        뼈전이에 대한 방사선 치료 이후 압스코팔 효과로 폐전이 호전을 보인 간세포암 1예

        The abscopal effect is a rare phenomenon that characterized by tumor regression of untreated metastatic lesions after a local radiotherapy. The mechanisms of abscopal effect are speculated to be associated with cytokine release and host immune system. In this case, we report a case of abscopal effect in a 64-year-old male with hepatocellular carcinoma with lung and bone metastasis, who had complete response in the lung after receiving radiation to the metastatic bone lesions.         Hepatocellular carcinoma (HCC) is the third most common malignancy in Korea where chronic hepatitis B virus is prevalent. More than 60-70% of HCC cases are diagnosed at an advanced stage that are not eligible for curative therapy such as surgical resection, liver transplantation, radiofrequency ablation, and percutaneous ethanol injection. According to Barcellona Clinic Liver Cancer (BCLC) staging and treatment, standard treatment of advanced HCC is sorafenib. And there are some reports that hepatic arterial infusion chemotherapy (HAIC) could be a beneficial therapeutic option for patients with advanced HCC. We report a case of advanced HCC with portal vein thrombosis that received liver transplantation after combination treatment of HAIC and sorafenib.

        Alcoholic liver disease includes steatosis, steatohepatitis, progressive fibrosis, and cirrhosis. Alcoholic hepatitis (AH) is an acute inflammatory syndrome characterized by rapid development of jaundice and liver-related complications, and it is associated, particularly if severe, with high mortality. Several prognostic models using laboratory or histologic markers have been used to estimate short-term mortality and to identify those who may benefit from treatment. Recently, prognostic models for acute-on-chronic liver failure were used in patients with severe AH, and systemic inflammatory markers and markers for hepatic injury were also studied for identifying high risk patients. Further studies to find models which can predict prognosis more accurately are needed in the future.

        진행성 간세포암종 환자에서 소라페닙 외의 치료

        Sorafenib is the standard treatment for advanced hepatocellular carcinoma according to the Barcelona Clinic Liver Cancer staging system. However, because of its unsatisfactory efficacy, adverse effects, and high cost, the use of sorafenib is limited, and other treatment modalities are required. Recent studies reported that treatment modalities other than sorafenib, such as hepatic arterial infusion chemotherapy and transarterial radioembolization, showed comparable or better response rates and survival rates than sorafenib. In this review, treatment modalities that could be used as alternatives to sorafenib will be discussed. (J Liver Cancer 2016;16:1-6)

        동양 사상에 담긴 영재의 개념

        본 연구의 목적은 우리나라에서 공교육 차원의 영재교육이 본격화되고 있는 현 상황에서, 영재 개념 정립을 위한 한 방안으로 동양 사상에 나타난 영재(英才)의 개념을 검토하여 밝히는 것이다. 먼저, 동양 문헌에 나타난 영재(英才)의 어원을 고찰하였다. 영(英)은 ‘아직 열매가 열리지 않은 꽃봉우리’를 나타내고, 재(才)는 ‘초목이 움트는 시초’를 나타내는 말이다. 따라서 英才’란 ‘아직 열매가 열리지 않은 꽃봉우리로서, 빼어난 자질 또는 그런 자질을 가진 출중한 젊은이’를 뜻하는 말임을 알 수 있다. 다음으로는, 동양의 전통 사상에 비추어 영재의 개념을 고찰하였다. 孟子에 나오는 영재의 개념은 ‘한 세대를 통해서 명석함과 지혜로움이 뛰어난 인재’(一世睿之才), 즉 ‘작은 도움만으로 도를 깨칠 수 있는 우수한 자질을 가진 자’를 의미한다. 이것은 불가에서 말하는 ‘상근기’(上根機)의 개념, 유가(儒家)에서 말하는 ‘생이지지자’(生而知之者)의 개념과도 맥이 통한다. 마지막으로, 동양 사상의 영재 개념에 나타나는 몇 가지의 특징을 다루었다. 첫째, 동양 사상에서는 영재의 범위를 상위 0.01-1%의 극소수로 한정해서 보았다. 둘째, 영재 개념을 아동이나 청소년에 한정하여 사용하였다. 셋째, 영재 개념은 주로 생득적 자질을 의미하는 것이다. 넷째, 동양 사상에서 영재의 자질은 지력뿐만 아니라, 지혜와 덕성 등을 포함한 상당히 포괄적인 개념을 담고 있음을 알 수 있다. The main aim of this study is to inquire into and examine the concept of Yeong-Jae(英才, the gifted) in Oriental thoughts in order to establish the definition of that, in the situation that the gifted education is begun in earnest in Korea. For the first time, the etymology of Yeong-Jae(英才) has been examined in Oriental ancient literatures. The word of Yeong(英) bares the meaning of ‘a flower bud which has not any fruit yet’, and Jae(才) bares the meaning of ‘the origin which a grass sprouts.’ So, we get to know Yeong-Jae(英才) means ‘the outstanding native ability or the young person who has its quality.’ In the next time, the concept of Yeong-Jae was contemplated referring to the Oriental traditional thoughts. That in Maeng-Ja(孟子) mean the most distinguished person in his intelligence and wisdom through one century(一世睿之才), who has superior ability being able to realize the truth(道) by his teacher's slight help. In other point of view, that is similar to the meanings of Sanggeungi(上根機) in Buddhism that means ‘the man being able to enter into the level of awakening as soon as hearing the doctrine(法), and Saengijijija(生而知之者) in Confucianism that means ‘the prodigy being able to know without special learning due to a kind of native knowledge.’ For the last time, the some peculiarities in the concept of Yeong-Jae in Orientalism were considered in comparison with concept of the gifted in these days. First, the range of Yeong-Jae was extremely limited in Orientalism. Yeong-Jae implied the highest group of 0.01-1%, which are different from the range of the gifted in these days that is superior persons about 15-20%. Secondly, the word of Yeong-Jae was primarily used for the children or the youth. Thirdly, the word involves the meaning of the innate or native ability. Last, the concept of Yeong-Jae has comprehensive meaning involving not only general intelligence but wisdom and morality.

        최근 10년간 간세포암종 진단을 위한 가이드라인의 변화

        Hepatocellular carcinoma (HCC) is the third most common cause of cancer death in the world. There has been many advances in diagnosis of HCC during the last ten-year period, especially imaging techniques. The Korean Liver cancer study group (KLCSG), European Association for the Study of the Liver (EASL), American Association for the Study of Liver disease (AASLD) and Asian-Pacific Association for the Study of Liver (APASL) have made and changed HCC guidelines with advances of imaging technique and results of research on HCC. We reviewed the changes of imaging guidelines in HCC diagnosis according to the advances of imaging. In addition, further studies will be needed to solve the controversies in diagnosis of HCC smaller than 1 cm in size.

        Dewey에 있어 경험과 교육의 관계

        The aim of this study is in analysing the relationship of the conception of experience and education in John Dewey For the first, we need to understand that the experience used by Dewey involves several types and meanings. The experience has six types, primary and second experience in the side of its content, direct and indirect experience in the side of Its method, and educative and mis-educative experience In the aspect of value. In the other hand, the experience in Dewey Involves following meanings; two phases in the same times of content and method, and results, and two phases of acting and thinking in its activity. Its two aspects of process Dewey's concept of education is also comprehensive, covering not only the formal one, schooling but the informal one as self-renewal process by participating in the nature and the society So, it is not easy to grasp the relationship or the difference of experience and education But education is included in a part of experience, for there is two kinds of experience in the value, an educative one which gives rise to active interaction and promotes growth of further experiences, and an mis-educative one which arrest or distort growth, and for education is a development within experience The modifier of ' constant' has very Important meaning in a definition so called that ""education is a constant reorganizing or reconstructing of experience "", Thus it is important thing in education to choose, arrange and suggest experiences which could have student engage his present activities, and at the same time, promote having desirable future experiences.

        듀이의 아동 중심 교육의 진의

        본 연구의 목적은 듀이의 ‘아동 중심 교육’ 개념의 진의를 탐구하는 데에 있다. 먼저 그에게 있어 아동 중심 교육의 의미를 분석하였다. 듀이에 있어 ‘아동 중심 교육’이라는 말은 인생과 경험과 학습의 주체인 학습자를 위해 모든 교육적 장치를 재구성하는 것을 의미하며, 학생이 일방적으로 주도하는 교육이 아니라, 교사의 사려 깊은 지도를 요청하는 것임을 알 수 있다. 다음으로는 교육의 목적과 방법에 있어 듀이의 아동 중심 교육 개념과 관련된 부분을 다루었다. 교육의 목적은 아동 자신의 성장을 위해 내발적으로 나와야 한다는 내용을 고찰하였다. 듀이는 외부에서 학습자에게 부과되는 교육 목적을 비판하고, 교육의 목적도 학습자의 경험 활동 과정에서 자신의 필요에 의해 나오는 것일 때 교육적 의미가 있다고 생각하였다. 다만 교사의 협력적인 지도와 안내가 필요하다는 점도 분명히 밝히고 있다. 마지막으로 아동의 흥미를 중시하는 교육 방법론을 고찰하였다. 듀이가 교육에서 학습자의 흥미에 중요한 의미를 부여하는 이유는 흥미가 있을 때 활발한 상호작용이 일어나며 학습자 스스로 지속적인 노력을 해나갈 수 있다고 보았기 때문이다. 따라서 교육자의 중요한 임무는 현재와 미래의 경험 성장을 동시에 고려하여 환경이나 학습 자료를 현명하게 선택하여 제공하는 일이라고 간주했다. The main aim of this study is to examine the true meaning of 'child-centered education' in J. Dewey. To begin with, the phrase of 'child-centered education' in him do not implies that students lead entirely learning, but is a merely declaration about the essential intent of education, that is, all educational efforts are for the sake of learner's continuous growth. The purpose of education in his theory of education can be regarded as learner's growth of experience. While he criticized an imposed and ultimate aim from outside, he said an aim as ‘end in view’ has meanings only when it comes from learner's own need or from interaction with educator in the process of learning or activity. Dewey attached great importance to a learner's interest as an educational method, for he thought a child and his educational material interact actively and the learner makes efforts for himself to grow consistently, only when he has interest. Thus, educator's major business is to choose, arrange and suggest experiences which could have student engage his present activities, and at the same time, promote having desirable future experiences.

        자발성 세균성 복막염

        Spontaneous bacterial peritonitis (SBP) is defined as bacterial infections that occur in patients with cirrhosis and ascites without any significant intraperitoneal infection, accounting for approximately 10-30% of bacterial infections in hospitalized patients. SBP develops in patients with liver cirrhosis because bacterial translocations are increased by changes in the intestinal bacteria and mucosal barriers. In addition, the decreased host immune response cannot remove the bacteria and their products. The most common cause of SBP is Gram-negative bacteria, such as Escherichia coli and Klebsiella species, and infections by Gram-positive bacteria are increasing. SBP is diagnosed by the presence of >250 polymorphonuclear leukocyte/mm<sup>3</sup> in ascites after paracentesis. If SBP is diagnosed, empirical antibiotic therapy should be started immediately. Empirical antibiotic treatment should distinguish between community acquired infections and nosocomial infections. Cirrhotic patients with gastrointestinal bleeding or low ascitic protein concentrations should consider primary prevention and those who recover from SBP should consider secondary prevention. This review describes the pathophysiology, diagnosis, treatment, and prevention of SBP. (Korean J Gastroenterol 2018;72:56-63)
