3D 프린팅을 활용한 스토마(Stoma) 주얼리 제작 연구

현대 장신구는 4차 산업혁명을 거치며 3D 프린팅의 개발 및 발전과 더불어 디자인 개념과 기능, 착용 범위가 확장하고 있으며, 나아가 의학 분야와도 융합하고 있다. 본 연구는 장신구가 의학과의 결합을 통하여 단지 미적 가치 추구뿐만 아니라, 착용자에게 도움을 주는 기능 또한 지닐 수 있다는 가능성을 탐지함에서시작되었다. 후두제거수술(후두전절제술)을 받은 후두암 환자의 쇄골 중앙부에는 호흡을 위해 폐로 연결되는 영구기관공이 만들어지는데, 이는 기존 비강의 기능을 대신한다. HME(Heat and Moisture Exchanger)는 영구기관공을 보호하는 동시에 비강의 역할을 일부 대신하고 발성에 도움을 주는 의학 기구이다. 본연구의 목적은 후두전절제술 후 HME를 착용할 경우 장신구 착용에서 소외되는 사람을 위하는 동시에 의학기구와 문제없이 결합하는 장신구를 디자인하고 제작하는 것이다. 수공예적 장신구 제작 방식은 정확한 사이즈와 맞춤형 결합 구조를 구현하기에는 한계가 있다. 따라서 본 논문은 3D 프린팅을 활용하여 HME 와 결합하는 스토마 주얼리 디자인과 제작 과정을 이행하였다. 연구 결과 및 내용은 다음과 같다. 첫째, 후두암과 후두전절제술 후의 문제점을 파악하고 의학기구인 HME의 필요성과 특징을 살펴본다. 둘째, 장신구 제작에 적합한 3D 프린터와 그 활용 방법을 조사하고 스토마 주얼리 디자인 프로세스를 계획한다. 셋째, 스토마 주얼리의 특징과 추가적인 발전 방안을 모색한다. 이러한 연구 결과를 바탕으로 본 연구는 과거 의학기구와의 결합 시 난해하고 불가능했던 구조와 맞춤형을 요구하는 디자인을 적절히 구현하였다. 이로써 장신구의 기능과 활용 범위 확대에 도움이 되기를 기대한다. When it comes to contemporary jewelry, its design concept, function, and wearing range have been expanded along with the development of 3D printing through the 4th industrial revolution. Furthermore, it has convergence with medical field. This study began to be conducted in finding the possibility that jewelry can have the function to help out wearers beyond the pursuit of aesthetic value, through the combination with medical science. In the center of the collarbone of laryngeal cancer patient who had laryngofissure (total laryngectomy), permanent tracheostoma connecting to the lung for respiration is generated to replace the function of the nasal cavity. Heat and Moisture Exchanger (HME) is a medical device that not only protects the permanent tracheostoma but serves as the nasal cavity and helps out vocalization. The purpose of this study is to design and make the jewelry that can connect to the HME without any problem if a patient wears HME after the total laryngectomy. Making jewelry in a handicraft way has limitations to accurate size and customized combination structure. Therefore, this study utilized 3D printing to design and make the stoma jewelry connecting with HME. The study contents and results are as follows. First, the problems after the total laryngectomy performed due to laryngeal cancel are analyzed, and the necessities and features of the medical device HME are looked into. Second, the 3D printer and its application method for making the jewelry are analyzed and the stoma jewelry design process is planned. Third, the characteristics of the stoma jewelry and any additional development plan are prepared. Based on the study results, this study implemented the properly customized design that meets the requirements for the structural difficulties that occurred in connection with a medical device. The results of this study are expected to help to expand the function and application scope of jewelry.

This paper intends to examine why the financial cooperation is prominently advanced and the trade collaboration is stunted in the ASEAN+3 region. Three factors are presented in order to illuminate the main question: institutional frameworks, decision-making of regional leaders and external challenges. Institutional frameworks can be including regional organizations, institutions or multilateral agreements which have primary purposes on economic cooperation since the WTO system was built. In the ASEAN+3 region, the fact that the ASEAN+3 summit became a regularized annual meeting has consolidated the institutional framework in the region. Although the attempt to build the East Asia Economic Caucus was failed, that played a role in the blueprint of ASEAN+3 summit. The legacy has constantly continued as Chiang Mai Initiative Multilateralization. Generally, regional leaders seek the national economic profit to take economic risk aversion. No matter how proper the intention is, it is hard to achieve the regional economic integration if the influence of states and leaders is not considered. It is notable that the ASEAN+3 summit was successfully settled after China, Japan and South Korea positively participated in the meeting. The Asian financial crisis in the late 1990s is a representative economic external challenge on the region. After the crisis, the regional economy was bound together into a financial cooperation, and the regional liquidity fund, named Chiang Mai Initiative, was created as a starting point of regional economic integration. Whereas financial integration developed rapidly in the ASEAN+3 region, there was no remarkable momentum to stimulate the regional trade cooperation. Those three factors does not affect the regional economic integration respectively. Institutional frameworks were based on the economic integration, and decision-making of regional leaders gave the direction of the integration: finance or trade. The external challenge of the Asian financial crisis provides Asian countries with the most significant stimulation, but the importance of the external challenge cannot lessen the influence of two factors in economic integration.
