아밀로이드증은 불용성의 단백성 섬유 물질이 체내 조직에 다발성으로 침착되어 장기의 기능 장애를 초래하는 질환이다. 위장관은 아밀로이드증의 침범 부위 중 하나로, 위장관 아밀로이드증은 식욕 부진, 오심, 구토, 복부 팽만 등의 증상과 함께 흡수장애, 궤양, 장폐쇄, 경색과 천공 등 다양한 임상 양상과 경과를 보이지만 반복적 혈변을 주 증상으로 나타내는 경우는 매우 드물다. 저자들은 6개월 동안 10회 이상의 반복적 혈변을 주소로 내원한 68세 여자에서 대장내시경을 통해 횡행 결장의 다발성 윤상 궤양과 대장의 점막 부종 및 점막하 출혈을 확인하고, 직장 점막 조직 생검을 통해 드물게 위장관을 침범하는 AL형 아밀로이드증으로 진단된 증례를 경험하였기에 문헌고찰과 함께 보고한다.

Recurrent tuberculosis can be caused by a relapse of the original infecting strain, a re infection with a new strain of Mycobacterium tuberculosis or a paradoxical response associated with the anti-tuberculosis treatment. A drug susceptibility test is important when evaluating recurrent tuberculosis but a conventional microbiological test may be unavailable or unsuccessful. Recently, direct molecular testing as a line probe assay (GenoType® MTBDRplus, Hain Lifescience GmbH, Nehren, Germany) has shown promising results for the diagnosis or treatment of drug-resistant tuberculosis. We present two cases of rifampin-resistant, relapsed tuberculosis that were confirmed using a molecular technique.
Candida albicans 의 바이오필름 (Biofilm) 형성 기전과 병태 생리

Candida species are frequently found in the normal microorganism of humans, which facilitates their encounter with most implanted biomaterials and host surfaces. Medical devices such as stents, prostheses,implants and various types of catheters have all been shown to support colonization and biofilm formation by Candida. Candida albicans remains the fungal species most commonly associated with biofilm formation and the increase in Candida infections in the last decades has almost paralleled the increase and widespread use of a broad range of medical implant devices, mainly in populations with impaired host defenses. The formation of C. albicans biofilms carries important clinical repercussions because of their increased resistance to antifungal therapy and the ability of cells within biofilms to withstand host immune defenses. Also, biofilm formation on medical devices can negatively impact the host by serving as a reservoir or source for future continuing infections. This review article aims to provide insights on various aspects of C. albicans biofilms, formation and structure, their role in pathogenesis and antifungal drug resistance. C. albicans는 건강한 사람의 구강이나 질 내,위장관에서 정상적으로 집락을 이루고 있으나 면역력이 떨어진 사람에게는 기회 감염을 일으키며,혈행성 전신 파종증으로 발전될 수도 있다. 현재C. albicans는 병원에서 획득되는 감염의 매우 주요한 원인균으로 자리하고 있다. 면역억제제나세포 독성 약제의 사용, 정상 세균을 억제하는강력한 항생제 처치, 그리고 여러 가지 인체 삽입기구 또는 보조물 이식 등 많은 의학적 처치들이 칸디다증 발생을 유발하는 위험 요인으로 작용하고 있다. 대부분의 C. albicans는 자연 환경에서 자유롭게 떠있는 상태보다는 세포외 물질들의 표면에 고착하여 바이오필름 형태로 존재하는데, 생체에 생기는 바이오필름은 칸디다 감염증 성립과 그 발전 과정에서 결정적인 역할을 한다. 또한 생체뿐만 아니라, 카테터, 삽입 보철물등의 의학적 처치기구에 형성되는 바이오필름은지속적 칸디다 감염증의 감염원으로써 매우 중요한 의미를 가진다. 또한 삽입 의료기구의 사용이증가함에 따라 바이오필름 관련 감염 발생은 현재는 물론 앞으로 점점 더 커지게 될 의학적 위급 상황이 되고 있다. 따라서 이를 해결하기 위한 C. albicans 바이오필름의 제어 방법 개발 및항진균제 개발을 위하여, 바이오필름에 대한 군집 및 개체군 수준, 그리고 세포 및 분자 수준에서의 총체적인 연구가 반드시 시행되어져야 할것이다.
증례 : 소화기 ; 가족성 위-식도 미만성 평활근종증을 보인 2대의 Alport 증후군

위-식도 미만성 평활근종증은 평활근세포의 비정상적 증식에 의해 위-식도벽이 매우 두꺼워지는 드문 양성의 질환으로, 산발성 혹은 상염색체, 성염색체 유전으로 발생한다. 특히 세대에 걸쳐 평활근종증이 나타나는 경우 Alport 증후군과 밀접한 유전적 연관성을 갖고 있음이 1983년 보고되었고, 이후 이에 대한 몇몇 증례들이 발표되었다. 저자들은 신기능 저하, 난청 및 선천성 백내장으로 Alport 증후군을 진단받고, 동반된 위-식도 미만성 평활근종증에 대한 수술적 절제를 시행 받은 남아의 어머니에게서 다른 증상의 동반없이 위-식도 및 성기의 미만성 평활근종증을 확인하고 수술적 치료를 시행한 증례를 경험하였다. 이는 Alport 증후군 보유자로 의심되는 여자가 자신의 남아에게 심각한 Alport 증후군을 유전시키고, 이와 함께 2대에 걸쳐 위-식도 미만성 평활근종증이 유전된 예로 아직까지 국내에 보고된 바 없기에 이를 보고하는 바이다. Diffuse gastroesophageal leiomyomatosis is a rare, benign neoplastic condition characterized by thickening of the esophageal wall as a result ofaberrant smooth muscle proliferation. Gastroesophageal leiomyomatosis may occur in isolation or as a familial disorder and has been associated with other conditions, such as Alport syndrome. Alport syndrome is a hereditary disorder of basement membrane type IV collagen, characterized by progressive nephritis, sensorineural deafness and ocular abnormalities. In this article, we present an interesting case of gastroesophageal leiomyomatosis occurring in a single family in whichthe mother and her son had Alport syndrome. This case is the first report in Korea that gastroesophageal leiomyomatosis is associated with two generations of Alport syndrome. (Korean J Med 79:171-176, 2010)
침습성 폐 아스페르길루스증의 역학과 임상상: 전국 다기관 연구

Background: Invasive pulmonary aspergillosis (IPA) is an important cause of morbidity and mortality in immunocompromised patients. However, few data on clinical characteristics and outcomes of IPA in Korea have been reported. We conducted a nationwide multicenter study in Korea for evaluation of the epidemiology and clinical outcomes of invasive pulmonary aspergillosis. Materials and Methods: A retrospective cohort study was conducted in 10hospitals in Korea. We reviewed all adult patients who met the revised EORTC/MSG definitions between 2008 and 2010. Results: A total of 334 cases, which included proven (26, 8%), probable (159, 48%),or possible (149, 44%) IPA, were identified. Patients with proven or probable IPA were evaluated, and, of these 185 IPA patients, 105 (57%) had neutropenia, 30 (16%)underwent hematopoietic stem cell transplantation, 25 (14%) underwent solid organ transplantation, and 32 (17%) without neutropenia and transplantation received immunosuppressive agents or corticosteroid. Aspergillus spp. were isolated from 42 patients (23%), and positive fungal culture rates from sterile fluid, sputum, and bronchoalveolar lavage fluid (BAL) were 67% (6/9), 21% (32/150), and 20% (9/44),respectively. Results of assays for sensitivity of serum and BAL galactomannan were 84% (155/184) and 89% (25/28), respectively. Amphotericin-B deoxycholate and itraconazole were most commonly administered as a primary therapy in 107 (58%)and 34 (19%) patients, respectively. Of 133 patients (73%) who received salvage therapy after primary antifungal therapy for a median period of six days (IQR 3-12), 82(62%) patients were treated with voriconazole. Of 185 patients, 82 (44%) died within three months after diagnosis of IPA. CT findings, including small airway lesions and micronodules, ground glass opacities, and pleural effusion and persistent positive galactomannan status showed an independent association with worse outcome, while proven diagnosis of IPA showed an independent association with better outcome. Conclusions: Microbiologic confirmation of IPA was low in Korea; therefore, many Korean physicians were dependent on the galactomannan assay for microbiologic diagnosis. Primary therapy with Amphotericin-B deoxycholate followed by salvage therapy with voriconazole was the most common antifungal strategy for treatment of patients with IPA in Korea. Overall mortality and IPA-related mortality were comparable with data from Western clinical trials.
