Purpose: We report 2 cases of infectious endophthalmitis after intravireal triamcinolone acetonide injection which was successfully treated with early vitrectomy. Methods: A 56 year old male patient with chronic cystoid macular edema of right eye and an 84 year old female patient with diabetic macular edema of right eye (pseudophakic) eye were treated with intravireal triamcinolone acetonide injection. Four days after the injection, both the patients complained of decreased visual acuity that had been developed 1 and 2 days prior respectively. Visual acuity was hand motion, inflammatory cells, flare, fibrins and hypopyon were found in the anterior chamber. Fundus was not visible due to vitreous opacity. Results: The two patients were presumed to have infectious endophthalmitis. Anterior chamber irrigation, vitrectomy, intravitreal antibiotics injection, and vitreous culture were performed. Coagulase negative staphylococcus was detected from vitreous culture, Within 1 week of the follow-up procedures, the anterior chamber and vitreous inflammation were improved and visual acuity recovered to the level attained before intravireal triamcinolone acetonide injection. Conclusions: If after intravireal triamcinolone acetonide injection, there is severely decreased visual acuity, inflammation of the anterior chamber and vitreous, flare and fibrin are detected, infectious endophthalmitis must be suspected. If diagnosed early and treated with vitrectomy, a favorable visual prognosis is expected.

Purpose: To report one case of atypical Miller Fisher syndrome with spontaneous recovery. Methods: A 44-year-old woman was visited our clinic complaining of diplopia for 2 days. She has irregular medication history for thyroid dysfunction, and there was no antecedent respiratory or gastrointestinal infection. The best corrected visual acuity of both eyes was 20/20. She showed bilateral proptosis and limitation of abduction and supraduction, mild blepharoptosis of left eye and esotropia of 30 prism diopter (PD). Results: Thyroid fuction test, orbital CT and electromyography were normal. Cerebrospinal fluid (CSF) analysis revealed albuminocytologic dissociation. Steroid pulse therapy was started promptly, but there was no improvement. Extraocular movement and diplopia gradually improved after 4 weeks of onset. and spontaneously recovered completely after 2 months. Conclusions: We reported a case of atypical Miller Fisher syndrome that completely and spontaneously recovered within 2 months., and represented normal findings by every examination such as orbital CT, thyroid function test, electromyography and CSF analysis.

Objective: The Relationship between preterm labor stress, fetal attachment and maternal roleconfidence in pregnant women with preterm labor, which was carried out in a descriptive form. Methods: The study objects were 121 pregnant women hospitalized for preterm labor at oneuniversity hospital and four women's hospitals in U metropolitan City. The data were based onthe self-report questionnaires made between October 1st, 2012 and January 31, 2013. Results: The degree of preterm labor stress and maternal-fetal attachment of pregnant womenwith preterm labor were found to be higher than the middle, and the degree of maternal role confidencewas found to be lower than the middle. There was a positive relation between pretermlabor stress and maternal-fetal attachment(r=.197, p=.030). There was a positive relation betweenpreterm labor stress and interaction with the fetus(r=-.185, p=.042). and role taking among thesubscales of maternal-fetal attachment (r=.346, p=.001). There was a positive correlation, therewas a positive relation between attribution characteristics and intention to the fetus among thesubscales of maternal-fetal attachment and maternal role confidence (r=.254, p=.005). Conclusion: It is necessary to develop a nursing intervention program for reducing preterm laborstress and increasing maternal-fetal attachment in order to increase the maternal role confidenceof preterm labor pregnant women, as well as a program that considers the characteristics that affectthe degree of preterm labor stress, maternal-fetal attachment and maternal role confidence.

4차 산업혁명으로 플랫폼 간의 정보공유와 융합이 빠르게 진행되며, 콘텐츠 공급자와 수요자의 폭발적 증가와 함께 디지털 콘텐츠 산업은 급속히 성장하고 있다. 또한, 과거 콘텐츠의 유통 및 보급을 담당하였던 기업들이 콘텐츠 제작에도 참여함에 따라, 산업 내 세부 영역 간의 구분도 흐려지고 있다. 본 연구를 통해 우리는 이처럼 변화하고 있는 디지털 콘텐츠 산업과 국내 디지털 콘텐츠 산업을 선도하고 중국, 베트남 등의 신흥시장에 적극적으로 진출하고 있는 ‘디앤씨미디어’의 성장 과정을 살펴볼 것이다. 디앤씨미디어의 신현호 대표는 영역 간 경계가 급속히 무너지고 있던 디지털 콘텐츠 산업에서의 경쟁우위는 양질의 지적재산권(웹소설 및 웹툰 등)을 확보할 수 있느냐에 달려 있다고 인식하였다. 이에 따라, 디앤씨미디어는 지속적인 지적재산권 확보와 소비자 유입이 가능한 시스템을 구축하고, 지적재산권을 생산하는 작가들의 육성 시스템을 발전시켜 활용하고 있다. 또한, 디앤씨미디어는 2017년 이후 중국, 베트남 등의 신흥국을 중심으로 글로벌 진출을 서두르고 있다. 디앤씨미디어가 국내에서 확보한 경쟁력있는 지적재산권을 해외시장으로 이전하여 활용하는 것으로 판단된다. 본 사례에서 보이는 디앤씨미디어의 국내외 시장에서의 성장전략은 관련 산업 및 이론을 연구하는 연구자 뿐 아니라, 실무자, 정책입안자들에게도 많은 시사점을 줄 것으로 기대된다. As the Fourth Industrial Revolution promotes sharing and converging information between platforms, the digital content industry is growing rapidly. And boundaries between digital contents industrial areas have been blurred because companies that provided service in distribution and publication have begun to create contents. In this study, we review the industrial environment of digital contents and look into the growth process of D&C Media. D&C Media is a digital contents production company founded in 2002. Hyunho Shin founded this company based on his know-how and experience on contents industry. He recognized that the competitive advantage in the digital content industry depends on the capability of creation of high quality intellectual property (web novels and webtoons). Thus, D&C Media has developed a platform which can promote inflow of consumers and writers, and a system which foster writers who can produce new intellectual property. And, D&C Media has been rushing to global market since 2017, especially in emerging countries such as Vietnam. D&C Media's success in the Vietnamese market can be seen as a result of the transfer of intellectual property rights produced in the domestic market to the Vietnamese market. D&C Media’s business in Vietnam can explain that firms increase their commitment to their local markets as they gain knowledge and experience on their markets. This case is expected to reveal the digital contents companies’ strategy for building core competence and entering into overseas market. And, this case will help students assess firm’s core competencies, and understand the types of global strategies or the entry mode of multinational companies.

Effect of Estradiol in an Azoxymethane/Dextran Sulfate Sodium-Treated Mouse Model of Colorectal Cancer: Implication for Sex Difference in Colorectal Cancer Development

Purpose This study demonstrates that estradiol downregulates inflammation and inhibits colorectal cancer (CRC) development in azoxymethane/dextran sulfate sodium (AOM/DSS) mouse model. Materials and Methods AOM/DSS-treated male and female mice were sacrificed at weeks 2, 10, and 16, to assess estrogen effects on colitis and carcinogenesis. Macroscopic and histologic severity of colitis and Western blot and quantitative real-time polymerase chain reaction were evaluated, to measure inflammatory mediators and cytokines. Results Compared with AOM/DSS-treated male mice (M-AOM/DSS group), AOM/DSS-treated male mice with estradiol administration (M-AOM/DSS+estr group) displayed at week 2 significantly decreased severity of colitis. At weeks 10 and 16, AOM/DSS-treated female mice (F-AOM/DSS group) and the M-AOM/DSS+estr group showed significantly lower tumor multiplicity compared with the M-AOM/DSS group. At week 2, F-AOM/DSS group had a lower level of nuclear factor-B (NF-B) expression and higher level of nuclear factor erythroid 2-related factor 2 (Nrf2) expression, compared to the M-AOM/DSS group. At week 2, expression levels of NF-B and its related mediators decreased in the M-AOM/DSS+estr group, while levels of Nrf2 and Nrf2-related anti-oxidant enzymes increased. In addition, estradiol significantly increased Nod-like receptor protein 3 (NLRP3) inflammasome expressions in AOM/DSS-treated male mice. In contrast, at weeks 10 and 16, Nrf2 and its-related anti-oxidant enzymes and NLRP3 inflammasome were highly expressed in M-AOM/DSS group and in F-AOM/DSS group, who developed cancer. Conclusion The data suggest that estradiol inhibits the initiation of CRC by regulating Nrf2-related pathways. Moreover, these imply the dual role of Nrf2 and NLRP3 inflammasome, including promotion of tumor progression upon tumor initiation.

Methyl Syringate, a Low-molecular-weight Phenolic Ester, as an Activator of the Chemosensory Ion Channel TRPA1

Transient receptor potential channel ankryn 1 (TRPA1) and transient receptor potential channel vanilloid 1 (TRPV1) are members of the TRP superfamily of structurally related, nonselective cation channels and are often coexpressed in sensory neurons. Extracts of the first leaves of Kalopanax pictus Nakai (Araliaceae) have been shown to activate hTRPA1 and hTRPV1. Therefore, the effects of six commercially available chemicals (methyl syringate, coniferyl alcohol, protocatechuic acid, hederacoside C, α-hederin, and eleutheroside B) found in K. pictus were investigated on cultured cells expressing hTRPA1 and hTRPV1. Of the six compounds, methyl syringate selectively activated hTRPA1 (EC50 = 507.4 μM), but not hTRPV1. Although methyl syringate had a higher EC50 compared with allyl isothiocyanate (EC50 = 7.4 μM) and cinnamaldehyde (EC50 = 22.2 μM), the present study provides evidence that methyl syringate from K. pictus is a specific and selective activator of hTRPA1.

임신 20주 이전 이른 조기양막파수의 자연 경과 및 예후 인자

Objective: 너무 이른 시기에 진단된 양막파수는 매우 곤란한 문제로서 환자 상담에 있어서도 어려움이 많다. 보존적 치료를 시행한 임신 20주 이전 이른 조기양막파수 환자의 자연 경과를 알아보고 그 예후 인자를 분석해보고자 하였다. Methods: 임신 10주에서 19주 사이에 조기양막파수를 진단받은 단태아 임산부를 대상으로 후향적 코호트 연구를 시행하였다. 양막파수의 진단은 분비물이 흐르는 증상이 있으면서 암니슈어 검사 (Amnisure ROM test™) 양성인 경우로 정의하였다. 양막파수가 진단된 후에는 흐르는 증상이 사라지고 암니슈어 검사가 음성이 될 때까지 항생제를 사용하며 보존적 치료를 시행하였다. Results: 총 19명 중 3/4에 해당하는 15명이 질출혈 혹은 피가 비치는 증상을 주소로 병원에 왔다. 조기양막파수 진단 후 양수양이 적절하게 유지되던 11명중 8명은 보존적 치료 중 양막파수의 증거가 사라지고 이후 합병증 없이 만삭분만 하였다. 이들에서 항생제 치료기간의 중앙값은 11.5일 이었다. 11명 중 2명은 각각 임신 26주와 32+3주에 조산하였고, 열흘 간 치료를 받다가 자의로 퇴원한 환자도 한 명 있었다. 반면에 양막파수 진단 후 남아 있는 양수양이 현저히 감소했던 8명의 환자는 모두 임신 예후가 좋지 못했는데 이들 중 3명은 임신 20주 이전 유산되었고, 2명은 조기양막파수 진단 후 입원 치료를 지속하다가 각각 임신 24+1주와 28+1주에 조산하였고, 3명은 치료를 포기하고 퇴원하였다. Conclusion: 임신 20주 이전 발생한 이른 조기양막파수는 그 증상이 절박유산과 비슷하게 나타날 수 있다. 양막파수 진단 후 양수양이 적절하게 유지되는 경우에는 임상적으로 양막파수 상태가 지속되기 보다는 항생제 보존적 치료 후 일과성으로 지나가는 경우가 많지만, 양수양이 현저히 감소된 경우에는 유산이나 조산 발생과 관련이 깊은 것으로 보인다.
