        혐기성 동정을 위한 임상의사결정 지원시스템 개발

        In the anaerobe identification, when we develop the clinical decision support system for department of laboratory medicine, we must consider expression of an incomplete knowledge structure and addition of an evolving knowledge based on an expert's informal and heuristic knowledge is very complicated work flow. In the present study, we developed the system for anaerobe identification to advise on identification of unknown bacillus using knowledge base and inference engine. In the future, we are planning to develop the clinical decision support system for the whole bacteria not only an anaerobe but also aerobe to offer an expert's static and dynamic knowledge. 혐기성 균의 동정과정은 업무영역 전체에 복잡성이 존재하며, 전문가의 비정형적인 경험적 지식을 주로 이용한다. 따라서 이와 같은 불완전한 지식체계를 시스템 내부에 표현하고 또한 사용자의 입장에서 진화하는 지식의 추가가 가능하여야 한다. 따라서 본 연구에서는 실질적으로 임상에서 이용이 가능하도록 혐기성 균을 모델로 임상의사결정지원시스템을 개발하여 원인 균 동정과정 시 동정경로 설정 및 해답의 도출에 조언이 가능하도록 하였다. 앞으로 혐기성 균뿐만 아니라 실제 진단검사의학과에서 분리빈도가 높은 호기성균을 포함하는 전체 세균을 대상으로 하는 확대된 영역의 임상의사결정지원시스템이 개발되면 전문가의 견해에서 정적, 동적, 지식을 제공해 줄 수 있는 기반이 되고, 이를 위해 본 연구가 기반으로 활용될 수 있을 것이다.

        전자의무기록에 대한 공인전자서명 적용 지침 개발

        One of the most secure ways of maintaining the confidentiality and integrity of electronic information is to use electronic signatures. So, in this paper, we developed guideline on electronic signatures for EMR(electronic medical record) based on the Medical Law and the Electronic Signature Act. This guideline is intended to introduce EMR easily in the medical field and to facilitate the promotion of EMR. We developed it through consulting from the advisory committee that was made up of experts in the fields of medical record, EMR system and electronic signatures. The contents of the guideline consist of subject and time stamp of electronic signatures, validity of a certificate, management of electronic signatures and custody and management of EMR. In the future, we will develop practical cases and promote educations and publicities of them to use in the medical institutes and EMR system related industries. 전자정보의 기밀성과 무결성을 유지하는 가장 안전한 보안방법 중의 하나가 공인전자서명이다. 이를 의료분야에 적용하기 위하여 본 논문에서는 전자서명법과 의료법에 기반을 둔 전자의무기록에 대한 공인전자서명의 구체적인 적용 지침을 개발하였다. 개발된 지침은 공인전자서명의 주체 및 시점, 공인인증서의 유효성 확인, 공인전자서명의 관리 책임, 전자의무기록의 보관 및 관리에 관한 적용 지침과 관련 해설을 그 내용으로 하고 있다. 향후에는 이 지침을 토대로 의료기관과 관련 업체에서 전자의무기록 시스템 구축 시 실제 활용할 수 있는 예시들을 개발하고자 한다. 이는 전자의무기록의 도입을 용이하게 하고, 그 보급을 촉진할 뿐 아니라 의료기관과 관련 업체들이 겪고 있는 애로사항을 해소하는데 크게 기여하게 될 것이다.

        PC-Cluster 기반 병렬형 유전자 서열 검색 시스템의 개발 및 성능 평가

        In recent, researchers in the field of Bioinformatics need to analyze thousands of genome sequences efficiently according to introduce of new analysis methods and technologies such as genome expression microchip. This rapid growth in the field of bio-engineering needs computing resources to analyze rapidly for genome sequences, but it does not introduce the computing resources due to an enormous investment expense. The core factor of this study is integrated environment based PC-Cluster system & high speed access rate up to 155Mbps, continuous collection system for bio-information at home and abroad. The results of the study are establishment & stabilization of information and communication infrastructure, establishment & stabilization of high performance computer network up to 155Mbps, development of PC-Cluster system with 32 nodes, a parallel BLAST on Cluster system, which can provides scalable speedup in terms of response time, and development of collection & search system for bio-information. 최근 들어 유전자 서열의 생산량 증가에 비례하여 유전자 발현 마이크로 칩과 같은 새로운 분석방법과 기술들이 도입되면서 연구자들이 매일 수천개의 서열을 효율적으로 분석해야 할 필요성이 증대되고 있다. 이러한 생명공학분야의 급속한 발전은 대용량 유전자 서열에 대한 빠른 분석이 가능한 컴퓨팅 자원을 요구하고 있으나 IT 인프라에 대한 막대한 투자비용으로 인해 관련 연구기관에서 쉽게 이들 컴퓨팅 자원을 도입하지 못하고 있는 실정이다. 본 연구에서는 저가의 PC서버를 고속의 네트워크로 연결한 PC 클러스터를 활용하여 시스템의 안정성과 신뢰성을 보장함과 동시에 범용성을 지닌 병렬형 유전자 서열 검색 시스템을 구축하였다. 이러한 효율적인 시스템 구축을 통해 생물정보 데이터베이스 및 서열 검색 시스템을 제공하고, 대용량 서열 데이터베이스의 검색 시간을 단축하였다.

        바깥코신경의 해부

        After rhinoplasty, numbness of the nasal tip has been reported by many surgeons. The nasal tip receives its main sensory nerve supply from the external nasal nerve. However, investigations of the external nasal nerve have not been previously studied. Therefore, anatomic study of the external nasal nerve was performed. Twenty external nasal nerves were dissected in 10 fresh cadaver noses. On dissection, the exit of the nerve between nasal bone and upper lateral cartilage was identified, and the distance from the point of exit to the midline of the nose was measured. The course and the running plane of the nerve were investigated. The nerve branching was also classified into type I; having only 1 nerve without any branch, type II; having 1 nerve proximally, then splitting into 2 main branches, and type III; having 2 main branches from the point of exit. The point of exit of external nasal nerve from distal nasal bone was located 6.5 to 8.5 mm lateral to nasal midline. Nineteen nerves(95%) run in the same deep fatty layer all the way down to the alar cartilages, but one nerve coursed medially between both medial crura. Regarding the branching type of the nerves, type I was found to be the most common, in 10 of 20 nerves. Type II was observed in 6 and type III was seen in 4. Based on our results, the followings are suggested during a rhinoplasty. First, it is best to avoid deep inter- or intracartiaginous incisions. Instead, the dissection should be maintained directly on the surface of the cartilage. Second, dissection at the junction of nasal bone and upper lateral cartilage area of one side should be limited within 6.5mm from midline. Lastly, when the nasal dorsum is augmented by onlay graft, implants or grafts less than 13mm wide at rhinion level should be used.

        수근관증후군의 고해상도초음파 소견과 근전도 및 임상증상과의 상관관계

        Several imaging methods are helpful in the diagnosis of carpal tunnel syndrome and they are, for example, traditional X-ray, CT and MRI, etc. CT and MRI have their advantage in direct visualization of structural abnormality of the carpal tunnel and median nerve, but these two modalities are expensive and take much time. Recently, high resolution ultrasonography has been reported to be effective in the study of musculoskeletal system. We designed a case control study to assess the diagnostic value of high resolution ultrasonography. Ninety-six hands of 48 patients, who were confirmed of diagnosis by self-administered questionnaires and electrodiagnostic test, underwent high resolution ultrasonographic studies. We compared sonographic findings with EMG data and patients' severity scores of self-questionnaires. Proximal swelling of median nerve at the entrance of the carpal tunnel correlated with the nerve conduction data. And compression of the median nerve under the transverse carpal ligament correlated with the subjective symptoms. Although ultrasonography is not an ideal method in diagnosis of carpal tunnel syndrome, it may be helpful for estimating the symptom severity and nerve conduction deficit.

        The purpose of this study is examine the present situation of overseas backpackers for europe, reveals the problems occurred in them and thus presents all the methods possible to solve and promote them. The contents of this study are as follows: Ⅰ. Introduction to the purpose and method of this study, Ⅱ. An outline of this study about Ⅲ. Present situation of overseas backpackers for europe Ⅳ. A making promotion methods for overseas backpackers for europe Ⅴ. Conclusion. The result of this study indicate as follows: First, travel agencies have to make products as much as considering travel condition of destination. Second, travel agencies have to seek special attractions and routes for overseas backpackers. Third, we need a training program to improve the quality of tour conductors for overseas backpackers. Fourth, travel agencies have to make product through customer's need and satisfaction survey. Fifth, we need to develop niche marke for for overseas backpackers. Sixth, travel agencies have to give right information to backpackers. Seventh, travel agencies have to reduce discount competition and sell high-profit products.

      • 지식베이스를 이용한 교육용 염색체 분석 시스템

        The task for chromosome analysis and diagnosis by experienced cytogenetists are being concerned as repetitive, time consuming job and expensive. Far that reason, chromosome analysis system based on knowledge base for CAI has been established to be able to analyse chramasomes and obtain necessary advises from the knowledge base instead of human experts. That is to say, knowledge base by IF THEN production rule was implemented to a knowledge domain with normal and abnormal chromosomes, and then the inference results by knowledge base could enter the inference data into the database. Experimental data were composed of normal chromosomes of 2,736 patients' cases and abnormal chromosomes of 259 patients' cases that have been obtained fram GTG-banding metaphase peripheral blood and amniatic fluid samples. The completed system provides variously morphological information by analysis of normal or abnormal chromosomes and it also has the advantage of being able to cansult with user on chromosome analysis and diagnosis.
