많은 무치악 환자들은 부적합한 의치의 장기간 사용으로 인해 불규칙한 하악 운동을 하는 경우가 빈번하다. 총의치 제작 시 기능과 심미성을 만족하기 위해서는 정확한 하악 운동과 중심위를 채득하는 것이 중요하다. 본 증례는 불규칙한 하악 운동으로 중심위 채득이 어려운 환자를 flat occlusal table을 가진 treatment denture를 이용하여 총의치 수복하였기에 보고하는 바이다. Long-term use of ill-fitting dentures may cause irregular movements of the mandible. In such cases, satisfactory outcomes both in terms of function and esthetics could be achieved by accurate registration of mandibular movement and centric relation when fabricating new dentures. In this case, treatment dentures with flat occlusal tables were used to register centric relation and mandibular movements of a patient with erratic mandibular movements.
구치부 보철 수복 공간이 부족한 환자에서 이중 스캔을 이용한 구강회복 증례

구치부 지지 상실은 대합치의 정출, 잔존 치아의 교모 등으로 교합평면의 부조화와 보철수복공간 부족을 야기할 수 있다. 이러한 환자는 수직교합고경 분석이 선행되어야 하며, 수직교합고경의 증가가 필요한 경우 적절한 임시 수복물 제작을 통해 기능적, 심미적 측면을 평가해야 한다. 최종 보철물 제작에 임시 수복물의 정보를 반영하는 방법으로 이중 스캔을 이용할 수 있다. 본 증례는 구치부 상실 및 대합치의 정출로 보철수복공간이 부족하나 수직교합고경의 감소가 미미한 환자에서 이중 스캔을 이용하여 수복하였다. Loss of posterior support may cause extrusion of antagonistic teeth and excessive wear of remaining teeth which often leads to the destruction of the occlusal plane. In such cases, it is critical to verify the need to increase vertical dimension of occlusion (VDO). Should you increase VDO, provisionalization is crucial in evaluating function and esthetics. Double scanning technique is a useful method when fabricating definitive restoration that mimic provisional restoration. In this case, a patient with apparently no loss of VDO and insufficient interocclusal space for dental materials due to loss of posterior support and extrusion of antagonistic teeth was rehabilitated using double scanning technique.
한국 주니어 테니스 선수의 근골격계 통증에 대한 설문 조사

The aim of this survey was to analyze the characteristics of musculoskeletal pain of Korean junior tennis players and its effect on games and psychology of players. Surveys were administered to 30 healthy male junior tennis players who took part in a 50th national junior tennis meet in 2015. We analyzed the incidence, location, cause and preferred prevention or treatment method of musculoskeletal pain, the correlation between training time and musculoskeletal pain as well as the effect of musculoskeletal pain on games and psychology of players. Overall, 22 out of 30 (73.3%) responded the experience of musculoskeletal pain. The incidence of lower extremity pain (16 out of 30, 53.3%) were higher than upper extremity pain (9 out of 30, 30%) and low back pain (8 out of 30, 26.7%). The long training hours per day (r=0.574, p=0.001) and old age (r=0.390, p=0.033) were correlated with intermittent back pain in univariate anlaysis. In multivariate ananlysis, the long training hours per day was a single risk factor of intermittent low back pain (p=0.038, odds ratio 10.01). 43.3% of players responded that the insufficent preventive conditioning program was thought to be the cause of musculoskeletal pain. The most preferred treatment or prevention method for musculoskeletal pain was rehabilitation (55.3%). Twelve players reported the negative affection of musculoskeletal pain to the performance in game. Six players experienced the frustration, 9 playersexperienced the loss of interest and 9 players experienced the emotional avoidance of games due to the musculoskeletal pain.

Spatial occurrences of pharmaceuticals and personal care products (PPCPs) were investigated via an application of multiresidue screening method using LC-HRMS in four different major rivers in South Korea. Based on the exposure, ecotoxicoloigcal data of detected PPCPs were collected and ecological risk assessment at screening level was performed. Sample pretreatment was conducted with solid phase extraction (SPE) using multilayer cartridge and instrumental analysis was performed using QExactive plus Orbitrap mass spectrometry. Of the 94 PPCPs monitored, 62 were detected and 52 were quantified at least once in all rivers. Monthly summed concentrations are higher in order of the Yeongsan, Nakdong (Geumho), Geum, and Han River and at the site downstream of the wastewater treatment plant (WWTP)` outfalls. Metformin, a type II diabetes drugs, had the highest concentration in 14 out 22 sites with a detection frequency of 100%, ranged from 505.5 ng/L (in Han river) to 18,200 ng/L (in Yeongsan River), followed by cetirizine metoprolol acid, valsartan, carbamazepine, and more varying with location. Results indicate that PPCP exposures in Korean surface water are similar in terms of detected PPCPs, however, their concentrations are highly variable regionally. Chronic toxicity data of No observed Effect Concentration (NOEC) was used for ecological risk assessment of PPCPs. If no data is available, ECOSAR was used for toxicity prediction. Further studies with monitoring will be conducted to identify the temporal variation of PPCP exposures, therefore, higher tier risk assessment considering location and time can be conducted.
