        의료기관인증평가 관련 직무 스트레스가 직군간 이직의도에 미치는 영향

        Purpose: This study aims to investigate the factors influencing health personnel's occupational stress on turnover intention regarding healthcare accreditation. Methods: A survey was conducted from May 17 to May 31, 2021, among participants to examine occupational stress and turnover intention among health personnel working at a 188-bed specialized hospital in Seoul that is preparing for healthcare accreditation. Results: Occupational stress regarding healthcare accreditation was found to have a positive correlation with turnover intention (r=.698, <i>p</i> <.001), and influenced turnover intention the most, which explains the variance of 55.8% (F=29.015, <i>p</i> <.001). There were significant differences between occupational groups in job stress (F=13.292, <i>p</i><.001) and turnover intention (F=10.930, <i>p</i> <.001) in the healthcare accreditation. Conclusion: Occupational stress regarding healthcare accreditation is higher in nursing than in other occupations, indicating the need to lower the turnover intention of nurses by preparing a national institutional standard for nursing manpower and also put in place an appropriate compensation system for each hospital seeking accreditation.

        의료기관에 입원한 환자를 대상으로 인공지능 방역로봇으로부터 환경관리를 받아 본 경험

        Purpose: To determine whether artificial -intelligence quarantine robots can positively contribute to environmental management in medical institutions. We conducted in-depth interviews to understand the experiences of hospitalized patients receiving environmental management from such robots. Methods: Data were collected during March 20~May 20, 2023. individual in-depth interviews were conducted with 15 hospitalized patients, who could provide specific details of their experience of receiving environmental management from a quarantine robot. Results: Qualitative content analysis identified four themes, including “First encounter with an amazing and new quarantine robot,” “Efficiently performs non-face-to-face quarantine work in hospitals,” “Problems with quarantine robots,” and “Recommendations for improving the functionality of quarantine robots in medical institutions so quarantine robots and humans can coexist.” Conclusion: Currently, quarantine robots have limited functionality, and additional research is needed to identify various other functions in consideration of future infectious disease situations and the level of quarantine appropriate in a hospital.

        COVID-19 확진 간호사의 심리사회적 적응: 국내 최대 확진자 발생 기간을 중심으로

        Purpose: This study aimed to understand the impact of nurses’ psychosocial experiences in COVID-19 on their mental health and the psychosocial adjustment and growth of patients with COVID-19. Methods: This study used qualitative content analysis. Data were collected through individual in-depth interviews with 10 nurses with COVID-19 from February 24, 2022, to March 17, 2022. Results: Six themes were identified in the qualitative content analysis, including ‘Psychological burden after diagnosis’, ‘Acceptance of confirmed results’, ‘Difficulties caused by change in an unfamiliar environment’, ‘Confusion after returning to daily life’, ‘Mental state after a complete recovery’, ‘Incomplete prevention of infectious disease and control guidelines’. Conclusion: In the context of the COVID-19 pandemic, the physical and mental health of nurses can have a significant impact on the quality of medical care; therefore, an understanding of their psychosocial adjustment will help support nurses adjust better in future pandemic situations.

        의료기관에서 수술실의 정규적 환경관리와 자외선 소독 추가 환경관리의 효 과 평가

        Background: This study evaluated the effects of routine and additional environmental management with pulsed xenon ultraviolet (PXUV) disinfection in hospital operating rooms (ORs). Methods: The study was conducted in 10 ORs of a hospital specializing in joint surgery with 174 hospital beds. We selected four surfaces with high frequency and one surface with low frequency of contact with the hands of operating room medical staff. After regular environmental management and additional PXUV disinfection from March to April 2020, a total of 250 specimens were collected by performing 50 culture tests on each selected surface. Results: We found 4.8% (6/125) positive cultures after routine environmental management, but only 0.8% (1/125) after PXUV disinfection. Conclusion: Although we could not conclusively evaluate the difference between routine and additional environmental management, PXUV disinfection showed a tendency to reduce environmental contamination.

        점별 상호정보량을 이용한 LDA 토픽모형 추론

        The LDA topic model is a statistical model for extracting topics within a large collection of documents. This model is a kind of clustering technique that considers a document as a set of multiple words and extracts a set of words that occur simultaneously in a document at a high frequency. Through the words in the document set, we can figure out which issues are related to the topic and calculate how the topics are distributed in the document. It is recognized as a standard method in the topic model field because of high performance and convenience. The methods of traditional approximation inference for the LDA topic model are not consider the relative importance of word based on Bag-of-Word. We propose an approximation inference method using PMI weighted Metropolis-Hastings within Gibbs (PWMH-Gibbs) with the relative importance of word. PWMH-Gibbs improves the performance of the LDA topic model by removing stopwords and common words. The numerical results indicate that PWMH-Gibbs can outperform callapsed Gibbs and MH within Gibbs producing much more accurate estimates. LDA 토픽모형은 수많은 문서집합 내의 토픽을 추출하는 통계적 모형으로, 하나의 문서를 여러 단어들의 집합으로 간주하며 문서에서 높은 빈도로 동시에 발생하는 단어집합을 추출해주는 클러스터링기법의 일종이다. LDA 모형은 문서집합 내 단어들을 통해서 해당 토픽이 어떤 이슈인지 파악할 수 있으며 토픽들이 문서상에서 어떤 분포를 가지는지 계산할 수 있어 높은 성능과 편의성으로 토픽모형 분야에서 표준적인 방법으로 인식되고 있다. LDA 토픽모형추론을 위한 근사추론 연구는 Bag-of-Word 기반으로 각 단어의 가중치를 동등하게 보아 상대적 중요성을 고려하지 않는다. 본 연구는 단어의 상대적 중요성을 반영한 점별 상호정보량을 이용한 근사적인 사후분포를 이용하고, 사후분포로 부터 표본추출을 위해 깁스내 메트로폴리스-헤스팅스(PWMH-Gibbs) 알고리즘을 이용한 근사추론 방법을 제안하고자 한다. PWMH-Gibbs 알고리즘은 불용어 및 빈번하게 사용되는 단어를 제거하여 LDA 토픽모형의 성능을 향상시킨다. PWMH- Gibbs 알고리즘을 이용한 근사추론 방법은 실 자료 분석을 통해 제안된 방법이 더욱 정확한 결과를 제공하는 우수성을 보여 준다.

        중소병원 적정성 평가를 담당하는 간호사의 업무경험

        Purpose : This study aimed to comprehensively understand the work experience of the person in charge of the adequacy evaluation of small-and medium-sized hospitals and explore its meaning and essence in-depth. Methods : This was a descriptive qualitative study. The study participants were 10 nurses who understood the purpose of this study and participated voluntarily. Data collection was conducted via in-depth interviews in January 2021. The interviews were conducted 1-2 times per participant and lasted approximately 40-50 minutes per session. Data analysis was performed using a qualitative content analysis. Results : The work experience of the person in charge of the adequacy evaluation of small-and medium-sized hospitals included four themes: "difficulty in preparing for evaluation," "negative views on evaluation," "lack of a support system," and "positive improvements and changes due to an evaluation." Conclusion : Based on the above results, an education program and support system should be developed to strengthen the competence of nurses in charge of the adequacy evaluation of small- and medium-sized hospitals.

        LP-M, a Novel Butanol-Extracts Isolated from Liriope platyphylla, could Induce the Neuronal Cell Survival and Neuritic Outgrowth in Hippocampus of Mice through Akt/ERK Activation on NGF Signal Pathway

        맥문동(Liriope platyphylla)은 동양의학에서 오래 동안 항염증제, 당뇨 혹은 비만 치료제, 그리고 신경세포활성화 약제로 사용되어 왔다. 본 연구에서는 맥문동으로부터 NGF의 발현을 촉진하는 새로운 물질을 개발하고 이들의 작용기전을 밝히기 위하여 맥문동으로부터 13가지의 새로운 추출물을 확보하고 이들의 기능을 세포주와 마우스 실험을 통해 분석하였다. 그 결과, 상대적으로 세포독성이 낮고 NGF의 분비량이 많은 LP-E, LP-M, LP-M50, LP2E17PJ 등 4가지 추출물을 확보하였다. 또한 이들 중에서 LP2E17PJ를 제외한 3가지 추출물에 의해 분비된 NGF는 PC12세포의 neuritic outgrowth를 촉진하였다. 더불어 추출방법과 추출량의 측면에서 효과적인 LP-M을 C57BL/6 마우스에 2주간 투여하여 뇌조직에서 NGF mRNA의 발현을 확인하였다. LP-M은 오직 피질에서만 두 종류의 receptor 중에서 low affinity neceptor를 통한 억제신호를 전달하였으며, 해마에서는 유의적인 변화를 유도하지 못했다. 그러나 high affinity receptor를 통한 신호전달을 해마에서만 활성화 신호를 전달하였고 피질에서는 변화를 유도하지 못했다. 이러한 결과는 맥문동 추출물인 LP-M은 마우스 대뇌의 해마에서 high affinity receptor를 통한 신호전달을 통해 neuritic outgrowth를 촉진함을 확인하였다. Liriope platyphylla has been used in oriental medicine as an effective medical plant to improve symptoms of cough, sputum production, neurodegenerative disorders, obesity and diabetes for long time. In order to investigate the effects of novel extracts on nerve growth factors (NGF)-stimulated neuritic outgrowth, the alteration of NGF expression and NGF receptor signaling pathway were detected in neuroblastoma cells and C57BL/6 mice. Of a total of 13 novel extracts, 4 extracts (LP-E, LP-M, LP-M50, LP2E17PJ) showed high viability on MTT assay. Also, all of these extracts induced NGF secretion and NGF mRNA expression in neuroblastoma cells. However, the NGF-induced neuritic outgrowth from PC12 cells was only stimulated by LP-E, LP-M and LP-M50. Furthermore, we selected LP-M as a best candidate, based on method and amounts of extraction, in order to verify its effect in mice. C57BL/6 mice were treated with 50 mg/kg of LP-M for 2 weeks and the effects on NGF regulation were analyzed with various methods. The expression of NGF mRNA was significantly increased in LP-M treated mice compared to vehicle treated mice. Also, the signaling pathway of p75NTR was inhibited in the cortex by LP-M treatment, with no change in the hippocampus of brain. However, the signaling pathway of TrkA was dramatically activated in only hippocampus via LP-M treatment. Therefore, these results suggest that the novel four extracts of L. platyphylla may contribute to the regulation of NGF expression and secretion in neuronal cells. LP-M was especially considered to be an excellent candidate for a neurodegenerative disease-therapeutic drug.
