        4 개의 굴절면을 갖는 모형안 설계

        한국인 표준모형안을 설계하기 위하여 서울보건대학 안경광학과 재학생 105 명(평 균 23.5 세)을 대상으로 자각식 굴절검사, 각막전후변의 곡률반경 및 두께. 전방깊이, 수정체두께 및 안축장을 측정하였다. 수정 전후변의 곡률반경은 Bennett 식을 기초로 계산하였다. 모형안을 설계하는데 필요한 여러 가지 광학상수에 대힌 데이터를 수집 하기 전에 광학적인 방법과 초음파를 이용한 측정법의 신뢰도와 정확도를 비교분석 하여 가장 적합한 측정볍을 선택하였다. 모형안의 등가굴절력과 주요점을 계산하기 위하여 안내매질의 굴절활은 Gullstrand - Emsley 모형안 및 Bennett 이 제시한 새로 운 모형안에서 제시된 값을 적용하여 가우스 단축결상식을 이용하였으며. 4 개의 굴절 떤을 갖는 모형안을 설계하였다, Gullstrand - Emsley 모형안 및 Bennett 이 제시한 새 보운 모형안과 비교했을 때, 한국인 정시안에서 각막전변의 곡률반경이 더 걸고 각박 의 꽁가굴절랙은 더 약하며, 수정체 전후변의 곡률반경이 더 짧고 두께가 더 앓으며 등까굴전력이 더 강하고, 안축장이 더 짧고 눈의 등가굴절력은 더 강한 것으로 나타 났다. 랜- 연구결과뜰 종합하면 저자들이 제시한 4 개의 굴절변을 갖는 한국인 모형안 은 G ullstrand - Emsley 모형안 및 Bennett 이 제시한 새로운 모형안과 차이가 있으며. 향후 한국인 정시안을 대변할 수 있는 모형안에 대한 연구가 펼요하다고 생각된다. Ocular components were measured by Orbscan Topography System. ultrasonic pachymetry, and A - scan ultrasonography under cycloplegic condition. Crystalline lens radii were calculated by Bennett’s method. Prior to obtain the data for modelling of schematic eye in Korean. we assessed the various methods for measurements of principal optical dimensions. Equivalent powers and principal points of schematic eye were calculated using the Gaussian paraxial equation. Refractive indices of intraocular media were used values in Gullstrand Emseley schematic eye. Korean four - surface schematic eye modelled by authors was significant different in Gullstrand - Emsley three - surface schernatic eye. It means that ernmetropic eyes of Korean were flatter anterior radius of cornea, steeper posterior radius of cornea, weaker equivalent corneal power. thinner lens thickness, weaker equivalent lens power, shorter axial length and stronger equivalent power of the eye than Gullstrand - Emsley schernatic eye.

        자동굴절검사 (Topcon KR -7100P) 의 신뢰도 및 정확도

        This study assessed the repeatability and accuracy of the Autorefractor(Topcon KR-7100P) in comparison to subjective refraction. Autorefraction were taken with a Test-Retest separation of at least 3 days. Repeatability and accuracy statistics and plots were generated using matrix representations of dioptric power. Coefficient of Repeatability(COR) of the autorefraction found to be 0.2713, 0.1105 and 0.3234 in the horizontal, torsional, and vertical components. Coefficient of accuracy(COA) of the autorefraction found to be 1.0632, 0.2397 and 0.2397 in the horizontal, torsional, and vertical components. Confidence regions for mean dioptric power of the Test - Retest Difference and Autorefraction - Subjective refraction difference were represented by cllipsoids in a three-dimensional space called h-space.

        서울 거주 어린이에서 안구성장이 근시진행에 미치는 영향

        서울 수색초둥학교 3-5학년 재화생 111 명(남 45명, 여 66명) 222안올 대상으로 굴 철이상도, 각막곡률 및 안구의 걸이롤 측정하여, 8개월 동안 변화 추이를 비교 분석 하였다. 조절마비 상태에서 자동굴절계로 측정한 둥가구면 굴철이상도의 펑균 변화도는 -0.32:3 ::t 0.346 D( -0.399 ::t 1.621 D에서 -0.722 ::t 1.719 D) 외 유의한 증가률 보였고 (t '" 13.985, p = 0.0000), 자동각막곡훌제로 측정한 각막굴철력의 평균 변화도는 -0.02 ::t 0.246 D(43.322 :t 1.241 D에 서 43.301 ::t 1.갱OD) 의 감소률 보였으며 (t = 1.239, p = 0.217), A -Scan Ultrasonography로 측정 한 안구의 길 이 는 O.잃6 :t 0.239 mm(23.231 ::t 0.875 mm 에서 23.467 ::t 0.940 mm) 의 유의 한 중가를 보였고 (t = • 14.730, p = 0.0000), 안축장/각 막곡률비는 0.029 ::t 0.035(2.980 ::t 0.1 14에서 3.009 ::t 0.12 1)의 유의한 증가훌 보였다 (t = --12.388, p = 0.0000). 안광학상수의 변화가 굴절상태의 변화에 미치는 영향을 분석한 결과, 안축장의 증 가가 근시도의 증가에 미치는 영향온 낮은 상관성올 보였고 (Y = 0.173 + -0.198 * X; r = -0.284, p < 0.(001), 각막곡률의 감소와 근시도의 증가 사이에는 상판성이 없었다 (Y = -0.031 + -0.033 * X ; r'" -0.046, p = 0.48). 또한 정시 및 왼시군(둥가구면 굴절이상도가 -O.50D 야하)과 근시군(둥가구면 굴 절이상도가 -O.50D 이상)의 근시진행도를 비교한 결과 정시 및 왼시군(평균 변화도 -0.234 ::t 0.330 D)보다 근시군(평균 변화도 -0.458 ::t 0.327 D) 에서 근시도가 더 크게 증가 하였고 (t'" -4.983, p = O.α)()()) , 안축장은 정시 및 원시군(평균 변화도 0.175 ::t 0.217 mm) 보다 근시군(평균 변화도 0.331 ::t 0.241 mm) 에서 더 크게 증가하였고 (t = 4.986, p = αoα삐), 각막곡률은 정시 및 원사군(평균 변화도 -0.013 ::t 0.254 D)보다 근시군(평균 변화도 -0.031 :t 0.233 D) 에서 더 크쩨 감소하였으나 유의성은 없었다(t = -0.513, p '" 0.608). 따라서 초등학생에서 신체의 성장과 함께 굴절상태가 근사쪽으로 진행하며 안축장 이 증가하는 반면 각막의 굴절력온 갑소하는 것으로 나타났고, 원시 및 정사군보다 근시군에서 근시도 및 안축장악 변화도가 더 큰 것으로 나타났다. To analyse refractive error changes and their correlation with growth of eye in Korean children, the cycloplegic autorefraction, autokeratometry and A - scan ultrasonography were performed over eight months on the healthy 111 children(45 boys and 66 girls) in third to fifth grade at Seoul Susak Primary School. The spherical equivalent of refractive error measured by the cycloplegic autorefraction was increased significantly of -0.323 ::1:: 0.346 D( -0.399 ::1:: 1.621 D to -0.722 ::1:: 1.719 D)(t = 13.985, p = 0.0000), the mean comeal power measured by the autokeratometry was decreased of -0.02 ::1:: 0.246 D(43.322 ::1:: 1.241 D to 43.301 ::1:: 1.240 D) (t = 1.239, p = 0.217), the axial length measured by the A -Scan Ultrasonography was increased significantly of 0.236 ::1:: 0.239 mm(23.231 ::1:: 0.875 mm to 23.467 ::1:: 0.940 mm)(t = -14.730, p = O.α)()O) and the AL/CR ratio was increased significantly of 0.029 ::1:: 0.035(2.980 ::1:: 0.114 to 3.009 ::L 0.121)(t = -12.388, p = 0.0000). In result of analysis for refractive eπor changes and their correlation with growth of eye, the myopia progression is caused by a statistically significant increase in axial length(Y = 0.173 + -0.198 * X ; r = -0.284, p < 0.0001), but showed the decrease of mean comeal power to the increase in myopia to be insignificant(Y = 0.031 + -0.033 * X ; r = -0.046, p = 0.48). In comparison of myopia progression between non-myopic group and myopic group, myopic group showed higher myopia progression and increase in axial length significantly than non-myopic group, the decrease in mean comeal power showed no significant difference between both groups. In Conclusion, there were showed the myopia progression, increase ofaxial length and decrease of mean comeal power in according to growth of body, and myopia progression and change ofaxial length in myopic group was higher than of non-myopic group in Korean children.

        We investigated the optical characteristics of a bi-convex lens using interferometric method. A Twyman-Green interferometer has been constructed for assessment of the lens. As a coherent light, a spatiallyfiltered and collimated He-Ne laser beam was employed. The fringe, formed by interference, was detected by a CCD camera with spatial resolution, 10m and displayed by a multi-synchronizing CRT monitor. The intensity distribution of the interferogram was digitized by a image card, and processed and analyzed by a personal computer. The focal point and power of a lens were determined with the accuracy comparable to the light wavelength. The astigmatic aberration and a fine mechanical defect on the surface of the lens under test were also observed.

      • 연속착용 콘택트렌즈에 의한 각막부종

        각막질환이 없고 콘택트렌즈를 착용하지 않고 있는 근시안을 대상으로 국내 제품의 3가지 소프트 콘택트렌즈를 착용시키고 9시간 동안 취침을 시킨 후 유발된 각막부종과 회복되는 시간을 측정하였다. 각막두께는 optical pachometer 를 사용하여 매번 10회씩 측정하여 평균값을 얻었다. 대조군의 overnight corneal swelling은 2.34%였으며, 각 렌즈에 대한 overnight corneal swelling은 A 렌즈 8.49%, B 렌즈 12.34% 및 C렌즈 11.19%로, A 렌즈와 나머지 렌즈 사이에 유의한 차이가 있었다(p<0.05). 또한 deswelling에 대한분석 결과대조군에서 기상 후 1시간이 지났을 때 잠자기 전의 각막두께로 회복되었으며, 콘택트렌즈 착용군에서는 A 렌즈를 착용했을 때에만 기상 후 3시간이 지난 후 잠자기 전의 수준으로 회복되었다. 콘택트렌즈의 Dk/L값과 overnight corneal swelling은 상관관계가 높은 것으로 나타났다. 이상의 결과로부터 본 연구에 사용된 국내 제품의 렌즈는 매일착용이 안전하고, 연속착용하는 경우 각막부종으로 인한 부작용을 예방하기 위한 정기검진이 필요할 것으로 생각된다. An overnight study was performed to measure the corneal swelling and deswelling induced by three soft contact lenses manufactured in Korea. All subjects were no lens wear or inexperienced to contact lens wear. The overnight corneal swelling of 5 subjects(l0 eyes) was measured with an optical pachometer. Corneal thichness measurements were taken just before eye closure and after nine hours of sleep. The overnight corneal swelling in control group was 2.34 %, and those in contact lens wearing groups were 8.49% for A lens, 12.34% for B lens, 11.19% for C lens, respectively, Overnight swelling for A lens was significantly less than that for the other lenses(p<0.05). Cornea] deswelling time to the level of a base line(pre-sleep) with no lens wear occurred within 1 hour in control group. Among the contact lens wearing groups, corneal deswelling occurred in 3 hours for A lens only, The average corneal swelling response for A and B lens was inversely proportional to the oxygen transmissibility(Dk/L) of the lenses. It was considered that the contact lenses in this study used were appropriate for daily wear mode, and careful evaluation should be need to prevent from adverse effects during extended wear.
