복압성 요실금 환자에서의 Tension-free Vaginal Tape수술과 Transobturator Vaginal Tape Inside-out 수술의비교 연구

Purpose: We compared the preoperative with the postoperative outcomes between tension-free vaginal tape (TVT) and transobturator vaginal tape inside-out (TVT-O) for the surgical treatment of female stress urinary incontinence Materials and Methods: From January 2003 to December 2005, 72 women, one is TVT group (n=52) and the other is TVT-O group (n=20), after operation following were assigned. The preoperative and postoperative evaluations analysis and urodynamic study and Korean version of I-QoL, surgical outcomes, complications and uroflowmetry were analysed. Results: All outcomes included the patients`states, incontinence quality of life (I-QoL)(p=0.013), the success rate of the cure (p=0.003) and the rate of the patient satisfaction (p=0.001) were not comparable statistically between two groups and there are no differences and no long-term complications such as bladder perforation or prolonged voiding difficulty. Preoperative and postoperative uroflowmetry was similar between the two groups. Mean operation time (TVT: 15.6 min, TVT-O: 13.2 min) was significantly shorter in TVT-O than in TVT. Conclusion: The observation of 1-year following-up shows the equally effectiveness between two groups (TVT and TVT-O) as the surgical treatment of stress urinary incontinence. (J. Background: We wanted to evaluated the long term results of the tension-free vaginal tape (TVT) Procedure for treating strss urinary incontinence in women. Method: During Octover 1, 1998 to December 31, 2005, we evaluated the 52 women who underwent the TVT procedure for stress urinary incontinence at least 1 year following surgery. Preoperatively, Patients were evaluated with history taking, Physical examinations and urodynamic study, Operation and hospitalization data were evaluated with duration of procedure, hospital days, foley catheter removal days and use of analgesics, Postoperative evaluations included the uroflowmetry, residual urine volume, surgical outcomes, complications and patients' satisfaction with the procedure. Results: The follow-up period was a mean of 16 months. Preoperative I-QoL total score was 58.0. Comparision the result between preoperative and postoperative Q max was significantly shoter in the postoperative result. TVT procedure remained cure in 96.2% (cured: 82.7%, improved: 13.5%) and successful in 96.1%. Also, 49(94.2%) Patients would like to recommended the TVT procedure to others. There was no serious or long-term complications, Conclusions: The TVT procedure appear to be effective and safe for the surgical treatment of female stress urinary incontinence and showed a good long-term cure rate.

Laminar flow and heat transfer in a channel with blockages are obtained numerically in a Reynolds-number range of $100{\leq}Re{\leq}400$. A boundary-fitted curvilinear coordinate system is generated for irregular boundary of the physical region, and solutions of Navier-Stokes equation and energy equation are obtained by finite analytic method in the transformed computational domain. The flow separates in downstream of the blockage and the length of separated-flow region increases with Reynolds number. The heat flux is high on the top of the blockages and increase in the heat transfer occurs where the fluid reattaches the wall. Comparison between computed streamlines and experimental flow-visualization is also presented and discussed.

The machinery accompanying the flow in rotating internal passage have the complex 3 dimensional Turbulent flow with high Reynolds Number in order to design these kind of machinary the Turbulent flow characteristics should be analyzed accurately. Rotation has a strong effect on the structure of Turbulent flows. In this study the influence of Rotation speed on the flow regine was investigated. The semi-parabolic solving procedure is used to prodict the Turbulent flow in channel, and the results are compared with Koyama's Experimental data. A channel of 760㎜ in Length with Cross section of 40×40㎜ is Rotated up to Speeds of 100 rpm. Air is used as the working fluid.

The fields of turbomachinery and electrical generators provide many examples of flow through internal passages, at the range of Reynolds Numbers occuring in practice. The flow motion is three dimensional and highly turbulent in most of cases. An adequate understanding. for the characteristic of these turbulent flow is needed for the design of thermo-fluid machinary with a good efficiency. The effect of the curvature on th structure of turbulence in the curved passage have been extensively by many workers, but the effects of curvature have not been well understood inspite of the impotance of the phenomena in the practical design process. In this study, the characteristics of the three-dimensional turbulence flow in a square sectioned 90o bend were investigated by numerical simulation and experiment, and the results were compared with the results obtained by simulation with four near wall turbulence models and experiments.
