        The Effect of Adding Kami-guibi-tang to Acetylcholinesterase Inhibitor Treatment on the Cognitive Function of Mild Alzheimer's Disease Patients: Study Protocol of a Randomized, Placebo-Controlled, Double-Blind Pilot Trial

        Background: Alzheimer's disease (AD) is a chronic neurodegenerative disease that causes disorientation, mood swings, problems with language, and difficulty remembering recent events. Acetylcholinesterase inhibitors (AchEIs) and memantine have been used to slow the course of the disease, but they can neither modify its progression nor prevent disease onset. Previous studies have suggested that Kami-guibi-tang (KGT) could be beneficial for supporting cognitive function in AD patients, but few clinical trials have been published. This pilot study aimed to evaluate the effect of KGT in improving cognitive function in AD patients. Methods: The study will be a randomized, placebo-controlled, double-blind, single-center trial conducted using subjects diagnosed with mild AD by neurologists. Study subjects will be randomly assigned to either a treatment or control group. The treatment group will receive KGT granules for 24 weeks, while the control group will receive placebo granules. AchEI administration will be maintained in both groups during the entirety of the study. Subjects will be assessed using the following exams: the Seoul Neuropsychologic Screening Battery (SNSB) for cognitive function; brain magnetic resonance imaging (MRI) for brain metabolite, neurotransmitter, and cerebral blood flow (CBF) measurements; the Korean version of Quality of Life-Alzheimer's Disease (KQol-AD) for quality of life; the Caregiver-Administered Neuropsychiatric Inventory (CGA-NPI) for neurobehavioral symptoms; blood tests for amyloid and tau proteins and general blood parameters; and electrocardiography (ECG) before and after taking the medication. Discussion: Our findings will provide insight into the feasibility of large-scale trials to consolidate evidence for the efficacy of KGT for dementia treatment.         Experiment on the effect of Artemisia sieversiana extract on hair loss prevention and cell growth

        Objectives: This study aimed to examine the safety, effects on proliferation of hair papilla cells, and anti-inflammatory and antioxidant mechanisms of Artemisia sieversiana Ehrh. ex Willd. (AS) extract. Methods: Safety tests through purity testing, acute toxicity tests, and repeated toxicity tests were performed using AS extract (ASE) which had been dried for over two years. Cell culture and proliferation tests were conducted; VEGF (vascular endothelial growth factor), bFGF (basic fibroblast growth factor), and EGF (epidermal growth factor) and protein expression analyses were performed for mechanistic evaluation; and inhibitory effects of ASE on the RNA expression of testosterone, 5α-reductase, and aromatase was assessed. The anti-inflammatory and antioxidant efficacy of ASE was confirmed by measuring the levels of nitric oxide, inflammatory mediators (TNF-α and PGE2), inflammatory cytokines (IL-1β, IL-6, and IL-8), and chemokine MCP-1. Results: The safety of ASE was confirmed. The mechanism of cell proliferation in human hair follicle dermal papilla cells involved the promotion of VEGF, bFGF, and EGF expression. ASE decreased mRNA expression of testosterone, 5α-reductase, and aromatase-1 in a concentration-dependent manner. PGE2 and TNF-α production by inflammatory mediators was also significantly decreased in a concentration-dependent manner, and inflammatory cytokine and chemokine expression was inhibited. Conclusions: ASE is suggested to promote papillary cell growth at the cellular level, to suppress expression of various enzymes involved in hair cycle and cell death, and to inhibit hair loss through anti-androgen, anti-inflammatory, and antioxidant effects.

        임상한의학 실시간 비대면교육의 평가를 위한 설문조사 연구

        Objectives: This survey research was conducted to evaluate real-time non-face-to-face education in clinical Korean medicine conducted during the COVID-19 outbreak. Methods: This study was conducted with all 3rd and 4th grade students of the Department of Korean Medicine, Gachon University. The Korean Internal Medicine (Pulmonary) lecture course was held from April 1st to July 1st, 2020 for one semester. The lectures were conducted through real-time non-face-to-face education using the Cisco Webex program, and the lectures were mainly conducted by sharing a PowerPoint summarizing textbook. The questionnaire consisted of 3 parts and a total of 18 questions. The survey was conducted anonymously to ensure the confidentiality of all responses. Results: The overall score for real-time non-face-to-face education was 74.69±18.15 points. The efficiency of real-time non-face-to-face education was 2.12±0.78 points, and opinions on the expansion of future use were 2.08±0.91 points and were generally positive. The ratio of non-face-to-face education and face-to-face education in future clinical Korean medicine theory lectures was about 63:27, with a higher proportion of non-face-to-face education. Conclusions: Promoting clinical Korean medicine as a non-face-to-face education has generally been positively evaluated in terms of efficiency and overall, and its use should be expanded in the future. In particular, it will be necessary to use non-face-to-face education in the theory classes of clinical Korean medicine. However, technical problems involving internet, video, equipment, and communication tools will need to be improved in the future.

        특발성 파킨슨병 환자에 대한 한의치료의 효과 : 후향적 연구

        Objectives: The purpose of this study was to investigate the effects of Korean medicine on patients with idiopathic Parkinson’s disease.Methods: The charts of 47 patients diagnosed with idiopathic Parkinson’s disease in the Department of Internal Korean medicine, Stroke and Neurological Disorders Center, Kyung Hee University Hospital, Gangdong between August 2012 and July 2016 were reviewed. The Unified Parkinson’s disease rating scale (UPDRS) was administered before and after treatment with Korean medicine.Results: Thee average UPDRS Ⅱ+Ⅲ, UPDRS Ⅱ, and UPDRS Ⅲ of the 47 patients decreased significantly from 22.26±15.15, 10.19±7.53, and 12.06±8.35, respectively, pretreatment to 16.96±13.63, 7.47±6.44, and 9.49±7.73, respectively, post-treatment. The average postural instability and gait difficulty (PIGD), tremor, and bradykinesia also significantly improved post-treatment.Conclusions: These results provide evidence that Korean medicine can improve the activities of daily living and motor function of patients with idiopathic Parkinson’s disease. In particular, Korean medicine may be effective for the treatment of PIGD, tremor, and bradykinesia.

        범불안장애를 호소하는 귀비온담탕 가미방 치험 1례

        Objective: This clinical study reports on the effect of traditional Korean medicine on a patient with generalized anxiety disorder.Method: A patient who suffered from generalized anxiety disorder was treated with herbal medicine (Guibiondamtang-gami), acupuncture, moxibustion, and pharmacopuncture. We assessed the changes of anxiety-related symptoms with the state-trait anxiety inventory (STAI) score, the Beck anxiety inventory (BAI) score, a heart rate variability (HRV) test, and a second derivative of photoplethymogram waveform (SDPTG) test.Results: After taking Guibiondamtang-gami for 22 days, the patient’s STAI score, BAI score, and LF/HF (low-frequency/high-frequency heart rate variability) ratio decreased, while the differential pulse wave index and stress power in the SDPTG test increased.Conclusion: This clinical case study has demonstrated that traditional Korean medicine may be an effective option for treating generalized anxiety disorder.

        한국과 중국의 코로나-19 진료지침의 한약에 대한 비교분석 및 고찰

        COVID-19 is an acute infectious disease caused by SARS-CoV-2 that has infected more than 700 million people around the world and left many deaths. In Korea and China, Korean medicine and Traditional Chinese Medicine were used to treat COVID-19, and the key element between them was herbal medicine. Clinical guidelines for herbal medicine have been published in Korea and China. In this review, the Korean and Chinese guidelines were compared and analyzed. This review was based on the latest revised clinical guidelines. The Korean guidelines covered all three guidelines published by The Association of Korean Medicine and The KM Professor Council of Internal Medicine of the Respiratory System. The Chinese guideline was published by the China National Health Commission. Based on research trends, a comprehensive comparison was made of the suggested prescriptions for each severity of COVID-19. It is expected that this review will be used as a reference for the treatment of COVID-19 or new infectious diseases in the future.

        IP기반 차세대 모바일 네트워크에서 사용자 이동패턴에 기반한 QoS 보장기법

        미래의 무선 시스템은 사용자가 이동하며 생활하는 것을 지원해야 한다. 이동성은 매우 다른 특성을 가진 여러 개의 중첩 네트워크의 사용을 통해 제공 될 것이다. 이러한 네트워크는 오늘날의 인기 있는 데스크탑 서비스인 웹 브라우징, 쌍방향 멀티미디어 및 모바일 화상회의 등의 원활한 제공을 지원해야 한다. 따라서 이러한 모바일 시스템 설계의 주요 과제 중 하나는 서비스 품질(QoS) 보장이 될 것이며 이것은 애플리케이션이 다양한 네트워킹 인프라 아래에서 요구하는 것에 따라 보장해야 한다. 우리는 응용 프로그램에 QoS 보장을 제공하는 자원의 예약 및 적응 기법을 사용할 필요가 있다. 그러나 전체 서비스 예약 및 사전 구성은 지나치게 많아서 결과가 매우 비효율적이고 신뢰할 수도 없는 방식이다. 이것을 극복하기 위해 사용자의 이동성 패턴을 이용할 수 있다. 사용자의 움직임을 미리 알고 있을 경우, 예약 및 구성 절차는 사용자가 방문할 가능성이 있는 네트워크의 영역으로 제한할 수 있다. 제안한 Proxy-UMP기법은 다른 기법들에 비해 탐색비용의 증가에 민감하지 않으며, SMR(Session Mobility Ratio)증가에 따라 총비용의 증가율이 적은 것을 보여준다. Future wireless systems will be required to support the increasingly nomadic lifestyle of people. This support will be provided through the use of multiple overlaid networks which have very different characteristics. Moreover, these networks will be required to support the seamless delivery of today’s popular desktop services, such as web browsing, interactive multimedia and video conferencing to the mobile devices. Thus one of the major challenges in the design of these mobile systems will be the provision of the quality of service (QoS) guarantees that the applications demand under this diverse networking infrastructure. We believe that it is necessary to use resource reservation and adaptation techniques to deliver these QoS guarantee to applications. However, reservation and pre-configuration in the entire service region is overly aggressive, and results in schemes that are extremely inefficient and unreliable. To overcome this, the mobility pattern of a user can be exploited. If the movement of a user is known, the reservation and configuration procedure can be limited to the regions of the network a user is likely to visit. Our proposed Proxy-UMP is not sensitive to increase of the search cost than other schemes and shows that the increasing rate of total cost is low as the SMR increases.
