        정상인에서 단기간의 Indomethacin 경구 투여가 장 투과성에 미치는 영향

        The side effects of non-steroidal anti-inflammatory drugs (NSAIDs) involve mainly gastroduodenal mucosal lesions, however, recent studies suggest that NSAIDs ingestion can cause also the small intestinal diseases, which are characterized sequentially by inflammation, ulceration, perforation, and diaphramatic disease. This study examined the change of intestinal permeability after short-term oral administration of indomethacin and co-administration of synthetic prostaglandin E₁ analogue, Misoprostol, in healthy men. The subjects drank test solution containing ^(51)Cr-EDTA(100 μCi), and this was followed by 10 hour urine collection. The amount of ^(51)Cr-EDTA in the urine reflects non-mediated intercellular intestinal permeability. Indomethacin significantly increased intestinal permeability to ^(51)Cr-EDTA, and co-administration of prostaglandin E₁ analogue significantly decreased this detrimental effect of indomethacin. These findings suggest that prostaglandins are involved in maintaining intestinal integrity in man.

        Even though hypothyroidism accompanies pericardial effusion, most patients have no symtom, and so its effusion is dignosed accidently. The pericardial effusion may be found in as many as 30% of adults patients, but pericardial effusion as the presenting feature of hypothyroidism is rare cause of dyspnea. We experienced two cases of hypothyroidism presenting with dyspnea in 46 year-old female and 36 year-old female patients. Their pericardial effusion was detected by echocardigraphy, and it resolved simultaneously with subsided dyspnea after 2 or 3 month therapy of thyroxine. (J Kor Soc Endocrinol 7: 295~299, 1992)

        규폐증과 병발한 전신성 경피증 1 예

        Systemic sclerosis is a multisystemic disorder characterized by fibrotic changes of the skin and a variety of internal organs, most notably the gastrointestinal tract, heart, kidney and lung. Silica has been suspected to cause systemic sclerosis identical to idiopathic form. The pathogenesis is not well understood but there is increasing evidence that immune system is involved. We report one male patient with systemic sclerosis and pneumoconiosis. He had occupational history of exposure to silica as a glass factory worker. The clinical and skin biopsy findings were compatible with systemic sclerosis and chest roentgenogram showed increased interstitial markings with multiple calcified hilar lymph node suggestive of pneumoconiosis.

        벤젠 노출 근로자의 생물학적 모니터링 지표로서의 요중 S - Phenylmercapturic Acid 에 관한 연구

        This study was to evaluate the associations between urinary S-Phenyl-mercapturic acid(S-PMA) as a new indicator of biological monitoring for low level of exposure to benzene and independent variables such as the air concentration of benzene in the breathing zone of workers, the years of work, and smoking. In this study the subjects were the total of 145 drawn from 53 workers who were occupationally exposed to benzene and 92 workers who were not. The results were as follows: 1. In the workplace geometric mean concentration of benzene in the breathing zone of workers was 0.31 ppm(0.02 - 3.26 ppm) for the spraying workers and 0.25 ppm(0.02 - 3.95 ppm) for the printing workers. 2. The geometric mean of uninary S-PMA for non exposed group was 8.9 ㎍/g creatinine(0.6 - 72.3 ㎍/g creatinine), 80.3% (74 workers) of the total non-exposed workers indicated less than 20 ㎍/g creatinine of uninary S-PMA. The difference of uninary S-PMA by sex, age, smoking was not significant. 3. The geometric mean of urinary S-PMA for workers who were exposed to benzene was 37.2 ㎍/g creatinine, and was four times higher than that of workers who were not exposed. And 79.3% (42 workers) of the total exposed workers indicated more than 20 ㎍/g creatinine of urinary S-PMA. 4. Regarding the level of benzene in the air, urinary S-PMA was the highest level of 147.9 ㎍/g creatinine in the workers who were exposed to air concentration of 0.5 ppm of benzene and was higher as the level in the air was increased. 5. The correlation coefficient between log urinary S-PMA and log benzene concentration in the breathing zone was 0.80, and the following linear equation was found between urinary log S-PMA and log benzene concentration in the breathing zone : log S-PMA(㎍/g creatinine) = 0.564 log benzene in air(ppm) + 0.192 (n=53, r=0.80, p=0.000) In conclusion, the concentration of S-PMA in urine proved to be good parameter for biological monitoring benzene exposure at the workplace even at low level of benzene in air.

        모 수은폭로 사업장의 작업환경개선에 의한 근로자의 요중 수은 및 공기중 수은 농도의 추적조사 연구

        In order to evaluate the effectiveness of environmental intervention of work place, metal mercury concentration in air and in urine of the total 43 workers for 3years from December 1991 to October 1993 in a fluorescent lamp manufacturing industry exposed to mercury, was measured before and after implementation of controls such as establishing exhaust ventilation at the department of exhaustion, coating the floor of work place with epostane, cleaning of the floor, improved housekeeping, and etc. The results were as follows. 1. Before the intervention(December 1991) 39.0% exceeded metal mercury Threshold Limit Value(TLV, 0.05㎎/㎥). After the intervention(October 1993) 10.1 % exceeded TLV and geometric mean of mercury in air was 0,1㎎/㎥, and showed effectiveness rate of intervention to be 74.4%. 2. After the intervention, geometric means of mercury concentrations in air were 0.013, 0.019㎎/㎥ and showed effectiveness rate of intervention to be 76.6%, 65.5% in A factory(right tube lamp) and at exhaustion department, respectively, A follow up survey fround statistically significant reductions in mercury concentration in air three years later. 3. Mercury concentration in urine of 11 workers(29.7%) exceeded warning level of 100㎕/ℓ before the intervention. After the intervention, of 3 workers(8.8%) exceeded warning level and geometric mean of mercury concentration(26.5㎕/ℓ) in urine was 2.4 times than that of before the intervention. Geometric means of mercury concentrations in urine of workers at exhaustion department, at sealing and aging department were 44.0, 77.7㎕/ℓ, respectively and they decreased 2.3, 3.2 times than that of before the intervention.

        심낭 삼출액과 복수를 주 중상으로 한 갑상선 기능 저하증 1 예

        Background: Hypothyroidism accompanies pericardial effusion frequently, but the patient who complains of any symptom of pericardial effusion is very rare. The diagnosis of pericardial effusion is usually made incidentally by routine check-up. Moreover it is extremely rare that pericardial effusion makes the presenting symptom of hypothyroidism. Methods: We have exprienced a case of hypothyroidism presenting with the symptoms of dyspnea and abdominal distension in a 46 year-old female patient. The cause of dyspnea and abdominal distension was massive pericardial effusion and ascites respectively. Results: The diagnosis of pericardial effusion and ascites was made by echocardiography and ascites tapping. The pericardial effusion and ascites did not subside inspite of the repeated pericardiocentesis and paracentesis, but 3 month later they were completely treated with thyroid hormone supplementation. Conclusion: So we recommend the thyroid function test in the case of massive pericardial effusion and ascites even if there was little symptom of hypothyroidism.

        교원병에 동반된 단백소실장질환 ( Protein Losing Enteropathy ) 에서의 테크네슘표지 인혈청알부민 스캔

        N/A Anterior abdominal scintigraphy after intravenous injection of Tc-99m-human serum albumin (Tc-99m-HSA 20 mCi) was done in 16 patients with connective tissue diseases and 15 healthy control patients. Patients with proteinuria or hepatopathy were excluded. 1) 7(44%) patients among 16 connective tissue disease patients without the apparent evidence of external protein loss showed abnormal intestinal accumulation of albumin. 6 patients with positive albumin scintigraphy showed hypoalbuminemia. 2) There was no false positive scintigraphic finding in control group. 3) The serum albumin level in connective tissue disease patients (3.1±0.6 g/dl, n=16) was lower than control patients(3.9±0.3 g/dl, n=15) (p〈0.0001). 4) The serum albumin level was lower in connective tissue disease patients with positive Tc-99m-HSA scan(2.8±0.6 g/dl, n=7) than the connective tissue disease patients with negative scan(3.3±0.3 g/dl, n=9) (p〈0.05). 5) The hemoglobin level in connective tissue disease patients with positive scan(10.6±2.91 g/dl) was lower than that of the control group (13.6±1.5 g/dl) (p〈0.05). Hypoalbuminemia is frequently involved in chronic connective tissue diseases. Protein losing enteropathy(PLE) is also responsible for the majority of the hypoalbuminemia in these patients, But it has been ignored because the conventional method for the diagnosis of PLE was difficult to difficult to perform. Tc-99m-HSA scan also must be validated by more extended study and comparison with the quantitative study such as stool α-1 antitrypsin measurement. There must be a reevaluation of PLE in various diseases especially in connective tissue diseases with easy, fast, economical, and noninvasive method.
