비임균성 뇨도염 환자에서 Chlamydia trachomatis 검출방법에 관한 연구 : 배양법 , 효소면역법 및 직접면역형광법의 비교 관찰

Chlamydia trachomatis is one of the most common cause in non-gonococcal urethritis. There are several diagnostic methods for Chlamyd.ia tra.chomatis; culture method using McCoy cell, enzyme immunoassay and direct immunofluorescence staining etc. Ye have studied a sensitivities of culture, chlamydiaxyme and direct immunofluorescence staining(DIF). 85 male patients previously conformed to non-gonococcal urethritis have been selected in this study. Three samples were concurrently collected in the same patient. First sample was used to inoculation in McCoy cell, 2nd sample was used to Chlamydiazyme test and 3rd sample was used to direct immunofluorescence staining method. The results are following. 1. All culture, Chlamydiazyme and DIF positive cases are 15/85(17.7%) . 2. Culture and Chlamydiazyme positive but DIF negative cases absent. 3. Culture and DIF positive, but Chlamydiazyme negative cases are 2/85(2.4%). 4. Chlamydiazyme and DIF positive, but culture negative cases are 9/85(10.6%). 5. Culture positive, but Chlarnydiazyme and DIF negative cases are 6/85(7.1%). 6. Chlamydiazyme positive, but culture and DIF negative cases are 7/85(8.2%). 7. DIF positive, but culture and Chlamydiazyme negative cases are 3/85(3.5%). 8. All culture, Chlamydiazyrne and DIF negative cases are 43/85(50.1%) . In summarized, any one positive cases of culture, Chlamydiazymc and DIF are 42/85(49.9%) .

The efficacy of Thayer-Martin media (abbreviated TM media) recently introduced by the government, is a great concern to us. The results of culture on TM media are compared with the results obtained from Gram stained filrn. The subjects were 235 male patients with urethral discharge who visited the VD clinic of Choong-ku Puhlic Health Center in Seoul during 3 months period from 21th May to 20th August 1979. CDC recommendations are closely followed for Gram staining as well as for cultural identification. Among 151 positives with Gram staining, 142 were also culture positive (94.0%) while 8 were negative(5.2%) indicating reasonably good correlation. One patient was culturally positive(0. 8%) while negative by Gram staining. TM media supplied by Seoul Metropolitian Institute of Public Health which was used for this study is considered to be adequate.

Forty-two volunteers with recurrent herpes simplex virus infections were entolled in a clinical trial to evaluate the effectiveness of topical Ginseng saponin ointment. Ten percent Ginseng saponin in a water-micible ointment base was applied. 3 times daily until crust formation on 42 patients. As control hydrophilic ointment was applied 3 times daily on 10 patients. Author compared the length of time required for the herpetic vesicles to form crust in these two groups. The time required in the group treated with Ginseng saponin for crust formation was 21.9+10.29 houra. In the control group it was 58.2+23.64 hours(P$lt;0.01). There was no untoward effect to Ginseng saponin ointment. The Ginseng saponin ointment seems to be one of the best topieal antiviral agents in the treatment of recurrent herpes simplex virus infections.

We reported previously that, combined with l. 0 gm of probenecid, sodium penicillin G. 4, 0 m.u.i.m. was slightly less effective than procaine penicillin G. 4. 8 m.u. i.m. in the treatment of uncomplicated male gonorrhea. We now present the effect of double dose of sodium penicillin G. in the treatment of uncomplicated gonorrhea. We now present the effect of double dose of sodium penicillin G. in the treatment of uncomplicated gonorrhea. The subjects were 262 male patients with uncomplicated gonoccccal urethritis at the VD clinic of Choong Ku Public Health Center in Seoul during 5 months period from January to May 198l. The following criteria were used to identify gonococcal infection - Gram negative intracellular diplococci in urethral smear and/or oxidase positive typical colony formation on Thayer-Martin media composed of Gram negative diplococci. They were given 4.0 m.u. sodium penicillin G. i.m. proceeded 15 to 30 min. by 1. 0 gm. probenecid p.o.. Four hours later, same amount of sodium peniciIlin G. i.m. was repeated. Concornitant eomparison of procaine penicillin G. was not done because of recent unavailability of the drug in Korea. To avoid pain, the drug is dissolved in 6 ml of normal saline plus 2 ml of 2% lidacaine. One hundred eighty five patients were able to be followed 3 to 7 days after the treatment. There were .24 (13%) failures and 17 (10. 6%) postgonococcal urethritis. During this study, l4 strains of PPNG (Penicillinase Producing Keisseria Gonarrhoeae) were detected with chromogenic cephalosporin slide method. There were 10 (5. 8%) failures among 171 patients with urethritis caused by non-PPNG. It is suggested that double dose of sodium penicillin G. is effective in the treatment of urethritis caused by non-PPNG. It is also suggested that beta-lactamase production should be tested before the initiation of treatment.
