Background: The OR(operating room) should provide an optimum environment that is safe for the patient and the working personnel. The air ventilation system of OR has been studied to decrease the contamination of air. We investigated the flow and contamination of the air in OR at our hospital. Method: Cultures in OR were made and the number of bacteria and fungi were calculated. A two-dimensional model for the cross-section of an operation room is developed for the CFD (computational fluid dynamics) analysis. The characteristics of air flow in the empty operation room and in the occupied operation room are calculated by using a CFD program. Results: The current air ventilation system of our hospital does not deliver the clean air to the operating site efficiently in occupied OR. Conclusions: We suggested a new location of air ventilation system which improves air venting with little increase of the cost of equipment.

우발성 전척추 마취시 발생한 동공부동 1례 보고

There have been no case reports of anisocoria from total spinal anesthesia. We have 1 case of total spinal anesthesia associated with anisocoria and unconsciousness. A 68 year old man with left femur neck fracture was scheduled for operation under epidural anesthesia. A lumbar tapping for epidural anesthesia was performed in a right later position at a level between L4-5, using a 18 gauge Tuohy needle. The epidural space was identified by using the "Loss of Resistance" technique with air. Epidural catheter was inserted and 10ml. of 2% lidocaine & 10ml. of 0.5% bupivacaine injected through the catheter. Shortly after the epidural injection of local anesthetics, unconsciousness, hypotension & bradycardia were developed. So intubation was done and vasopressorss were given to him. After that, blood pressure was severely evevated and we noticed anisocoria. So CT of patient's brain was performed and some air shadow was seen in the basal cisternal space(pneumocephalus). We concluded it may be followed by total spinal block. After operation was finished, he recovered satisfactory without any sequelae.

요하지통 환자에서 두 개의 캐눌라를 이용한 박동성 고주파 신경근 응고술의 임상 연구

Dorsal root ganglion(DRG) block by the local anesthetics and steroids which has done classically in low back pain(LBP) patients with radiculopathy has high incidence of recurrence rate and side effects of steroids. Recently a new technique of pulsed radiofrequency thermocoagulation(P-RFTC) was introduced and substituted for it because of the benefits such as relatively low recurrence rate, low risk of nerve injury and comfort during the procedure. We experienced 15 patients who had LBP with radiculopathy and no or little response to epidural steroid injection. When we performed DR ganglionotomy by P- RFTC under the C-arm guide in these patients, we tried to approach the exact DR ganglion using tow cannulas - the one for obtaining patient's subjective symptoms and injection of contrast dye, the other for making a RF lesioning after identifying the impedance of sensory and motor stimulation. In conclusion, DR ganglionotomy by P- RFTC using two cannulas in LBP patients is an easy and safe procedure with satisfactory results.

The study was undertaken to search for a new agent in natural crude plants which might ameliorate cardiovascular diseases. Extracts of the leaves of Diospyros kaki Thunb have been used as folk remedies of several diseases, including hypertension and disorders of peripheral vascular system. But any study on the pharmacological action of this plant has not yet been carried out and its active ingredients have not been identified. In this study, we isolated white crystal from the chloroform-soluble fraction of this plant. This white crystal was indentified as dimethyl-p-phthalic acid (DPP, ??). We examined the pharmacological actions of DPP: effects on the rat electrocardiogram, the isolated rat heart and the contractile force of right ventricular muscle induced by electrical stimulation. The observed results are as follows: 1) DPP increased the contractile force of isolated rat heart. 2) DPP increased the contractile-force of the right ventricular muscle induced by electrical stimulation. 3) DPP increased heart rate and shortened the QT interval in the rat electrocardiogram.

허혈- 재관류에 의해 유도된 흰쥐 간손상에 대한 Trithioformaldehyde(TTFA)의 효과

Trithioformaldehyde(TTFA) was synthetized from formalin and hydrogen sulfide in our Lab. TTFA is a cyclic aliphatic compound containing three sulfur atoms. In our previous studies, TTFA exerted an antifungal effects, radiation protective effect in mice and protective effect on acute CCl₄hepatotoxicity. Ischemia causes functional and structural damages to liver cells. Reperfusion is essential for the salvage of ischemic state. But reperfusion itself is also thought to be accompanied by its won component of injury. Ischemia of the left and median lobes was induced by ligation of left protal vein and hepatic artery for 15 min. 60 min after reperfusion experiments were initiated. In ischemia-reperfusion on liver, serum GOT and GPT activities, the level of MDA by a colorimetric reaction with thiobarbituric acid in the liver homogenate were significantly increased. In addition, levels of cytochrome P-450 decreased in microsomal fraction during ischemia-reperfusion. When rats were pretreated TTFA(200 ㎎/㎏ p.o. daily for 4 days), TTFA suppressed the elevation of serum GOT and GPT, MDA level and the decrease in cytochrome P-450 content. The suppressive effect of TTFA was similar with that of vitamin E and glycyrrhizin. The oxygen-derived free radicals play a major role in ischemia-reperfusion injury in the liver. TTFA exerts its beneficial effect in liver ischemia-reperfusion injury. Also, in ischemia-reperfusion injury the protective effects of vitamin E and TTFA were similar. Therefore, anti-oxidative effect of TTFA is considered a probable suppressing mechanism on liver injury caused by ischemia-reperfusion.

전신마취하에서 폐쇄신경차단이 TUR-B 수술에 미치는 임상적 효과

A Clinical study was made 10 cases of obturator nerve block on bladder cancer surgery under General anesthesia. The patient's were divided into 2 Group. Group 1; Without 0.5% Bupivacaine injection for control Group 2 ; With 0.5% Bupivacaine injection. The following results were as follows. A Comparision with the control group, Group 2 ; In case of Group 1, most of the patient's occured moderate jumping of the leg from stimulation of the obturator nerve blcck by low-frequency currents induced from the diathermy. but In case of group 2, most of the patient's did not occur jumping of the leg. In conclusion, obturator nerve block under the General anesthesia was valuable method for prevention of complication on treatment of the bladder cancer.

중증 근무력증 환자의 마취 2예 보고

Myasthenia gravis is a chronic neuromuscular disease with the chief complaints of muscle weakness and generalized fatigue. It is' generally thought that myasthenia gravis is caused by an autoimmune response associated with thymus gland. Many difficult problem may be encountered in the anesthetic and the postoperative respiratory management. The authors anesthelized 2case of myasthenia gravis for thymectomy with general anesthesia. We report the case with review of literature relevant to anesthetic management in patients with myasthenia gravis.

대요근 근구차단술에 대한 임상경험

N/A Psoas compartment block has been used to provide anesthesia for orthopedic surgical procedures and postoperative pain control. Nowadays this block is advocated for the man- agement of pain in the thighs, legs and lumbar area for various reasons. To relieve pain in the thigh and leg, 2l times of block were performed and observed clinically in the l7 patients at our hospital. The results were excellent for the relief of pain in metastatic cancer patients. And this block is satisfactory in the thigh pain but not in the leg pain. So we recommend psoas compartment block as a useful and simple method for the relief of pain in metastatic can- cer pain in the thigh.
