to expose its doubleness. The narrative of each story interestingly reveals the monstrosity of British colonists in the process of investigating hybrid and seemingly monstrous strangers. In doing so, Rhys criticizes the British imperialism by disclosing the way in which the intolerance of anything different and unplaceable lead people to violence and blindness.

2007-2011년 경기 북부 지역 병원에서의 요 배양에 대한 고찰

Background: Bacteria that cause urinary tract infections (UTIs) are found with different frequencies in different regions; moreover, antibiotic susceptibility can also vary by region. We retrospectively studied and compared the species and antimicrobial susceptibility of bacterial pathogens isolated from patients with UTIs in the northern Gyeonggi-do area. Methods: We analyzed urine specimens collected from patients who visited the Myongji Hospital between 2007 and 2011. The urine specimens were cultured, and bacteria were identified by biochemical examination with an API kit (bioMerieux Inc., USA). Antimicrobial susceptibility was determined by the disc diffusion method and the Vitek II system (bioMerieux Inc., USA). Results: A total of 11,818 (31.4%) urine specimens were culture positive. The most common species identified were Escherichia coli (37.1%), Klebsiella pneumoniae (7.4%), Enterococcus faecium (6.1%), and Candida spp. (5.5%). The proportion of isolates producing extended-spectrum β-lactamases significantly increased during the study period. Conclusions: E. coli, K. pneumoniae, and E. faecium were the 3 most common organisms identified. Of note, however, was the increasing frequency of Pseudomonas spp. and Proteus spp. isolated during the more recent years. Further studies are required from other centers in the northern Gyeonggi-do area. 배경: 요로감염의 원인균은 지역적 특성에 따라 빈도가 다양하고항균제감수성 결과가 다르게 나타날 수 있다. 이에 저자들은 요로감염의 원인균과 항균제감수성 결과를 후향적으로 연구하였다. 방법: 경기도 북부에 위치한 명지병원에서 2007부터 2011년까지의뢰된 요 배양을 대상으로 하였다. 요검체를 접종 후 배양하였으며, 균동정은 생화학검사와 API 키트로 시행하였고, 항균제감수성검사는 디스크 확산법 및 Vitek II 시스템(bioMerieux Inc., USA)을이용하였다. 결과: 총 11,818 (31.4%)검체에서 양성이 확인되었으며 가장 빈도가 높은 것은 Escherichia coli 37.1%, Klebsiella pneumoniae 7.4%,Enterococcus faecium 6.1% 그리고 효모형 진균 5.5% 순이었다. Extended-spectrum β-lactamase 생성 세균은 연구기간 동안 증가하였다. 결론: E. coli, K. pneumoniae와 E. faecium이 본 연구에서는 가장 흔한 요로감염의 원인균이었다. Pseudomonas aeruginosa와Proteus spp.는 연구 기간 동안 꾸준히 증가되는 추세였다. 경기도북부의 다른 기관과의 연관 연구가 더 필요하다.

악성 위종양의 신경학적 합병증으로 발현된 연수막암종증 1예

Leptomeningeal carcinomatosis, an important and serious neurological complication of malignant cancers, is associated with severe disability and high mortality. It is most commonly seen in patients with breast cancer, lung cancer and malignant melanoma. However, leptomeningeal carcinomatosis induced by gastric cancer has been rarely reported, although gastric cancer is one of the most common causes of cancer-related deaths in Korea. We report a case of a 76- year-old woman with leptomeningeal carcinomatosis presenting as a neurological complication of gastric cancer and confirmed by cytology examination of the cerebrospinal fluid and brain magnetic resonance imaging with gadolinium enhancement. 연수막암종은 악성 종양의 매우 심한 신경학적 합병증으로서 일상생활의 기능저하를 초래하며, 높은 사망률을 보이는 질환이다. 연수막암종은 유방암, 폐암, 그리고 악성 흑색종에서 주로 보이는 합병증이다. 하지만 연수막암종은 악성 위종양의 합병증으로는 거의 보고되고 있지 않다. 하지만 본 저자들은 악성 위종양으로 진단받은 76세 여자 환자가 뇌자기공명 사진과 척수액 세포진검사로 인해 연수막암종으로 확진된 증례를 경험하였기에 보고하고자 한다.

金石範文學における「朝鮮的なもの」

在日朝鮮人文學には「朝鮮的なもの」が多く表われている. 金石範は、この「朝鮮的なもの」を作品に反映すべきだと主張していたにもかかわらず、多くの作家の作品と比べ、「朝鮮的なもの」の描き方に不自然さが感じられる. それは、他の作家の作品には、在日朝鮮人の有り樣が私小說的な描寫のなかで「朝鮮的なもの」が自然に表われるのに對し、私小說的な表現を回避した金石範の作品には、主人公とあまり關わりのない「朝鮮」が表われているからである. つまり、金石範の作品における「朝鮮的なもの」は說明する形で表現されているため、他の作家の作品より不自然に感じられるのである. 彼が「朝鮮的なもの」を表現すべきだというのは、日本語で作品を書きながらも「作家としての主體」「朝鮮人としての主體」を失わないために必要な重要な表現手段である. 彼が、このような意識を持つようになったのは、朝鮮を植民地化し、抑壓した戰前の日本で生まれ、また民族意識が高まった時期に、「組織」の仕事に携わろうと拘ったことなどが絡んでいたためであろう. 金石範はこのような事情から日本語で作品を書くためには「朝鮮人」、そして「作家」の主體性の確立が必要だと考えられた. 金石範は虛構·想像的な形式を通して「朝鮮的なもの」を表現し、その作品には、濟州島を中心に朝鮮の風習や慣習などが分かりやすく記されているのが分る. 「朝鮮的なもの」の表現として朝鮮の文化、祭祀やシャ-マニズムが取り入れられている. 祭祀は日本において「朝鮮」を味わうことのできる唯一の空間である. 彼の作品には、祭祀の供え物や祭儀などについて細かく說明されている. 男性主人公の女性への眼差しは、儒敎的な考え方に立つ男性支配的イデオロギ-、男性優位主義をベ-スとして表われるものだと言えよう. 在日朝鮮人社會における朝鮮の文化を受け繼いではいるのは「女性」と言えるが、とりわけ、金石範文學において「女性」の役割は大きく、「朝鮮」に繫げたいという氣持ちが作品に色濃く描かれていることが分かる.
