Helicobacter pylori (H. pylori) infection can cause gastric dysplasia and cancer via chronic inflammatory changes in the gastric mucosa that may lead to gastric atrophy and intestinal metaplasia. Many epidemiologic studies have demonstrated the prophylactic effects of H. pylori eradication on gastric cancer, and H. pylori eradication after endoscopic resection of early gastric cancer may prevent the occurrence of metachronous gastric cancer or dysplasia. Despite insufficient data on the effect of H. pylori eradication on gastric dysplasia and cancer before endoscopic resection, some studies have shown that H. pylori eradication can induce the regression or slow the progression of some gastric dysplasia. Therefore, eradication therapy before endoscopic resection may be effective in selected cases of low-grade dysplasia. However, endoscopic resection should be considered as the first-line treatment in high-grade dysplasia or early gastric cancer owing to the potential morphologic changes to some dysplasia or cancer that can be incurred by the eradication process, which may make it difficult to perform a subsequent endoscopic procedure.

The gastrointestinal tract is the most common site of extranodal lymphoma, and more than 95% of gastrointestinal lymphoma cases are of the non-Hodgkin type. The stomach is the primary site of extranodal non-Hodgkin lymphoma, and more than 98% of primary gastric lymphoma (PGL) cases are diffuse large B-cell lymphomas (DLBCL) and mucosa-associated lymphoid tissue (MALT) lymphomas. Because PGL usually presents with nonspecific clinical symptoms and various endoscopic features, its diagnosis is mainly dependent on histopathologic findings. Chemotherapy is the first-line treatment choice for gastric DLBCL, whereas Helicobacter pylori eradication is the primary treatment for gastric MALT lymphoma. However, some cases of gastric adenocarcinoma resemble PGL endoscopically, complicating treatment decisions. There are various types of gastrointestinal lymphomas, such as natural killer T-cell, lymphomatous polyposis, mantle cell, follicular, immunodeficiency-related gastrointestinal, and Hodgkin. These can invade the small and large intestines. Although these lymphomas are rare, clinical outcomes differ among these types. Therefore, it is necessary for endoscopists to distinguish the clinical and endoscopic findings of the different lymphomas on biopsy to improve diagnostic accuracy.

A novel coupling system of capillary electrophoresis (CE) and monolithic molecularly imprinted polymer (MIP) was developed for selective and sensitive determination of sweeteners; sucrose, sucralose, and aspartame, based on an in-column and open-tube molecularly imprinted solid-phase extraction (MISPE) concentrator. The MISPE was fabricated by coating a short layer of MIP material (about 3?mm long) directly at the inlet end of the inner wall of capillaries under the irradiation of light emitting diode (LED). Compared with the conventional methods using hydrodynamic injection by MISPE fiber, the MISPE concentrator provides increase in sensitivity without significant loss of separation efficiency. In this work, LED-induced in-multiple capillary MISPE was used for extraction and determination of artificial sweeteners, sucrose, sucralose, and aspartame, from beverage samples. The results of MISPE showed good extraction ability with the enrichment ratios of >150 for sucrose and sucralose, and of >200 for aspartame with acceptable analytical performance. The reproducibility of on-line MISPE concentrator was 1.6% for sucralose and 2.7% for aspartame under the same fixed set of experimental conditions. The developed multiple MISPE system has been successfully adapted for practical application for commercial soda drink samples.

The morbidity of travelers’ diarrhea (TD) is still high. This study examined the incidence of common pathogens and characteristics of TD among Korean travelers who visited South-East Asian countries. We performed a prospective study involving 479 Korean travelers with diarrheal disease from February 2009 to April 2009 and stool samples were examined and questionnaire surveys were done after arrival. Enterotoxigenic Escherichia coli (ETEC)was found in 36.0% of TD cases, as were the following: Enteroaggregative Escherichia coli (EAEC) in 27.0%, Vibrio parahaemolyticus in 13.1%, and Norovirus in 11.5%. The detected rate of classic TD was higher in men (P = 0.007), in patients who had a shorter duration trip (P = 0.023) and in patients who drank more than 1 liter of water per day (P = 0.037). Positive stool culture rates were higher in men (P = 0.005), in hospitalized patients (P = 0.013). and in those who consumed impure water or raw foods (P = 0.033). A higher severity of disease corresponded to a significantly higher culture positivity rate (P = 0.029). We should consider the possibility of other pathogens in addition to ETEC in patients with TD who visit South-East Asia. Travelers need to educate about risk factors associated with TD.

Backgrounds : Recently, the role of pharmacists in Antimicrobial Stewardship Program (ASP) has been emphasized and its effect has been reported. However, it is difficult for pharmacists to be actively involved in Korea mostly due to limited human resources. Therefore, a pharmacy intervention program for ASP was designed in this study to determine its effects on appropriate antibiotic use. Methods : One pharmacy resident and one clinical pharmacist participated in the ASP in Seoul National University Bundang Hospital. They monitored the dosage of antibiotics, drug interactions, and premedication to prevent adverse effects such as premedication therapy of amphotericin B for 30 minutes daily using a computer program in the hospital. Retrospective chart review was conducted to compare the appropriateness of the selected antibiotic use before and after implementation of the pharmacy intervention program for ASP. Results : No significant difference was found in the proportions of patients prescribed with inappropriate antibiotics before and after the program. However, the proportion of inappropriate prescriptions was reduced from 23.8% to 9.7%. The proportion of inappropriate prescriptions in which pharmacists intervened was increased from 8.6% to 20.7%. Seventy-five percent of the interventions were conducted by two pharmacists involved in this program. Twenty-five percent of the interventions were conducted by the pharmacists not involved in this program. Conclusion : Appropriateness of antibiotic use was improved after implementation of the pharmacy intervention program for ASP. Therefore, ASP could be effectively applied in Korean hospitals.
Steepest Descent Method를 이용한 정전계 문제의 해석

Steepest Descent Method를 정전계 문제의 해석에 적용하였다. 반복법과 moment method와 같은 direct method와의 차이점을 기술하였다. 반복법을 이용하여 정확한 해에 단조적으로 수렴하며 대형 system의 해석에 적합함을 보였다. 정전계 문제에 대해서 해석한 수치결과를 제시하였으며 moment solution과 잘 일치하고 있음을 보였다. The method of steepest descent is applied to the analysis of electrostatic problems. The differences between iterative method and direct method, e.g. the method of moments, are not lined. It is shown that this method converges monotonically to the exact solution and is suitable for solving a problem of large system. Numerical results are presented for electrostatic case which show a good agreement with momet solution.

Purpose: The aim of this study was to investigate new point-connecting measurements for the hallux valgus angle (HVA) and the first intermetatarsal angle (IMA), which can reflect the degree of subluxation of the first metatarsophalangeal joint (MTPJ). Also, this study attempted to compare the validity of midline measurements and the new point-connecting measurements for the determinationof HVA and IMA values. Materials and Methods: Sixty feet of hallux valgus patients who underwent surgery between 2007 and 2011 were classified in terms of the severity of HVA, congruency of the first MTPJ, and type of chevron metatarsal osteotomy. On weight-bearing dorsal-plantar radiographs, HVA and IMA values were measured and compared preoperatively and postoperatively using both the conventionaland new methods. Results: Compared with midline measurements, point-connecting measurements showed higher inter- and intra-observer reliabilityfor preoperative HVA/IMA and similar or higher inter- and intra-observer reliability for postoperative HVA/IMA. Patients who underwent distal chevron metatarsal osteotomy (DCMO) had higher intraclass correlation coefficient for inter- and intra-observer reliability for pre- and post-operative HVA and IMA measured by the point-connecting method compared with the midlinemethod. All differences in the preoperative HVAs and IMAs determined by both the midline method and point-connecting methods were significant between the deviated group and subluxated groups (p=0.001). Conclusion: The point-connecting method for measuring HVA and IMA in the subluxated first MTPJ may better reflect the severityof a HV deformity with higher reliability than the midline method, and is more useful in patients with DCMO than in patients with proximal chevron metatarsal osteotomy.
