Background: Despite recent interest in rapid response systems (RRS), the current vital sign-based scoring systems have provided limited accuracy for predicting patient deterioration. The present study aimed to evaluate a more intuitive method for RRS nurses to judge patient acuity, and to compare the results with several early warning scores for predicting patient outcomes. Methods: We retrospectively reviewed admitted patients who were identified by the RRS between January 2015 and March 2016 at a 1,356-bed tertiary teaching hospital. During the study period, five RRS nurses assigned a Patient Acuity Rating (PAR) of 1-7 during their evaluation of the patient. We compared the abilities of the PAR and several early warning scores to predict a composite outcome (intensive care unit admission and/or death within 7 days) using receiver operating characteristic (ROC) curve analysis. Results: A total of 786 cases were identified using the RRS. Within 7 days of the triggering event, the rates of ICU admission, mortality, and the composite outcome were 21.9% (171 cases), 10.1% (79 cases), and 29.3% (230 cases), respectively. Compared to the various early warning scores, the PAR more accurately predicted all outcomes (areas under the ROC curve, mortality: 0.77, intensive care unit admission: 0.74, composite outcome: 0.78; all, P < 0.05). Conclusions: Well-trained RRS nurses` clinical judgment is simple and accurate for predicting patient outcomes. However, further studies are needed to compare the PAR and other objective prediction models.

Primary progressive aphasia (PPA) is classified into agrammatic, semantic, and logopenic variants, but differentialdiagnosis is often challenging. There is accumulating evidence that the neuropathology of logopenic PPA is distinct fromthat of the other types. We report herein a woman with logopenic PPA who was diagnosed by clinical, neuropsychological,and radiologic data during 2 years of follow-up. Interestingly, her cerebrospinal fluid biomarkers were found to besimilar to those found in Alzheimer’s disease, with a decreased concentration of Aβ42 and an increased concentration ofpTau181 (tTau).
Clinical Significance of Pleural Attachment and Indentation of Subsolid Nodule Lung Cancer

Purpose Lung cancers presenting as subsolid nodule commonly have peripheral location, making the cancer-pleura relationship noteworthy. We aimed to evaluate the effect of pleural attachment and/or indentation on visceral pleural invasion (VPI) and recurrence-free survival. Materials and Methods Patients who underwent curative resection of lung cancer as subsolid nodules from April 2007 to January 2016 were retrospectively evaluated. They were divided into four groups according to their relationship with the pleura. Clinical, radiographical, and pathological findings were analyzed. Results Among 404 patients with malignant subsolid nodule, 120 (29.7%) had neither pleural attachment nor indentation, 26 (6.4%) had attachment only, 117 (29.0%) had indentation only, and 141 (34.9%) had both. VPI was observed in nodules of 36 patients (8.9%), but absent in nonsolid nodules and in those without pleural attachment and/or indentation. Compared to subsolid nodules with concurrent pleural attachment and indentation, those with attachment only (odds ratio, 0.12; 95% confidence interval [CI], 0.02 to 0.98) and indentation only (odds ratio, 0.10; 95% CI, 0.03 to 0.31) revealed lower odds of VPI. On subgroup analysis, the size of the solid portion was associated with VPI among those with pleural attachment and indentation (p=0.021). Such high-risk features for VPI were associated with earlier lung cancer recurrence (adjusted hazard ratio, 3.31; 95% CI, 1.58 to 6.91). Conclusion Concurrent pleural attachment and indentation are risk factors for VPI, and the odds increase with larger solid portion in subsolid nodules. Considering the risk of recurrence, early surgical resection could be encouraged in these patients.

지난 2009년 도시철도 최초로 급행열차를 도입한 9호선의 성공적인 운영을 계기로 급행열차의 도입·운영 에 대한 다양한 시도와 연구가 수행되고 있으며, 국가철도망구축계획 및 도시철도망 구축계획에서도 급행열차 운 행에 대한 정책이 수립되고 있다. 본 연구에서는 현재 급행열차를 운행하고 있는 9호선의 열차운행조건에서 정차 역 개수를 서로 달리하는 쾌속열차와 특급열차를 운행하는 방법 및 효과에 대해 고찰한다. 운행 시나리오는 일반· 급행·쾌속의 3종 열차운행과 특급열차를 추가한 4종 열차운행으로 구성하였으며, 정차역은 쾌속 8개, 특급 4개 역 에 정차는 것으로 설계하였다. 열차성능시뮬레이션(TPS) 및 열차Diagram(DIA) 시뮬레이션을 통하여 효과를 분 석한 결과 동일한 편성수 및 운행비율로 운행할 경우 3종 열차 운행 시는 현재 대비 약 8.2% 시격 감소 효과가 나타났으며 4종 열차 운행 시는 20%의 시격 감소효과가 발생하는 것으로 나타났다. With the successful operation of Line No. 9 that introduced express trains for the first time in 2009, various attempts and studies have been carried out on the introduction and operation of express trains, and policies for express train operation have been established in the National Railroad Network Building Plan under the Ministry of Land, Infrastructure and Transport and the Seoul Metropolitan Government's plan to build an urban rail network. In this study, the effects and methods of operating heterogeneous trains at different stations as well as express train services on a same line are considered through train performance simulation and train diagram.

On April 5, 2019, 5G communication technology was commercialized in South Korea. It is expected that virtual environment technology will be used in various fields thanks to development of telecommunication technology. However, failure to address the problem of cyber motion sickness, a user side effect of virtual environment technology, will limit the activation and diversity of the technology's application areas. In previous studies, there were many ways to improve symptoms through visual environments and user movements. In this study, the results of a study were checked to see if the effects of symptom mitigation can be obtained by listening to a particular sound source of cyber motion sickness among users using a virtual environment.
