Background: Histone deacetylase 1 (HDAC1) is associated with the expression and function of estrogen receptors and the proliferation of tumor cells, and has been considered a very important factor in breast tumor progression and prognosis. Several studies have reported an association between HDAC1 expression and poorer prognosis in cancers including breast cancer, with a few exceptions. However, because of the dearth of studies on HDAC1 expression in breast cancer, its significance for breast cancer prognosis has not been well defined. Therefore, we examined HDAC1 expression in invasive ductal carcinoma (IDC), the most common breast cancer, and investigated its potential prognostic significance. Methods: We used 203 IDC tissue samples. Immunohistochemical stains for HDAC1 and real-time polymerase chain reaction for HDAC1 mRNA were performed and the results were compared to generally well-established prognostic factors in breast cancer and patient survival rates. Results: HDAC1 expression was significantly reduced in proportion to higher histologic grade, higher nuclear pleomorphism score, and higher mitotic counts, and with lower estrogen receptor expression. Furthermore, it was significantly associated with the survival rate. Conclusions: HDAC1 expression is a good prognostic indicator in IDC.

Cases of acute myocardial infarction (AMI) that occur during pregnancy or postpartum are rarely reported. Ergot derivatives are known to induce the spasmodic contraction of coronary arteries. Administration of ergot derivatives can cause AMI, even in normal healthy people. In several reported cases, ergot derivatives triggered severe AMI during the postpartum period. Here, we report the case of a forty-year-old woman who was successfully impregnated by artificial fertilization and died after treatment with ergot derivatives. The autopsy revealed AMI with severe coronary atherosclerosis. This is the first case that reports aggravation of pre-existent severe coronary atherosclerosis after postpartum infusion of ergot derivtives.
ROS1 Expression in Invasive Ductal Carcinoma of the Breast Related to Proliferation Activity

Purpose: ROS1 is an oncogene, expressed primarily in glioblastomas of the brain that has been hypothesized to mediate the effects of early stage tumor progression. In addition, it was reported that ROS1 expression was observed in diverse cancer tissue or cell lines and ROS1 is associated with the development of several tumors. However, ROS1 expression has not been studied in breast cancer to date. Therefore,we investigated ROS1 expression at the protein and gene level to compare expression patterns and to verify the association with prognostic factors in invasive ductal carcinoma (IDC) of the breast. Materials and Methods: Tissue samples from 203 patients were used. Forty-six cases were available for fresh tissue. We performed immunohistochemical staining and real-time polymerase chain reaction (PCR). Results: ROS1 expression was significantly lower in proportion to higher histologic grade, higher mitotic counts, lower estrogen receptor expression, and a higher Ki-67 proliferation index, although ROS1 expression was not significantly associated with the survival rate. The result of real-time PCR revealed similar trends, however not statistically significant. Conclusion: Higher ROS1 expression may be associated with favorable prognostic factors of IDC and its expression in IDC is related to the proliferation of tumor cells.
No Detection of Simian Virus 40 in Malignant Mesothelioma in Korea

Background: Simian virus 40 (SV40), a polyomavirus, was discovered as a contaminant of a human polio vaccine in the 1960s. It is known that malignant mesothelioma (MM) is associated with SV40, and that the virus works as a cofactor to the carcinogenetic effects of asbestos. However, the reports about the correlation between SV40 and MM have not been consistent. The purpose of this study is to identify SV40 in MM tissue in Korea through detection of SV40 protein and DNA. Methods: We analyzed 62 cases of available paraffin-blocks enrolled through the Korean Malignant Mesothelioma Surveillance System and performed immunohistochemistry for SV40 protein and real-time polymerase chain reaction (PCR) for SV40 DNA. Results: Of 62 total cases, 40 had disease involving the pleura (64.5%), and 29 (46.8%) were found to be of the epithelioid subtype. Immunostaining demonstrated that all examined tissues were negative for SV40 protein. Sufficient DNA was extracted for real-time PCR analysis from 36 cases. Quantitative PCR of these samples showed no increase in SV40 transcript compared to the negative controls. Conclusions: SV40 is not associated with the development of MM in Korea.
폐의 염증성 근섬유모세포종(염증성 섬유육종) -1예 보고-

Inflammatory myofibroblastic tumor, also known as inflammatory fibrosarcoma, has been frequently diagnosed as inflammatory pseudotumor. Although there are six cases reported as inflammatory pseudotumors or inflammatory myofibroblastic tumors in the lung, no cases of pulmonary inflammatory myofibroblastic tumor with features of inflammatory fibrosarcoma have been reported in Korea. We experienced a case of inflammatory myofibroblastic tumor (inflammatory fibrosarcoma) characterized by high cellularity, severe nuclear pleomorphism, necrosis, or increased mitotic counts. A 31-year-old male patient with a solitary pulmonary nodule on the routine chest x-ray received a right lower lobectomy. The tumor was an ovoid solid mass with multifocal necrosis, showing diffuse irregular proliferation of spindle cells with high cellularity and focal nuclear pleomorphism, admixed with dense lymphoplasmacytic cells. Although spindle cells are focally immunoreactive for smooth muscle actin, the ultrastructural examination failed to demonstrate smooth muscle differentiation. In cases of inflammatory myofibroblastic tumor (inflammatory fibrosarcoma), a complete excision and close follow-up without radical surgery, radiation, or chemotherapy are needed.
재발성 정맥혈전증을 유발한 Antithrombin III와 Protein C 복합 결핍증 1예

Primary venous thrombosis caused by deficiency or qualitative abnormality of antithrombin III, protein C and protein S is usually inherited as an autosomal dominant trait. Usually, deep vein thrombosis or pulmonary thromboembolism is developed by such abnormalities, however, mesenteric vein thrombosis is rarely reported. A 27-year-old man with previous history of deep vein thrombosis underwent segmental resection of jejunum due to mesenteric vein thrombosis complicated by necrosis of jejunum. Postoperative investigation disclosed combined deficiency of antithrombin III and protein C. His son also showed deficiency of antithrombin III. Postoperatively, he is on life-long warfarin therapy without experiencing recurrence of venous thrombosis.(Korean J Med 62:570-574, 2002) Antithrombin III, protein C, protein S 결핍증에 의한 원발성 정맥 혈전증은 상염색체 우성으로 유전되는 질환이다. 심부 정맥 혈전증이나 폐 색전증 등이 이러한 원인질환에 의해 발생할 수 있으며 장간막 정맥혈전증은 비교적 드물게 보고되었다. 본 증례는 과거력상 하지 심부 정맥 혈전증으로 혈전 제거술을 시행 받았던 27세 젊은 남자에서 재발한 장간막 정맥 혈전증의 증례이다. 환자는 장간막 정맥 혈전증에 의한 소장 괴사로 응급 장 절제술을 시행 받았다. 수술 후 혈전증의 원인을 찾기 위해 시행한 검사에서 antithrombin III와 protein C가 결핍되어 있었고 환자의 아들에서도 antithrombin III가 결핍되어 있었다. 이 환자는 혈전증이 재발된 상태였고 장괴사로 인한 응급수술이 필요했던 생명이 위험할 정도의 합병증을 동반한 상태였으므로, 수술 후 warfarin 요법을 시행 중이며 현재까지 약 1년 이상 정맥 혈전증의 재발은 관찰되지 않았다. 감사의 글Antithrombin 유전자의 염기서열분석 검사를 해주신 서울대학교 박선양 교수께 감사드립니다.
