        갑상선 절제술 중 발견된 총경동맥 꼬임 1예

        An abnormal course of carotid artery may surprise surgeons during surgery and result in an transient ischemic attack or other ischemic symptoms. We report an unusual case of kinking in the common carotid artery located below the right thyroid lobe, which was found during a total thyroidectomy in a 74-years old patient. We emphasize in this case how important it is to palpate the neck before surgeries such as tracheostomy, thyroid surgery and so on. (Korean J Otorhinolaryngol-Head Neck Surg 2008;51:911-3)

        측두골 외상후 진주종성 중이염으로 오인된 중이강내 뇌허니아 1예

        Brain herniation into the middle ear cavity is a rare entity that occurs mostly as a complication of otologic surgery. Other causes include a congenital skull base defect, infection, trauma, neoplasm and irradiation. It gives rise to cerebrospinal fluid otorrhea, progressive hearing loss, tinnitus, meningitis and other neurologic symptoms. Such patients tend to be misdiagnosed as having chronic otitis media and are often treated for long periods with inadequate conservative therapy. We report a case of brain herniation into the middle ear following traumatic temporal bone fracture, which was treated surgically via a transmastoid approach.         Lev Shestov’s Groundlessness in D. H. Lawrence and Georges Bataille

        It has become one recent trend in literary criticism to use Georges Bataille’s theory as an efficient reading tool of D. H. Lawrence’s novels. Many literary critics successfully locate the radical affinity of Lawrence’s work with Bataille’s work in terms of the recurrent questioning of the representing authority and the highly provocative and experimental narrative style. Clearly these approaches cannot be easily ignored as opportunistic revision of D. H. Lawrence studies. In fact, both Lawrence and Bataille are intellectually indebted to Lev Shestov. They embrace the unique combination of non-dogmatic thinking and aphoristic narrative style in Shestov’s work. And the inventory of the Shestovian properties in the works of Lawrence and Bataille is rather extensive. However, any unmediated application of Bataille’s theory to Lawrence’s work fails. To appreciate and substantiate the complexity of the relationship between them we need to investigate their deceptively close similarities.

        세계시민주의와 D. H. 로렌스

        Globalization, once hailed as the spirit of redistribution and equality, is rapidly degenerating into a conglomerating organism of capitalist’s selfish desire. Recent critical engagement with globalization leads to an attempt to find an alternative in the resurging cosmopolitanism. For instance, Jacques Derrida rejuvenated the 18th century urbane epistemology of cosmopolitanism in opposition to the French patriotic and xenophobic oppression on immigrants. Derrida urged us to remember the ancient practice of hospitality towards others, and replicated Immanuel Kant’s cosmopolitan project of equality and universality. Yet we witness cosmopolitanism, especially combined with secularism, revealing its properties of parochialism. Étienne Balibar warns us against the danger of secular cosmopolitanism when he tries to rescue the hijap wearing female Islamic students at school from legal punishment in France. This returns us to D. H. Lawrence who lived in the twentieth century cosmopolitan Europe and mediated on the possible implications and manifestations of cosmopolitanism. In his writings Lawrence foresees the fatal conflation of cosmopolitanism and universalism and its impact on human psychology.

        좌측 악하부 종괴로 나타난 매독성 림프절염 1예

        Syphilis, a chronic systemic infection caused by Treponema pallidum, is usually sexually transmitted and characterized by episodes of active disease interrupted by a latent period. After an incubation period of averaging 2-6 weeks, a primary syphilis with a firm, non-tender chancre appears, often associated with regional lymphadenopathy. Secondary syphilis shows localized or diffuse mucocutaneous lesions and generalized non-tender lymphadenopathy. In about onethird of untreated cases, the tertiary stage appears, characterized by progressive destructive mucocutaneous, musculoskeletal or parenchymal lesions, aortitis or symptomatic central nervous system diseases. Without serological testing, the precise diagnosis is very difficult. In fact, it has often been called the great imitator because it was often confused with other disease. We report a very unusual case of luetic lymphadenitis presented as a solitary submandibular mass.

        Isolated sudden hearing loss with vertigo is usually peripheral origin. We report two cases with anterior inferior cerebellar artery infarction (AICA) manifesting sudden hearing loss with vertigo as an isolated symptom. Patient 1 was a 64-year-old man presented with right sided sudden hearing loss and vertigo accompanying horizontal beating nystagmus to the left. He had no other neurologic symptoms. MRI showed right AICA infarction involving lateral pons and middle cerebellar peduncle. Patient 2 was a hypertensive 56-year-old man. Left sided sudden hearing loss with vertigo was as an initial manifestation. Two days later, left sided facial palsy developed and MRI showed acute infarction in left lateral pons, middle cerebellar peduncle, and cerebellum. AICA infarction can be presented the hearing loss and vertigo as an isolated symptom and mimic the syndrome of peripheral origin.         D. H. 로렌스의 「섬을 사랑한 사나이」와 세계시민주의

        D. H. Lawrence, in his “The Man Who Loved Islands,” challenges against Kant’s separation of space from time and geographical unification. The story, as if accommodated in fable-like narrative, is invested with the author’s philosophical investigation on the Kantian geographical cosmopolitanism. Harvey’s critical engagement with the Kantian geography casts new light on Lawrence’s reflection on space and time in the story. Lawrence, who lived in the twentieth century cosmopolitan Europe and mediated on the possible implications and manifestations of cosmopolitanism, foresees the fatal conflation of cosmopolitanism and universalism and its catastrophic impact on human psychology.
