¹³³Xenon을 利用한 皮膚瓣의 血流量測定에 關한 實驗的 硏究

Various tests to measure the circulation in skin flap have been proposed such as the histamine wheal test, the atropine test, the nicotinic acid test, plethysmography, thernography, and the fluorescein dye test. If a simple and accurate method could be developed to quantitatively determine the regional blood flow in the skin flap, such a method would have a great significance in plastic and reconstructive surgery. Measurment of blood flow in a tissue on the basis of the observation of the clearance rate of a radioactive isotope injected directly into that tissue has been developed since Kety's report. By contrast with other radioactive isotopes, ¹³³Xe diffuses freely across cell membranes, and therefore local blood flow can readily and accurately be calculated. A study was made to determine the local blood clearance rate of experimental flap of rabbits using local clearance method of ¹³³Xenon (¹³³Xe). A total of 40 rabbits, 3.0-4.0kgm were dividid into control, 1:4 and 1:6 skin flap groups. ¹³³Xenon disolved in sterile saline was injected intracutaneously in a concentration of about 100μCi/㎖, amuont 0.1㎖. The detector(gamma counter) was placed at a distance of 10cm from the skin surface. At various intervals, the clearance rate of ¹³³Xenon was checked by detector and blood sampling from femoral puncture. The following results were obtained, 1) ¹³³Xe clearance rates were the greatest in the control group, the next in the group of 1:4 skin flap and the lowest in the group of 1:6 skin flap. 1:4 skin flap and the lowest in the group of 1:6 skin flap. 2) The changes of radioactivity of ¹³³Xe in the control group were more rapidly decreased during 60minutes after injection of ¹³³Xe, thereafter slowly decreased. The changes of radioactivity of ¹³³Xe in the group of 1:4 skin flap were slowly decreased during 2 hours after injection of ¹³³Xe, thereafter revealed plateau in nature. The changes of radioactivity of ¹³³Xe in the group of 1:6 skin flap were slowly decreaseed during 60 minutes after injection of ¹³³Xe, rapidly decreasd between 60 minutes and 120 minutes, thereafter showed plateau in changes. 3) The degree of flap necrosis in the experimental groups of the skin flap was larger in the group of 1:6 skin flap than in the group of 1:4 skin flap.

This study showed following results. The time interval for maximal enhancement of meningioma was 120∼180 sec after injection of contrast material. All 12 cases showed good relationship between the tumor and surrounding vessels. A case of aneurysm associated with tumor was also well visualized. Three-dimensional volume rendering imaging of meningiomas obtained by using double contrast-enhancement technique was effective in the evaluation of relationship between the tumor and adjacent vascular structure. This technique was considered to be useful in making the preoperative plan.
