        폐에서 발생한 흉선종 -1예 보고 -

        Thymomas are common anterior mediastinal tumors. In rare instance, thymomas develop in ectopic thymic tissue and are found in various locations in the thorax outside of the anterior mediastinum. They have been discribed in middle mediastinum, within the lung, or in the extrapleural tissue. Intrapulmonary thymoma is a very rare disease and is infrequently documented. The authors described a case of intrapulmonary thymoma in the left lung, which was proved by tranthoracic needle biopsy & operation with pathology. A review of the literature concerning the subject is also given.

        Unusual Tc-99m MDP Uptake in the Keloid Developed after Subtotal Gastrectomy

        A 63-year-old male who had subtotal gastrectomy for early gastric cancer three months ago underwent Tc-99m bone scintigraphy for the evaluation of skeletal metastases. He had no symptoms such as fever, tenderness, or wound discharge. On physical examination, the surgical scar along the midline of the upper abdomen had keloid formation and there was no radiographic evidence of calcification. Bone scintigraphy (Fig. 1A & 1B) demonstrated an unusual linear increased uptake along the midline of the upper abdomen that corresponded to the skin incision for subtotal gastrectomy. Usually, an incisional scar will not be visualized in Tc-99m methylene diphosphate (MDP) scintigraphy beyond two weeks after surgery.1) Upon reviewing the literature, there were only a few reports where localization of Tc-99m MDP in surgical scars were found two months after surgery.2) It was also reported that a few cases with Tc-99m MDP uptake in the keloid scar developed after surgery. Although there are several potential mechanisms that may explain the uptake of Tc-99m MDP in scar tissue, the primary mechanism in older scars is suggested to be a result of pathological calcification.2) Siddiqui et al3) suggested it could be due to microscopic calcification in small resolving hematomas. However, the primary mechanism in keloid scar is not well-known. We should obtain oblique or lateral views to differentiate the uptake in healing surgical scars from the artifactual uptake.

        위장관 계실의 방사선학적 고찰

        Diverticulum is considered as a common lessin involving any gastrointestinal tract from the pharynx to the rectum. We reviewed 5806 cases of upper G-I series and 801 cases of double contrast barium enema during the period from Jan. 1978 to Dec. 1981 in the Department of Radiology, School of Medicine, Jeonbug National University Hospital to analyze diverticula of the entire gastrointestinal tract. The results were as follows : 1. Roentgen examination of 5806 esophagus, stomachs, duodenums and small bowels, and 801 colons during the past four years : diverticula of the esophagus, 60 cases(1.0%); diverticula of the stomach, 42 cases (0, 7%); diverticula of the duodenum, 358 cases (6.2%); diverticula of the small bowel, 20 cases(0.3%); diverticula of the colon, 26 cases(3.2%). The location of diverticula in order of frequency was duodenum, colon, esophagus, stomach and small bowel. 2. The most common site of diverticula of each gastrointestinal tract was as follows : diverticula of the esophagus, middle portion (84.7%); diverticula of the stomach, the cardia (59.5%); diverticula of the duodenum, the second portion (81.3%); diverticula of the small bowel, the jejunum (96.4%)-especially the colon, the right sided conon (80.8%)-the cecum and ascending colon (57.1%). 3. Diverticula may occur at any age. The majority of diverticula of the entire gastrointestinal tract were observed over 40 years of age. Especially in diverticula of the duodenum and colon, their frequency increase with age. 4. Duodenal diverticula were observed more frequently in women than in man but in diverticula of the esophagus, stomach, small bowel and colon, male was more frequency affected. 5. Mutiplicity of diverticula of the entire gastrointestinal tract was 16.2% of 506 cases. In diverticula of the esophagus, stomach, duodenum and small bowel, single lesion was more frequency found. On the other hand, the majority of diverticula of the colon were found as multiple lesion (69.2%).

        Meckel's diverticulum is one of the most frequent congenital malformations of the gastrointestinal tract in man and found in approximately 1 to 3% of the general population. This remnant of the omphalomesenteric duct gives rise to symptoms of primarily rectal bleeding in the young. The customary radiographic approach to the gastrointestinal bleeder ususally fails to identify this lesion. Most of the symptomatic Meckel's diverticula are lined with ectopic gastric mucosa. Therefore Tc-99m pertechnetate which is concentrated by the gastric mucosa has been found to be useful in localizing these divertecula. The authors described a case of bleeding Meckel's diverticulum whose pathologic diagnosis was expected preoperatively with Tc-99m pertechnetate abdominal scintigraphy and confirmed postoperatively.

        聖母病院 麻藥使用 實態調査

        The rate of crimes related with the narcotics is recently sharply increasing and the abuse and misuse of drugs cause social problems in Korea. So the investigation and campaign through mass media became actively going on. But there is no special institution caring for the drug addict in Korea. Although the Ministry of Health and Social Affairs authorized 13 national or public and 6 private hospitals as special treating centers in 1987, the lack of specialists and funds makes it almost impossible to perform proper treatments. The major illegally abused narcotics for the hallucination in Korea are methamphetamine (by the name of philophone) and marihuana ; Compared with cocaine and heroin in Western countries. In the moment, hospital phamacists are urgently required to re-examine and research the pharmacology, dependence and side effect of the narcotic drugs. This paper deals with the pharmacolgy, mechanism, applied disease, dependence and withdrawal syndrom of frequently used therapeutic narcotics and shows the examples and frequency of narcotic drugs used in St. Marys Hospital.

      • 국가 초미세먼지 측정망의 시간지연 정보 제공 문제점에 관한 연구

        본 논문은 현재 국가 초미세먼지 측정망인 에어코리아에서 제공하는 실시간 초미세먼지 정보의 시간 지연수준에 대하여 대전광역시 유성구 일대에서 국가측정소 2곳과 초소형 에어로졸 컨디셔너를 내장한 광산란 미세먼지 측정기 1대를 설치하여 현장 측정 데이터를 비교 분석한 결과, 국가측정소의 실시간 정보 제공이 1시간 이상 지연 됨에 따라, 순간 발생하는 고농도에 시민들이 무방비 상태로 노출되어 1시간 이상 고농도를 흡입하는 문제점과 대기 확산으로 공기질이 개선되어도 늦은 정보 제공으로 시민의 행동 제약 문제가 발생할 수 있음을 확인하였다. 지금까지 광산란 방식 미세먼지 측정기는 초 단위 실시간 정보 제공이 가능하지만 대기중 흡습 미세먼지에 대하여 광산란 특성 변화로 측정 정확도가 현저히 저하되는 문제가 있어 실내용으로 사용이 한정되고 있는데, 우리는 이러한 문제를 해결하기 위하여 광산란 방식 미세먼지 측정기에 내장할 수 있도록 초소형 히터를 개발하였다. 세계 최초로 실시간 흡습미세먼지를 상대습도 35%로 자동 건조 가능한 초소형 히터를 탑재한 초소형 에어로졸 컨디셔너를 개발하였다. 초소형 에어로졸 컨디셔너를 내장한 공감센서를 대기정체상태에서 1.75 km 떨어진 국가측정소(대전광역시 유성구 관평동)와 4.88 km 떨어진 국가측정소(대전광역시 유성구 구성동) 측정 결과와 비교 분석한 결과, 야간에 고농도 미세먼지가 갑자기 발생한 경우 에어코리아 정보에 의존하는 시민은 최소 1시간 15분 이상 고농도 PM2.5를 흡입하게 되는 문제와 대기 확산으로 공기질이 좋아진 경우 여전히 국가 측정소 정보는 실시간 대기질 상태 반영을 하지 못하는 문제점을 실외 실측을 통하여 확인할 수 있었다.

      • 종양과 정상조직의 Ga-67 의 Uptake : Transferrin-의존성과 Transferrin-비의존성에 의한 섭취기전 연구 Demonstration of Transferrin-Dependent and Transferrin-Independent Mechanism

        The mechanism by which ^67Ga accumulates in tumors and normal tissues is controversial. It is widely accepted that ^67Ga, as an of analog of iron, binds to circulating transferrin and gains access to cells by means of the transferrin receptor. However, there is a significant amount of evidence that uptake of ^67Ga may not be universally mediated by transferrin. To determine if transferrin is required for uptake of ^67Ga in vivo, we first compared the uptake of ^67Ga by two types of implanted tumors and by normal tissues both in normal wild-type Balb/C mice and in a strain of severly hypotrasferrinemic Balb/C mice. Also, the distribution of ^67Ga was compared by visually identifying scintigraphic images of the tumor-bearing mice. Tumor were grown in mice from cultured XS63 myeloma cells and Balb/3T3 cells transformed by the Morlony murine sarcoma virus(MMSV/3T3). Uptake of ^67Ga by normal sofr tissues was markedly less in hypotransferrinemic than in normal mice and uptake of ^67Ga by bone was equivalent in the two types of mice regardless the presence of transferrin. The image of the hypotransferrinemic mouse was a scintigraphic pattern similar to that of a "bone scan", while the image of the normal Balb/C mouce was that of normal ^67Ga scan in human because of evenly distributed radioactivity among all organs. One type of tumor was strongly gallium-avid, the other was not. For the more gallium-avid tumor, XS63, uptake of ^67Ga was similar and the ratio of tumor-to-background activity was significantly higher in the hypotransferrinemic than in the normal mice. It waws evidents as a foci of increased activity in both mice on the scintigraphic image. But improved delineated of it form decreased surrounding normal soft tissues occurred in hypotransferrinemic mice. For the less gallium-avid tumor, MMSV/3T3, uptake was significantly les in hypotransferrinemic than in normal mice. Uptake in MMSV/3T3 tumor was decreased in hypotransferrinemic mice to approximately the same degree as that for normal soft tissues. No visually appreciable occurred in both mice were "cold" in comparison to surrounding normal structure. These data suggest that uptake of ^67Ga by normal nonosseous soft tissues and by some tumors(less gallium-avid tumor) may be a transferrin-dependent process, but that uptake of ^67Ga by bone and by at least one type of gallium-avid tumor may be mediated by mechanisms independent of transferrin.

        Hydroxyapatite 안구보충물삽입술 후 골신티그라피를 이용한 섬유혈관증식 평가 : 평면영상과 SPECT 영상에서의 비교 Comparative Analysis of Planar and SPECT Imaging

        Purpose: Complete fibrovascular ingrowth within the hydroxyapatite ocular implant is necessary for peg drilling which is performed to prevent infection and to provide motility to the ocular prosthesis. We compared planar bone scintigraphy and SPECT for the evaluation of the vascularization within hydroxyapatite ocular implants. Materials and Methods: Seventeen patients (M:F=12:5, mean age: 50.4±17.5 years) who had received a coralline hydroxyapatite ocular implant after enucleation surgery were enrolled. Patients underwent Tc-99m MDP planar bone and SPECT imaging by dual head gamma camera after their implant surgery (interval: 197±81 days). Uptake on planar and SPECT images was graded visually as less than (grade 1), equal to (grade 2), and greater than (grade 3) nasal bridge activity. Quantitative ratio of implanted to non-implanted intraorbital activity was also measured. Vascularization within hydroxyapatite implants was confirmed by slit lamp examination and ocular movement. Results: All but three paties were considered to be vascularized within hydroxyapatite implants. In visual analysis of planar image and SPECT, grade 1 was noted in 9/18 (50%) and 6/18 (33%), respectively. Grade 2 pattern was 7/18 (39%) and 4/18 (22%), and grade 3 pattern was 2/18 (11%) and 8/18 (44%) respectively. When grade 2 or 3 was considered to be positive for vascularization, the sensitivity of planar and SPECT imaging were 60% (9/15) and 80% (12/15), respectively. In 3 patients with incomplete vascularization, both planar and SPECT showed grade 1 uptake. The orbital activity ratios on planar imaging were not significantly different between complete and incomplete vascularization (1.96±0.87 vs 1.17±0.08, p>0.05), however, it was significantly higher on SPECT in patients with complete vascularization (8.44±5.45 vs 2.20±0.87, p<0.05). Conclusion: In the assessment of fibrovascular ingrowth within ocular implants by Tc-99m MDP bone scintigraphy, SPECT image appears to be more effective than planar scintigraphy.
