        Profile of the Staphylococcal Exotoxin Gene and its Relation with Canine Atopic Dermatitis

        Staphylococcus spp. is one of the most common bacteria isolated from the lesions of atopic dermatitis (AD) in humans, and their colonization is known to be a possible trigger factor of clinical signs. The aim of this study was to determine the prevalence of Staphylococcus spp. in canine AD (CAD), the types of exotoxins present, and their relation with the clinical severity of CAD. From 79 dogs with AD, 72 samples of Staphylococcus spp. were isolated (91.1%), and 65 (90.3%) were confirmed as Staphylococcus pseudintermedius. Concerning the profile of the exotoxin gene, 50 isolates (69.4%) contained at least one exotoxin gene, and 28 isolates (56%) were found to contain more than 2 different exotoxins. There was a significant difference in clinical severity with the presence of staphylococcal exotoxins (P = 0.028), whereas no correlation was found with the presence of Saphylococcus spp. (P = 0.598). The clinical severity of CAD increased only in relation to staphylococcal enterotoxin D (SED) and exfoliative toxins (P < 0.05). Some clinical evaluation criteria (erythema, papule/pustule) were correlated with the presence of the exotoxin gene (P < 0.05). This study showed that the high prevalence of Staphylococcus spp. and staphylococcal exotoxins in lesions from dogs with AD may be regarded as an important trigger factor for exacerbation of the clinical signs of CAD. 포도알구균은 아토피 피부염 환자의 피부병변에서 가장 많이 발견되는 세균으로, 이들 균집락의 정도는 사람 아토피 피부염의 임상증상 악화요인으로 알려져있다. 이에 본 연구에서는 개 아토피 피부염 환자의 피부에서 포도알 구균의 존재를 확인하고, 이들 균주가 생산하는 외독소 유형을 분석하여 개 아토피 피부염 환자의 임상증상과의 연관성을 알아보았다. 79마리의 개 아토피 피부염 환자 중 91.1%인 72마리에서 포도알구균이 검출되었으며, 이 중 62마리 에서 Staphylococcus pseudintermedius가 가장 높은 빈도로 확인되었다. 외독소 유전자 분석에서는 69.4%인 50마리에서 1가지 이상의 외독소 유전자를 포함하였고, 이들 중 56%인 28마리에서 2가지 이상의 다른 외독소 유전자를 가지고 있는 것으로 나타났다. 개 아토피 피부염 환자를 포도알구균의 존재 유무에 따라 분류하였을 때, 임상증상 점수의 차이에 통계적인 의미는 없었지만 (P = 0.598), 외독소 유무에 따라 임상증상 점수를 비교하였을 때는 의미있는 차이를 보였다 (P = 0.028). 또한 외독소 유형에 따라 분류하였을 때 외독소 중 SED와 exfoliative toxins에서 임상증상에 의미있는 차이를 보였으며 (P < 0.05), 외독소 유무에 따라 분류하였을 때는 임상증상 점수 중 특히 발적과 구진/농포에서 의미있는 차이를 보였다 (P < 0.05). 이와 같은 결과를 통해 개 아토피 피부염에서 포도알구균이 생산하는 외독소가 개 아토피 피부염의 증상악화와 관련이 있는 것으로 사료된다.

        Evaluation of the effect of a 0.0584% hydrocortisone aceponate spray on clinical signs and skin barrier function in dogs with atopic dermatitis

        The purpose of this study was to evaluate the effects of a topical spray containing 0.0584% hydrocortisone aceponate (HCA) on canine atopic dermatitis (CAD) and to evaluate the skin barrier function during the treatment of CAD. Twenty-one dogs that fulfilled the diagnostic criteria for CAD were included in this study. The HCA spray was applied once a day to the lesions of all dogs for 7 or 14 days. Clinical assessment was performed before (day 0) and after treatment (day 14), and clinical responses were correlated with changes in skin barrier function. CAD severity significantly decreased after 14 days of HCA treatment based on the lesion scores (p < 0.0001), which were determined using the CAD extent and severity index (CADESI-03) and pruritus scores (p < 0.0001) calculated using a pruritus visual analog scale. Transepidermal water loss, a biomarker of skin barrier function, was significantly reduced compared to baseline (day 0) measurements (p =0.0011). HCA spray was shown to be effective for significantly improving the condition of dogs suffering from CAD. This treatment also significantly improved cutaneous hydration and skin barrier function in the animals.

        Clinical use of a ceramide-based moisturizer for treating dogs with atopic dermatitis

        In humans, skin barrier dysfunction is thought to be responsible for enhanced penetration of allergens. Similar to conditions seen in humans, canine atopic dermatitis (CAD) is characterized by derangement of corneocytes and disorganization of intercellular lipids in the stratum corenum (SC) with decreased ceramide levels. This study was designed to evaluate the effects of a moisturizer containing ceramide on dogs with CAD. Dogs (n = 20, 3∼8 years old) with mild to moderate clinical signs were recruited and applied a moisturizer containing ceramide for 4 weeks. Transepidermal water loss (TEWL), skin hydration, pruritus index for canine atopic dermatitis (PICAD) scores, and canine atopic dermatitis extent and severity index (CADESI) scores of all dogs were evaluated. Skin samples from five dogs were also examined with transmission electron microscopy (TEM)using ruthenium tetroxide. TEWL, PICAD, and CADESI values decreased (p < 0.05) and skin hydration increased dramatically over time (p < 0.05). Electron micrographs showed that the skin barrier of all five dogs was partially restored (p < 0.05). In conclusion, these results demonstrated that moisturizer containing ceramide was effective for treating skin barrier dysfunction and CAD symptoms.

        두 마리 개에서의 flurbiprofen 중독 2례

        Two dogs were presented with melena, vomiting and depression after accidental swallowing of candy form of Strepsils (flurbiprofen), which is one of non-steroidal anti-inflammatory drugs used in human medicine for controlling a sore throat. These dogs had common signs of anemia induced by gastrointestinal ulceration and hemorrhage with azotemia and leukocytosis. The dogs were treated with blood transfusion, fluid therapy, proton-pump inhibitor, antiemetics, mucus protectant and antibiotic. Although most of clinical signs of two dogs were resolved, azotemic problem with evidence of renal injury have remained.

        Superficial Necrolytic Dermatitis (Hepatocutaneous Syndrome) in a Dog with History of Long-Term Phenobarbital Administration

        A dog with a history of long-term phenobarbital administration presented to Seoul National University Hospital for Animals with clinical signs of footpad ulceration and facial ulcerative dermatitis. Skin lesions (facial/footpad dermatitis), histopathologic signs (parakeratotic hyperkeratosis, edema and hyperplasia of the epidermis), and liver involvement (increased plasma liver enzymes and nodular lesions) suggested a diagnosis of superficial necrolytic dermatitis (SND). The patient died suddenly at home of unknown cause after 8 months medical treatment, but intravenous amino acid supplementation and supportive hepatic medication proved as effective for the skin conditions.

        Effect of Hydrocortisone Aceponate - Gentamicin - Miconazole Topical Otic Combination for Treating Canine Otitis Externa

        Fifty-four dogs with otitis externa were enrolled in the study for the Evaluation of efficacy of a Hydrocortisone aceponate - Gentamicin - Miconazole otic combination (Easotic^®, Virbac, Carros, France). Otitis externa patients were treated by Easotic^® once daily for 5 days and 2 days off treatment and evaluated on 7th day. If otitis externa persisted, additional Easotic^® treatment was administered once daily for 5 days and rested 2 days and reevaluated on 14th day. For the evaluation of efficacy of Easotic^®, eight clinical signs were scored on a severity scale and infectious agents from ear sample were also graded using semi-quantitative scale at each visit. Sum of clinical scores and cytological scores was defined as Global Clinical Score. When Easotic^® was applied once daily for 5 days,global clinical score was reduced 76.0%. When Easotic^® was administered for 10 days, during first 5 days administration,46.6% reduction of global clinical score was detected. During additional 5 days administration, 82.2% reduction of global clinical score was observed compared with Day 0. Any relevant adverse effect was not reported during the study in all cases. Thus, Easotic^® treatment once daily for 5 days and 10 days appears to be effective and safe treatment for canine otitis externa.
