하부요로 폐색 증상을 보인 고령 환자에서 진단된 원발성 전립선 림프종 1예

본 예는 전립선 비대로 인한 하부요로 폐색증상을 호소하는 고령의 환자에서 방사선 검사 및 생검을 통해 원발성 전립선 림프종을 진단한 에로 국내에서 아직 보고된 증례가 없기에 문헌고찰과 함께 보고하는 바이다. Malignant lymphoma involving the prostate, whether presenting as primary extranodal lymphoma or as secondary spread to the prostate from other site, are rare. Most are secondary, so primary lymphoma of the prostate is extremely rare. We report a case of primary prostate non-Hodgkin's lymphoma who presented with symptoms of lower urinary tract obstruction and constipation. The patient underwent cyclophosphamide, vincristine, prednisone combination chemotheraphy for 3 cycles and than now under involved-filed radiotheraphy.

다문화가정 결혼이주여성의 인권 개선방안 - 대구광역시를 중심으로 -

본 연구는 선행연구를 통해 한국의 다문화가정에 있어 결혼 이주여성들이 경험하는 배우자 및 가족에 의한 가정폭력과 사회적·제도적으로 인한 인권침해의 실태를 살펴보고그 해결책을 찾고자 한다. 최근 우리사회에서 문제가 된 다문화가정의 결혼 이주여성을상대로 한 가정폭력 사건들을 살펴보고, 이러한 문제를 유발하는 국제결혼의 특징과 결혼 이주여성들의 결혼 동기 및 경로에 따른 사회 인구학적 특징에서 찾아볼 수 있어,2006년 경기가족여성개발원의 조사를 기본 자료와 2006년부터 2010년까지 대구광역시 여성 긴급전화 1366을 통해 남편의 가정폭력으로 인해 1366을 찾은 결혼 이주여성들의 상담내용을 기초로 하였다. 이들이 느끼는 실질적인 다문화가정 결혼이주여성들의 어려움과 폭력상황의 심각성으로 인한 인권침해에 대해 살펴보고 이에 대한 지방자체단체 차원에서의 해소방안을 찾고자한다. This study is about domestic violence on migrant women caused by the spousesand families in multicultural families and Human Rights Violations caused by social and institutional reasons in South Korea. Furthermore, in order to find a solution tothe recent domestic violence incident that was investigated against immigrant womenin the multicultural issues in our society, we studied the motivations and pathcharacteristics and features of international marriages. We also studied to find outsolutions on difficulties among international marriage families and Human RightsViolations caused by domestic violence.

본 연구는 정신장애인의 장애 등록 과정을 당사자의 관점에서 이해하는데 그 목적이 있다. 이러한 목적을 위하여, 정신장애 등록을 한 후 약 2년이 경과한 정신장애인 6명을 대상으로 2012년 5월부터 8월까지 심층 면접을 통하여 자료를 수집하였다. Colaiazzi의 현상학적 접근을 활용하여 분석한 결과, 정신장애인들이 장애 등록 과정에서 경험한 본질은 3개의 영역과 6개의 주제, 12개의 범주로 나타났다. 3개의 영역은 ‘부정과 머뭇거림’, ‘직면과 갈등 그리고 자기 수용’, ‘보호’로 나타났다. 6개의 주제는 ‘내려 놓음’, ‘문턱 위에서’, ‘정신장애인으로 산다는 것은’, ‘가면 뒤에서 살아가기’, ‘똑같은 사람’, ‘울타리 안으로 들어옴’으로 나타났다. 이러한 연구 결과를 바탕으로 정신장애인의 장애등록 경험에 대하여 심층적으로 논의를 하였으며 정책적, 실천적 제언을 하였다. This study aims to understand the life experiences of the mentally ill regarding disability registration from their own standpoints. To achieve this objective, the 6 mentally disabled, who have been two years after the registration period were selected for interviews. Data were collected through in-depth interviews from March to June, 2012. Collected data were analyzed with Colaizzi's Method in phenomenological analysis. As a result, the topics were identified as the nature of three domains, six subjects, and twelve Categories. Based on these findings, the study discussed experiences of people with mental disabilities about disability registration and political and practical suggestions for social work fields made.

요양보호사의 직업적 정체성이 이직의도에미치는 영향에 관한 연구 -직무만족의 매개효과-

The purpose of this study was to find out how to reduce turnover intention by enhancing occupational identity and job satisfaction through examining the relationship between the occupational identity and the turnover intention through the mediation effect of job satisfaction of case managers in nursing homes. We surveyed 271 case managers in the Gyeong Buk province of south korea. There is positive correlation between occupational identity and job satisfaction while there is negative correlation between occupational identity and turnover intention. Occupational identity of case managers influenced job satisfaction and turnover intention directly. This study proved that job satisfaction has a mediation effect between occupational identity and turnover intention. We suggested educational support, improvement of work environment and Considering the job and organizational environment, provide compensation system for case managers. Promotion of the local community is also critical for case managers. 본 연구는 요양보호사의 직업적 정체성과 이직의도 간의 인과관계와 두 변인 사이에서 직무만족의 매개효과를 살펴봄으로써 직업적 정체성, 직무만족의 강화를 통해 이직의도를 낮출 수 있는 방안을 강구하는데 그 목적이 있다. 이를 위해 경북지역 271명의 요양보호사를 대상으로 설문조사를 하였으며 조사결과를 바탕으로 직업적 정체성과 직무만족과 이직의도와의 관계를 살펴본 결과 직업적 정체성과 직무만족은 정(+)적 상관, 이직의도와는 부(-)적 상관관계가 있는 것으로 나타났다. 또한, 요양보호사의 직업적 정체성이 직무만족과 이직의도에 직접적인 영향을 주었으며 직무만족은 직업적 정체성과 이직의도에 매개효과가 있음이 통계적으로 확인되었다. 이를 바탕으로 요양보호사의 직업적 정체성과 직무만족 강화를 통해 이직의도를 낮출 수 있는 개선방안으로 교육의 지원, 직무 및 조직환경을 고려한 보상체계 마련, 요양보호사의 사회적 역할 및 기능에 대한 지역사회홍보가 필요함을 제안하였다.

A Butterfly-Shaped Primary Cardiac Lymphoma That Showed Bi-Atrial Involvement

We described here a patient who presented with symptoms of heart failure who was found to have severe bilateral impairment of atrioventricular inflow. Primary cardiac lymphoma (PCL) with extensive involvement of the two atria, pericardium and myocardium is an extremely rare tumor in immunocompetent patients. We report here a case of PCL in an immunocompetent patient with involvement of both atria and the atrial septum. The tumor had a butterfly shape. We could not do surgical excision because of the massive pericardiac invasion. The diagnosis was B-cell lymphoma and this was confirmed by the pericardiac biopsy.

A Case of Three Consecutive Events of Acute Myocardial Infarctions in Three Different Vessels

A 51-year-old man was being admitted to the emergency department with chest pains. He had a history of acute myocardial infarction (MI) on two prior occasions and was successfully treated with drug eluting stents. He was diagnosed with 3 consecutive events of acute MI in 3 different vessels. The consecutive events of acute MI in different vessels are a very rare case. He did not have risk factors, such as coagulation abnormality, clopidogrel resistance, patient’s compliance and vessel abnormality, except for his cigarette smoking. We reported the first case with 3 consecutive events of acute MI in each 3 vessels during a long-term interval.
