        대장암 생존자의 운동 참여 경험

        Purpose: The purpose of this study was to investigate the exercise participation experiences of colorectal cancer survivors. Methods: A phenomenological method was used in this study. Ten adult colorectal cancer survivors were recruited and data were collected through in-depth interviews. Results: The factors involved in the experiences of colorectal cancer survivors’ exercise participation were categorized into 3 parts: (1) mental depression and isolation due to physical change, (2) barriers of exercise, (3) exercise participation for recovery. Though participants were aware of their changed body after cancer treatment and faced difficulties participating in exercise, they want to overcome these challenges through exercise participation. Conclusion: The results of this study show the exercise participation experiences of colorectal cancer survivors. Based on these findings, exercise maintenance can be promoted more effectively, and a higher exercise adherence of colorectal cancer survivors can be achieved.

        소아청소년암 생존자의 운동 경험과 운동 제약에 대한 질적 사례연구

        본 연구는 아동·청소년 시기에 암을 진단받은 소아청소년암 생존자의 개별적인 운동 경험과 운동 제약을 파악하고, 공통적으로 나타나는 이슈와 실천적 함의에 대해 심층적으로 탐색하고자 하였다. 소아청소년암을 진단받고 치료를 완료한 9명의 청소년을 연구 참여자로 선정하였으며, 개인별 특수성을 이해하기 위해 질적 사례연구로 진행되었다. 개방형 질문을 사용하여 심층 면담을 1대1로 실시하였다. 운동 경험은 ‘발병 전 운동 경험’, ‘발병 후 운동 경험’으로 분류되었다. 참여자들은 암 발병 전에는 일반 청소년들과 다름없는 운동 경험을 가지고 있었지만, 진단 이후 치료 기간에 급격하게 신체활동량이 감소하였으며, 암 치료가 종료된 이후에도 이전과 같이 충분한 운동 참여를 하지 못하고 있었다. 참여자가 경험하는 운동 제약은 함께 운동할 친구 혹은 장소 부족과 같은 ‘환경적 제약 요인’과 낮은 체력 혹은 치료 후유증과 같은 ‘신체적 제약 요인’으로 분류되었다. 이러한 운동 제약으로 말미암아, 참여자들은 신체 활동 권고량을 충족하지 못하고 있었으며, 비활동적인 생활 습관을 가지고 있었다. 향후 소아청소년암 생존자의 운동 경험과 제약을 깊이 있게 이해하고, 이들이 운동에 활발하게 참여할 수 있도록 지역 사회와 학교에서 다양한 방안을 모색해야 할 것이다. The purpose of this study was to identify the exercise experience and perceived exercise barriers of childhood cancer survivors diagnosed with cancer in childhood and adolescence, and to explore common issues and practical implications in depth. Nine adolescents who were diagnosed with childhood cancer and completed treatment were selected as study participants, and a qualitative case study was conducted to understand individual characteristics. In-depth interviews were conducted one-on-one using open-ended questions. Our results showed that the exercise participation experience was classified as ‘exercise experience before diagnosis cancer’ and ‘exercise experience after diagnosis cancer’. Before cancer diagnosis, participants of the study experienced similar exercise and physical activity (PA) participation as children and adolescents without cancer. However, during cancer treatment, their PA participation decreased dramatically, even after cancer treatment, participants do not recover their pre-diagnosis PA levels. Exercise barriers which they experienced were classified into ‘environmental constraint factors’ such as lack of exercise partner and facilities, and ‘physical constraint factors’ such as such as low fitness levels and side effect of cancer treatment. Due to these exercise barriers, participants did not meet the recommended amount of PA levels for adolescents and had an inactive lifestyle. Based on the in-depth understanding of the exercise participation experience and exercise barriers of childhood cancer survivors, various methods will be proposed about participation in regular exercise in local communities and schools for childhood cancer survivors.

        암경험자를 위한 가정기반 신체활동 증진 프로그램의 임상 현장 적용 가능성 탐색 연구

        PURPOSE: Although physical activity may reduce the risk of cancer recurrence and risk of mortality in cancer patients, majority of cancer survivors are still remained physically inactive. Since most of cancer patients visit their oncologists on a regular basis, it would be ideal to provide exercise counselling program along with their hospital visit. However, the feasibility of hospital based exercise counselling program has not been tested. Therefore, the purpose of this exploratory study was to evaluate the feasibility of hospital-based exercise counselling program on the level of physical activity and body composition in cancer survivors. METHODS: Among 118 cancer survivors underwent exercise counselling program at the cancer prevention center of university affiliated hospital, 47 cancer survivors (mean age, 51.8 years) revisited after an 8-week. At baseline and after 8 weeks, the level of physical activity and body composition were measured. RESULTS: Compared to their baseline value, we observed significant increase in walking (baseline vs. 8 weeks: 170.7±197.8 vs. 362.2±343.4 minutes/week, p<.05) and frequency of resistance exercise (baseline vs. 8 weeks: 0.21±1.2 vs. 2.5±2.5 day/week, p<.05). Percent body fat was decreased (baseline vs. 8 weeks: 30.3±9.6 vs. 29.7±9.5%, p=.025) and skeletal muscle mass was increased at the 8th week follow up visit compared to their baseline value (baseline vs. 8 weeks: 22.9±3.7 vs. 23.2±3.8 kg, p=.019). CONCLUSIONS: The hospital-based exercise program was feasible intervention to increase the level of physical activity and improve body composition in cancer survivors. Considering that the current study is not a randomized controlled trial and also small portion of cancer survivors returned to the follow up test, the efficacy of this exercise counselling program should be tested with proper study design.

        한국 성인의 악력과 당뇨병 유병률의 연관성: 국민건강영양조사 2014-2018

        PURPOSE: The study aimed to examine whether handgrip strength (HGS) expressed as absolute or relative to body weight is associated with fasting glucose (FG), hemoglobin A1c (HbA1c) and the prevalence of diabetes mellitus (DM) in different age categories. METHODS: A total of 28,129 adults from the Korea National Health and Nutrition Examination Survey of 2014-2018 was analyzed. To examine the relationship between HGS and variables related to DM, participants were categorized into three groups according to their HGS (Tertile). Then, participants were further categorized into six groups according to their age. One-way ANOVA and logistic regression analyses were performed. RESULTS: Compared with participants in the upper tertile of absolute handgrip strength (AHGS), those in the lower tertile were older, shorter and heavier and also had higher FG and HbA1c. When age was adjusted, the prevalence of DM was 1.19 times (95% CI: 1.03- 1.38) higher among men in the lowest tertile of AHGS. On the other hand, compared with participants in the highest tertile of relative handgrip strength (RHGS), those in the lowest tertile had 2.10 times (95% CI: 1.87-2.41) and 2.42 times (95% CI: 2.08-2.81) higher prevalence of DM in men and women, respectively. When the associations were examined according to age subcategories, significant associations between AHGS and the prevalence of DM were seen only in men in their 50s and 60s, but not seen in women in all age groups, with the exception of the 60s. However, significant associations between RHGS and the prevalence of DM were seen in all age subcategories. CONCLUSIONS: We concluded that the association between HGS and the prevalence of DM was dependent on age and RHGS is a stonger measure than AHGS.

        수술 직후 유방암 환자의 운동제약 및 촉진요인: 질적 내용분석

        PURPOSE: This study aimed to understand the barriers to exercise and facilitators of exercise for up to 4 weeks following breast cancer surgery. METHODS: A descriptive qualitative research method was used in this study. Twelve patients were recruited through purposive sampling immediately after breast cancer surgery. RESULTS: Physical aspects (pain at the surgery site, reduction in the range of motion, and decrease in fitness), environmental aspects (difficulty in movement due to drain, lack of information on exercise), and psychological aspects (concerns about side effects, fear of pain, and fear of injury during exercise) were identified as barriers to exercise. Expectation of positive effects (recovery from surgery, prevention of lymphedema, usefulness for future radiation therapy, prevention of cancer recurrence, and health management) of exercise and social support (hospital education, support from medical staff, and exercise information received via mass media) were identified as facilitators of exercise. CONCLUSIONS: We recognized different barriers to exercise and facilitators of exercise among patients who recently underwent breast cancer surgery. Future exercise intervention studies should consider minimizing such barriers and maximizing the facilitators identified in our study.

        Purpose: The purpose of this study was to understand the experience of exercise participation among patients following transverse rectus abdominis myocutaneous (TRAM) flap breast reconstruction surgery. Methods: A phenomenological method was used in this study. Exercise experiences for twelve patients, who had undergone TRAM flap breast reconstruction, were collected through focus group interviews. Results: The factors that contributed to exercise barriers in the experience of TRAM flap breast reconstruction patient exercise participation were categorized into 3 groups: ‘fear of exercise after surgery’, ‘weakened emotional condition’, and ‘lack of exercise information’. Exercise facilitators after TRAM flap breast reconstruction were also categorized into 3 groups: ‘desire to improve appearance’, ‘feasiable exercise program’, and ‘exercise experience’. Conclusion: The results of this study reveal the exercise barriers and facilitators for patients following TRAM flap breast reconstruction, which should be considered to develop effective exercise programs.
