The purpose of the study is to develop a Web-based health information service system for the disabled. To identify users' information needs, we conducted an online and offline survey of the disabled and their caregivers as well as welfare institutes' staff. Based on the user's information needs, a Web-site was developed. The Web-site was evaluated by users for their satisfaction with the Web-site and by experts for the quality of the site. Content of the Web-site includes information on rehabilitation, welfare, and diseases causing the disability, definition and classification of disability, prevention from disability, management of the disabled, and disability related policies. Besides this information, FAQ on the disabled, statistics, links to the disabled institutes and Web sites, risk factor appraisal tool, and aiding tools for the disabled are added. The Web site is a subsystem of the Healthguide. Users rated efficiency, convenience, and design of the site highly in their satisfaction rating. Staff at welfare institutes rated design, efficiency, and relevance of the site highly in their satisfaction rating. Experts rated easiness, reliability, clearness and appropriateness of the site highly in their site evaluation. It is expected that this Web-based health information service system for the disabled will contribute to the disabled's health promotion as well as provide a community for the disabled related researchers, professionals and experts to share information on the disabled's health.

간질환 환자의 대체요법 이용실태 및 의사결정요인

Liver disease was characterized as a difficult disease to cure and long-standing. The patients with liver disease take various types of the treatment-seeking behaviors including using of alternative therapies throughout their disease process.This study was conducted to examine utilization of alternative therapies and understand decision factor related to use ofalternative therapies among patients with liver disease.This study used the methodological triangulation. In the first, the decision factor related to alternative therapies were described based on qualitative data collected from in-depth interviews with 22 patients with liver diseases. Next, theywere tested using quantitative data collected from a structured questionnaire andinterviews involving 126 patients with statistical method.Sixty four point three percent of subjects had experience of using alternative therapies and 31.7% were still using alternative therapies. On the average the patients used alternative therapies for 25.5 months and they paid 165,000 Won per month foralternative therapies. Decision criteria related to use of alternative therapy were complications, social group influence about alternative therapy, confidence in alternative therapy, secrecy about alternative therapies to doctor, doctor's approval, and who decide on the use of alternative therapy. These variables in the alternative therapy group were statistically different from those of the hospital treatment only group.The results from this study can help healthcare personnel understand the education needs of liver patients regardingusing alternative therapies, and hence aid in the development of educational strategies for patients with liver disease. (Korean J Health Promot Dis Prev 2005 ; 3 :212~222) 간질환은 원인이 복합적이고 급성질환과는 달리 환자역할의 기간이 길어지고 질병의 조절만 되고 완치가 되지 않아 , 환자들은 병원치료를 하면서도 다양한 대체요법을 이용하고 있는 실정이다 . 본 연구는 간질환 환자들이 치료추구행위로서 대체요법 이용에 대한 실태와 이에 관련된 의사결정요인을 파악하고자 시행되었다 .본 연구는 질적 , 양적 연구방법론을 활용한 방법론적 트라이앵귤레이션을 이용하였다 . 첫 번째 단계로 , 간질환 환자 22명을 대상으로 심층면접을 통하여 대체요법 이용과 관련된 의사결정요인을 기술하였다 . 두 번째 단계로 , 126명의 간질환 환자들을 대상으로 대체요법 이용실태 및 의사결정요인 등을 포함한 구조적 설문지와 면접을 통해 자료를 수집한 후 통계적 방법 으로 자료를 검증하였다. 대상자의 64.3%가 진단 후 대체요법 이용 경험이 있었으며 병원치료를 받고 있는 현재에도대체요법을 이용하고 있는 대상자는 31.7%이었다. 대체요법 이용과 관련된 의사결정요인은 간질환 합병증 여부 , 대체요법에 대한 신뢰 , 대체요법 이용에 대한 준거집단의 영향, 의사의 동의 및 대체요법 이용에 대한 비밀 , 그리고 의사결정 주체자로 나타났고 , 이러한 변수들은 대체요법과 병원치료를 병합하고 있는 군과 병원치료만 단독으로 하고 있는 군과 비교해보았을 때 통계적으로 유의한 차이를 보 였다.간질환 환자의 대체요법 경험율은 비교적 높았으며 대체요법 이용과 관련된 의사결정요인은 의사의 동의와 대체요법 이용에대한 비밀이 가장 크게 영향을 미치고 있었다. 본 연구는 간질환 환자의 대체요법 이용에 대한 의사결정과정의 이해로 인해올바른 치료를 위한 간호교육전략을 세우는데 기초 자료로 활용할 수 있을 것이다.

The study was conducted to project supply and demand of the physicians from year 1991 to year 2010 based on the analysis of supply and demand of the physicians up to year 1989. Results of the study will provide information for the physicians manpower planning of the 7th 5-year Economic Social Development Planning(1992-1996) and contribute to the overall health manpower planning for the 21the century. It is projected that physician will be oversupplied from the very near future based on the current productivity or underestimated based on the optimal productivity. Thus, it is desirable not to change size of training and education during the 7the 5-year planning period and re-examine the status of the physician manpower at the end of the 7th 5-year period taking into consideration medical services utilization pattern, patients' satisfaction, and physicians' productivity.

An Introduction to Logistic Regression: From Basic Concepts to Interpretation with Particular Attention to Nursing Domain

Purpose: The purpose of this article is twofold: 1) introducing logistic regression (LR), a multivariable method for modeling the relationship between multiple independent variables and a categorical dependent variable, and 2) examining use and reporting of LR in the nursing literature. Methods: Text books on LR and research articles employing LR as main statistical analysis were reviewed. Twenty-three articles published between 2010 and 2011 in the Journal of Korean Academy of Nursing were analyzed for proper use and reporting of LR models. Results: Logistic regression from basic concepts such as odds, odds ratio, logit transformation and logistic curve, assumption, fitting, reporting and interpreting to cautions were presented. Substantial shortcomings were found in both use of LR and reporting of results. For many studies, sample size was not sufficiently large to call into question the accuracy of the regression model. Additionally, only one study reported validation analysis. Conclusion: Nursing researchers need to pay greater attention to guidelines concerning the use and reporting of LR models.

국가핵심기술 보호제도: 모자이크 이론의 역설

우리나라는 해외유출 시 국가안보 등에 중대한 영향을 줄 수 있는 기술을 국가핵심기술로 지정하고 관련 법제 및 지원사업 등을 통해 보호하고 있다. 그러나 여전히 국가핵심기술에 대한 인식이 부족하고 보호제도의 실효성이 부족하다는 주장이 제기되고 있다. 뿐만 아니라 국가핵심기술 보호 관련 연구는 매우 부족하고 특정한 주제 분야에 치우쳐져 있어 제도 개선에 한계가 있다. 이에 본 연구에서는 모자이크 이론 관점에서 현 국가핵심기술 보호제도의 문제점을 고찰하고 ‘과한 비공개를 통한 보호’는 오히려 보호를 취약하게 할 수 있다는 역설에 대해 논의한다. 국가핵심기술 보호제도의 문제점에 대한 근본적인 해결방안으로 ‘적정 수준의 공개를 통한 보호’를 제안하고, 국가핵심기술 실태조사의 부분적 공개방안, 국가핵심기술 명장제도 등을 세부 방안으로 제시하였다. 본 연구는 모자이크 이론의 관점에서 국가핵심기술 관련 현 제도의 문제점을 논하였다는 점에서 학술적으로 의미가 있고, 실무적으로는 국가핵심기술 보호제도의 문제점 개선에 기여할 수 있을 것으로 기대한다. In Korea, technologies that have a significant impact on national security when it releases to foreign nations are designated as national core technologies and the technologies are protected through related laws and support projects. However, it has been argued that there is a lack of awareness of the national core technology and the effectiveness of the protection system is not sufficient. In addition, existing studies related to the protection of national core technologies are insufficient and oriented to specific subject areas, indicating the improvement of the system. Therefore, this study examines the problems of the current national core technology protection system from the perspective of the mosaic theory and confirmed the paradox that “protection through excessive confidentiality” could ratherweaken protection. As a fundamental solution to the problems of the national core technology protection system, “protection through the appropriate level of disclosure” is proposed. A partial disclosure plan of the state-of-the-art national core technology survey, and the national core technology master system are also suggested. This study has academic significance in that it discusses the problems of the current system of the national core technology of Korea from the perspective of the mosaic theory. It is also expected that this study may contribute to solving the problems of the national core technology protectionsystem in practice.

Clinical Terminologies: A Solution for Semantic Interoperability

To realize the benefits of electronic health records, electronic health record information needs to be shared seamlessly and meaningfully. Clinical terminology systems, one of the current semantic interoperability solutions, were reviewed in this article. Definition, types, brief history, and examples of clinical terminologies were introduced along with phases of clinical terminology use and issues on clinical terminology use in electronic health records. Other attempts to standardize the capture, representation and communication of clinical data were also discussed briefly with future needs.
