한의학 정신요법을 이용한 소아 전환장애 환자 1례 -지언고론요법(至言高論療法), 지지적 정신치료와 행동치료-

Objectives: This case report presents a 7-year-old female patient diagnosed with conversion disorder improved by application of oriental medical psychotherapy, the Giungoroen. Her chief complaint was dysesthesia of anal. Methods: Through interview and several psychological testing such as HTP test, DAF test, JTCI 7-11, we assessed her psychological state and seek for the psychological reason induced her physical symptom, dysesthesia of anal. After assessment we concluded her main psychological problem was loss of love relationship with her parents. So we decided to use supportive psychological therapy and behavioral therapy-like changing upbringing attitudes of her parents, for example- with Herbal medicine treatment. Results: After 2-month of therapeutic period, chief physical symptom of patient-dysesthesia of anal-was reduced to 20% compared with first visit day. Conclusions: The Giungoroen corresponds to supportive psychotherapy and behavioral therapy. And this oriental medical psychotherapy is fairly effective on the treatment of conversion disorder in childhood.
한약(韓藥) Extract제제의 투여로 호전을 보인 광장공포증을 동반한 공황장애 환자 1례(例) -계지가룡골모려탕(桂枝加龍骨牡蠣湯)과 작약감초탕(芍藥甘草湯)-

Objectives: This case report presents a 33-year-old male patient who complained chest pain and anxiety, diagnosed with panic disorder with agoraphobia improved by Herbal Extracts, SRP(Stress Reduction Program) and Acupuncture treatment. Methods: In early treatment period, we evaluated his general psychological state through interview and self-questionnaires(STAI, SCL-90-R, STAXI, BDI) for treatment planing. And then we had prescribed herbal extracts for 20 weeks with applying SRP and acupuncture treatment. After 20 weeks we assessed his psychological symptoms with same evaluation tools. Results: After 20 weeks, We found that patient`s somatic and psychological symptoms(anxiety, depression, anger) were alleviated. Frequency and severity of panic attacks were reduced also. Conclusions: Herbal extracts, especially Gyejigayonggolmoryo-tang(桂枝加龍骨牡蠣湯) and Jakyakgamcho-tang(芍藥甘草湯), may be useful therapeutic tool for control of panic attack symptoms and anxiety.
노인 불면에 대한 EFT 불면 치료 프로그램(EFT-1)의 효과 평가를 위한 예비적 연구

Objectives: The aim of this study was to evaluate the effects of EFT-I(EFT program for insomnia) for insomnia in the elderly as a preliminary study. Methods: This study was a single group pre-post comparative study that involved 10 elderly women(mean age=76.3±4.29), who visited a senior welfare center, complained of insomnia symptoms. Subjects received 8 sessions(twice a week for 4 weeks and 1 hour for each session) of EFT-I group training. Insomnia severity, depression, state anxiety, and life satisfaction of each subject were evaluated twice at pre and post of EFT-I. Korean Sleep Scale, Short form of Geriatric Depression Scale(SGDS), State-Trait Anxiety Inventory(STAI), and life satisfaction scale were used as evaluation tools. The data were analyzed using paired-samples T-test. Results: Insomnia severity, life satisfaction, depression, and state-anxiety were significantly improved by EFT-I. Conclusions: Result of this study showed that EFT-I can be a useful treatment program for elderly insomnia. Larger Clinical trials are needed to verify effect of EFT-I as a community based insomnia management program for the elderly.

Objective: This study was designed to research the effect of the korean traditional herbal medicine and oriental psychotheraphy on globus hytericus and swallowing difficulty caused by conversion disorder. Methods: A 10 years old male patient suffer from globus sensation and swallowing difficulty for 2 months. We treated him with the korean traditional herbal medicine(Banhahubak-tang) and oriental psychotheraphy for 27 days. Results: After this treatment, globus sensation and swallowing difficulty were decreased and appetite were increased. In addition, compulsion had been severe and it was improved after this treatment. Conclusions: Psychiatrists have reported that conversion disorder is not common in children especially under 10 years old. Globus hystericus, dysphagia is symptoms of conversion disoder but globus hystericus is very rare in childhood. In this case, we recognized that the korean traditional herbal medicine and oriental psychotheraphy could be effective for the clinical symptoms of globus hystericus and swallowing difficulty.

Objectives: The aim of this study was to investigate the characteristics of Hwa-Byung(HB) patients, draw the clinical key issue of HB, and provide the data as the basis for development of Clinical Guideline of HB. Methods: The study participants included 151 subjects who thought they have HB in 9 site. For all patients, we used HB epidemiologic study protocol, which include the Korean version of the Structured Clinical Interview for the DSM-IV Diagnosis (SCID-1), diagnostic criteria of HB, symptoms check list, pattern identification tool, psychiastric psychological questionnaires, and Framingham coronary risk score(FRS). Results: 1. For all participants, 62 % of subjects were diagnosed with HB. These patients had comorbid DSM-IV diagnoses with depression (65%) and anxiety (27%), But 22% of these patients had only HB. 2. HB patients had various physical symptoms when visiting, were diagnosed as various disease, and undergone medical treatments in the past. Among the diagonsed illness, Gastronitestinal diseases (51.6%), psychiatric disorders(40.9%), endocrine diseases(39.8%) were being the most frequent. The frequent physical symptoms of HB patients reported were chest discomfort, head ache, palpitation, frequent sigh, amnesia, shoulder pain, dry mouth, eye fatigue etc. 3. HB patients had high scores in psychologic questionares, CES-D, STAI, and STAXI. It means that HB patients might be low-level emotional stability. 4. Participant had negative opinion about the treatment of HB that it would be difficult or impossible (65.7%), but they neglect the need of medical treatment. Conclusions: This result shows that HB is comorbid with various psychiatric disease, but it has different physio-psychological symptoms from others and there were only HB patients. So we identify HB is independent disease. Moreover, there were the wrong perception of HB which blocking treatments. These problems support that the necessity of development of Clinical Guideline of HB.
