간세포암이 의심되어 혈관조영술을 시행한 19명의 복강동맥조영술 39예와 상장간막동맥조영술 21예의 영상을 후향적으로 분석하여 간혈관조영술에서 간동맥색전술 시행전 촬영방법의 기준을 찾고자 하였다. 복강동맥조영술에서 조영제 주입 후 종양 혈관의 조영 시기 및 간문맥의 조영 시기와 상장간막동맥조형술에서 간문맥 조영 시기를 측정하였고 촬영한 영상 중 간동맥 색전술의 기준검사로서 적절한 영상을 선택하여 필름에 담고 이 영상수를 각 시기별로 구분하였다. 복강동맥조영술에서 종양은 조영제 주입후 3.2±0.8초에, 간문맥은 9.7±1.7초에, 상장간동맥조영술에서는 간문맥이 11±2.7초에 조영되기 시작하였다. 필름에 담은 영상수는 복강동맥조영술에서 간문맥이 조영되기 전에 2-7매, 평균 3.9매이었고 간문맥이 조영된 이후에 1-3매, 평균 1.6매이었으며 상장간동맥조영술에서는 간문맥이 조영되기 전에 2-5매, 평균 3.2매이었고, 간문맥이 조영된 이후에 1-3매, 평균 1.3매이었다. 간세포암이 의심되어 혈관조영술을 실시할 환자의 복강동맥조영술은 조영제 투입후 10초까지 2초에 1장씩, 이후 3초에 1장씩 촬영하고, 상장간동맥조영술에서는 3초에 한장씩 촬영하는 것이 적절하리라 판단된다. The hepatic angiograms(39 celiac and 21 superior mesenteric angiography) of 19 high risk patients of hepatocellular carcinoma were evaluated retrospectively to find out proper filming methods for the preliminary studies before transarterial chemoembolization. The time of appearance of tumor vessels and portal veins at celiac angiography and portal veins at superior mesenteric angiography was measured. From the preliminary studies the images necessary for transarterial chemoembolization were selected and counted. The tumor vessels and portal veiny were appeared at 3.2±0.8 and 9.7± 1.7seconds after contrast injection respectively at celiac angiography and portal veins at 11±2.7seconds at superior mesenteric angiography. The number of films taken before and after the visualization of portal vein were two to seven(mean, 3.9) and one to three(mean, 1.6) at celiac angiography and two to five(mean, 3.2) and one to three(mean, 1.3) at superior mesenteric angiography respectively. It may be proper to take one film on every two second before and on every three second after 10 seconds following contrast injection respectively at celiac angiography and also one every 3rd second at superior mesenteric angiography as a basic study before transarterial chemoembolizaion.
