      • 간호학생의 임상실습 교육을 위한 임상지도자 활용 모델 적용 효과에 관한 연구

        This study examined effect of preceptorship on clinical education of 39 senior nursing students. Competency scores and satisfaction with preceptors scores were investigated. Competency scores of the students before the clinical education (pre-competency scores) were compared with the competency scores after the clinical education (post-competency scores), and satisfactory scores were also examined after the clinical education. Study results showed that the total post-competency scores were increased compared to the total pre-competency scores, and this increment was statistically significant. However, analyzing sub-domains of the competency scores indicated only the nursing-skill domain scores were increased while the scores of interpersonal domain and management domain were increased but statistically insignificant. Satisfaction with preceptors scores were also more than the average scores reflecting the students were satisfied with their preceptors generally. The students, However, grade low in the preceptors' intellectual ability while their professional and interpersonal ability were rather satisfied. This study indicated preceptors should make more efforts to teach interpersonal and management skill and increase their intellectual ability. This study recommended a systematic reward system for the preceptors to increase the motivation to teach students is needed.

        방문간호서비스 질 평가도구 개발

        본 연구는 2008년 7월 시행을 앞두고 있는 노인장기요양보험제도의 재가급여 중 방문간호의 지속적인 질 관리와 향상을 위하여 실시된 방문간호서비스 질 평가 도구 개발 연구이다. 본 연구의 진행은 노인간호 전문가 7인으로 이루어진 자문위원회 회의를 통하여 이루어졌다. 국내ㆍ외 노인 간호 관련 질 평가지표에 대한 문헌고찰과 자문회의 검토를 통하여 예비 질 평가 도구를 개발하였으며, 예비 질 평가도구에 대하여 50인의 전문가 집단을 구성하여 1차 내용 타당도 검사를 실시하였다. 예비 질 평가도구의 1차 내용타당도 검사결과를 수정ㆍ보완한 후 2차 내용타당도 검사를 실시하였으며, 개발된 방문간호서비스 질 평가도구를 이용하여 훈련된 평가자 4인이 전국 14개 방문간호기관을 직접 방문하여 시범평가를 실시하였다. 시범평가 실시 후 실제 평가도구 적용 시 평가기준을 분명하게 할 필요가 있다고 평가자들이 지적한 문항들에 대하여 자문회의를 통하여 질 평가 도구를 수정ㆍ보완하여 방문간호서비스 질 평가도구를 완성하였다. 최종적으로 개발된 질 평가도구는 구조, 과정, 결과로 구성되어 있으며, 총 20개 영역, 59개 항목, 88개 문항으로 이루어진 4점 척도의 도구이다. 20개 영역은 인사, 업무 공간, 물품확보 및 관리, 조직, 재정, 경영방안, 의사소통 체계, 보고체계, 정보관리, 비용청구, 자원연계, 효과, 방문간호 책임자 역량, 방문간호사 교육, 서비스 계획, 서비스 수행, 서비스 평가, 감염관리, 환경 및 안전대책, 자체평가로 구성되어 있다. 본 연구결과로 개발된 방문간호서비스 질 평가도구를 활용하여 향후 시범사업이 종료되고 본 사업이 진행될 때 체계적인 방문간호 질 관리 체계를 구축해 나갈 필요가 있다. The purpose of this study was designed to develop Home Care Quality Indicators(HCQIs) to evaluate and improve quality of 'Elderly Care Program' that has been conducted in 14 institutions in the 2nd demonstration project. For this study, the preliminary HCQIs were developed through literature reviews and analysis of existing quality indicators. An expert development panel reviewed established and new quality indicators twice for applicability to home care services for long-term care. The final HCQIs were confirmed and applied in fourteen home care service agencies to test feasibility. In results, the preliminary HCQIs had 19 domains, 68 categories and 99 indicators. After two content validity testing, the final HCQIs consisted of 20 domains, 59 categories and 88 indicators. They were applied in fourteen home care service agencies and it was revealed that all the indicators were applicable. The quality indicator set provides a comprehensive home care quality framework for long-term care future and will allow for comparative research. Provision of these evidence-based measures could improve patient quality of life and longevity.

        심장질환자의 심리사회적 요인 및 증상 경험간의 관계 : 6개월 추적조사

        Purpose: The purpose of this study was to identify the relationship between psychosocial factors and symptom experience of patients with cardiovascular disease over a 6-month follow-up period. Method: Baseline data for each of the 138 patients were collected. Eighteen patients were dropped for a final total of 120 patients in the present study. To monitor patients' symptom experience after discharge, each patient was interviewed by telephone, and interviewed directly with a questionnaire at each clinic visit. Results: Mean scores for hostility, anger, anxiety, and depression were 51.63, 22.87, 48.95, and 41.21, respectively. Mean scores for the level of symptom experience at discharge, 3 months and 6 months after discharge were 32.83, 24.79, and 26.70, respectively. There were significant differences in the level of hostility by gender, BMI, job, and monthly income. Also the differences in the level of anxiety and depression were statistically significant according to gender and existence of spouse. In the regression analysis, depression was identified as the significant factor associated with symptom experience at discharge, 3 months, and 6 months after discharge. Conclusion: This study presented baseline data to elucidate whether psychosocial factors were associated with the progression of cardiovascular diseases. Accordingly, it suggests that nurses need to take into consideration psychosocial factor as well as physiological factor in symptom management for patients with cardiovascular disease.

        This study was conducted to develop a home care nursing management system based on the validated and useful data base found through literature review. The contents and structure according to a development procedure for a computer system were as follows. 1. A data base on home care nursing patients was accumulated by putting data respectively in both steps and fields - from selection criteria. basic information. prescription. plan of home visits. to application of nursing process. 2. Accumulated data was classified and designed to search by basic information. drug/injection prescription. examination prescription, treatment prescription. supply. and a record of the nursing process. 3. Various forms of retrieval including graphs were elaborated in terms of diagnosis and intervention aspects.

      • 일 가정간호 교육기관 교육생의 임상수행능력 평가에 관한 연구

        This study was conducted to investigate the evaluation of students on a practical performance of the home health care and to provide data for practical education on the home care nurse institution. Research was performed from March to December. 2002. 40 Respondents participated in the evaluation four times. The data were analysed by using the SPSS/PC + version 10. 0. The results of this study were as followings: 1. The average age of the subjects was 32. Majority of the subjects were interested in home health care. Majority of the subjects were staff nurse(57.5%) and workers at general hospitals(50%). 2. According to the result of the comparison of practical performances. performance evaluations of 20 items were higher when they admitted special practice than when they admitted any other practice. There was an positive increase in the evaluation of health assessment. nursing documentation. explanation. coping with emergency status. coordination and self assertion. In addition to health assessment. CPR. oxygenation. education & consultation. developmental assessment of child. nursing documentation. mastering nursing practice and consideration of client were significantly different. 3. The subjects were useful for practical circumference where they are working at. From these research findings. the following suggestions have been drawn: it is needed to manage practical curriculum variously and to include not only clinical aspect but also organic or administrational aspect and to reflect on students' needs and to choose to be reasonable items and to develop instruments for evaluation capability of home care nurse.

        From 1990 to 1997 Home Care Education Programs have been offered at 11 Home Care Education Institutes. But there have been no revisions in the program. Especially in the clinical practicum, the Ministry of Health and Welfare proposed 248 hours as 'Family Nursing and Practice'. But each of institutes has developed their own program, and the clinical practicum is very different. Institutions for home care practice have been very limited and even now only 40 hospitals started home care in the second demonstration project. A few community-based institutions have also been offering home care services. This study was conducted to analyze home care clinical practicum offered at Y university, and plan for a revised home care clinical practicum. Y university clinical practicum was revised to include 'visits to community institutions', 'laboratory practice', 'hospital practice', 'discharge planning and home care practice', and 'home care specialty practice'. The results of the evaluation and plan for a revision are as follows: 'Visits to community institutions' is a practice that helps the students understand community resources which are available to home care nurses, and as an orientation to institutions for practice. 'Laboratory practice' is to used to improve nursing skills that are applicable to home care. Problems that the students identified in the laboratory practice were 'lack of opportunity for individual practice', and 'inadequate theoretical preparation for practice'. To address these problems the basic nursing skills laboratory was open and could be used freely by the home care students, and practice could be done after the theoretical lectures. 'Hospital practice' is a practicum in which the students apply nursing skills to patients and to obtain assessment skills for discharge planning. Using a preceptorship, five days for hospital practice should be offered. 'Discharge planning and home care practice' was done at Wonju Christian Hospital. This institute is too far away that this practice should be provided at different institutions as soon as it is possible to contract with home care institutions. Patients groups in different home care institutions are very different, so the 'Home care specialty practice' should be done after analyzing patient groups and choosing specialty areas. These areas are' care of patients with respiratory dysfunction', 'care of patients with neurologic dysfunction', 'care of cancer patients', 'care of patients on dialysis', and 'wound care'. This practice should be offered with a preceptorship, so preceptors, clinical directors, and students should meet for home visit orientation.
